Loss Prevention Committee Minutes for March 18, 2010 Page 4 of 10 S-1 SCOTT SANTILLE 01/05/10 STREETS DEPARTMENT Description of Incident: Before moving the skid steer, Scott raised the bucket and the arm hit the front door - bending the frame of the door and-breaking the windshield wiper arm. Supervisor's Comments & Recommendations: Make sure door is closed before operating equipment. LPC Comments & Recommendations: Concur SR-1 NEAL HUNTER— UTILITIES 01/13/10 Description of Incident: While performing JULIE locates, locator slipped out of Neal's hand and fell onto a concrete bumper stop. Supervisor's Comments & Recommendations: Have a firmer grip on the locator. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for March 18, 2010 Page 5 of 10 P-1 JOSEPH SACOMANO — POLICE DEPT. 01/20/10 Description of Incident: Officer Sacomano was driving a marked police Squad #243 (Unit #2 on Crash Report) westbound on Nerge Road in the right lane of traffic when another vehicle (Unit#1) attempted to execute a left turn from southbound Charlela Lane onto eastbound Nerge Road, and collided with Ofc. Sacomano's vehicle. The intersection of Nerge and Charlela is a T intersection with a stop sign controlling the flow of traffic on southbound Charlela at Nerge. Unit#1 failed to yield the right of way at a stop intersection and was determined to be at fault in the crash. Supervisor's Comments & Recommendations: The fault driver failed to yield the right of way at a stop intersection. Driver needs public education. LPC Comments & Recommendations: Concur F-3 WILLIAM MARK SELLERS — FIRE DEPT. 01/21/10 Description of Incident: During a drill, the spring clip broke on the pager's belt clip holder. Sometime later during the course of running emergencies and getting in and out of the vehicle, employee lost the phone pager. Supervisor's Comments & Recommendations: None P-2 PAUL PETZOLD — POLICE DEPT. 01/22/10 Description of Incident: Ofc. Petzold advised that he noticed damage to the right front bumper. He said he doesn't know when or where the damage occurred. Based on the location of the damage, it appears that Ofc. Petzold hit a snow bank causing a dent in the bumper. Supervisor's Comments & Recommendations: Ofc. Petzold was counseled as to driving safely. He was advised that while turning or parking his vehicle, he should be sure that there is ample amount of space. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for March 18, 2010 Page 6 of 10 F4 10/02 - FIRE DEPARTMENT. 01/26/10 Description of Incident: Firefighter/Paramedic was at the patient's head end using the correct posture and positioning for his body to assist three other firefighters in lifting a patient from the floor to a stair chair, using proper lifting techniques. Due to the patient's positioning level, severe pain level, and posture for the injury, it became an awkward lift and there was no way to do different movements or add more personnel. As Firefighter/Paramedic was lifting, he felt a sharp pain in his left shoulder, and immediately reported such to Lt. Peterson as the patient was placed on the stair chair. Supervisor's Comments & Recommendations: The patient being attended to was not an overweight patient, just in a very awkward position with an injury that had postured itself in a very awkward position with a severe pain level. Crews had to do as much as possible with treatment and pain meds for the patient to minimize movement to prevent further injury and reduce the pain level. Also due to the patient's position, mechanical lifting aids provided could not be used. Remind all crews to use proper positioning as was done. LPC Comments & Recommendations: Concur E-1 RAY BAUER— ENGINEERING &COMM. DEV. 01/27/10 Description of Incident: Vehicle struck a large rock (located in overhand area just beyond the end of the parking space) while backing into parking space resulting in slight damage to rear bumper. Supervisor's Comments & Recommendations: Driver must be more aware of surrounding conditions. In this case the rock that was backed into would have been noticed if the driver surveyed the area more closely prior to backing up. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for March 18, 2010 Page 7 of 10 BS-1 BRIAN MISIAK- BUILDING SERVICES 01/28/10 Description of Incident: During a snow removal operation, the snow blower hit the light. Supervisor's Comments & Recommendations: Some type of shield or goggles might help to prevent this from happening. LPC Comments & Recommendations: Concur P=3 WILLIAM CALLAGHAN — POLICE DEPT. `02/ .9/10 Description of Incident: While at the scene of a house fire, Officer Callaghan was instructed to take a traffic control post at Idaho and Utah. Because there was fire apparatus parked in front of him, he placed his squad in reverse. Officer Callaghan advised that immediately after taking his foot off the brake he struck a vehicle parked (unoccupied) directly behind his Squad. It was subsequently determined that the parked vehicle was the Fire Chiefs village owned Squad. There was no damage observed on Squad #234, and only minor damage to the Fire Chiefs vehicle. Supervisor's Comments & Recommendations: The crash would have been avoided if Officer Callaghan had been more aware of his surroundings and used due caution while backing up. LPC Comments & Recommendations: Driver Refresher Course Loss Prevention Committee Minutes for March 18, 2010 Page 8 of 10 W-1 KEITH CONLEY_ PUBLIC WORKS - OPERATIONS 02/10/10 Description of Incident: Employee was distracted by a vehicle pulling into the parking lot and backed into the light pole. Employee notified his zone supervisor by radio. Zone supervisor failed to follow-up on the report, initially reported as an unknown Public Works vehicle/driver causing the damage. Supervisor's Comments & Recommendations: Without denying access to the parking lot until snow removal operations are completed, this type of accident is difficult to avoid. Blocking off the parking lot is not practical as employees and the public are accustomed to entering the parking lot at will. Plow driver's should maintain spatial awareness of potential obstructions nearby and if distracted, stop the vehicle as soon as it is safe, exit the vehicle (if safe to do so), and attempt to acquire awareness of the area and potential obstructions around them. LPC Comments & Recommendations: Concur F-5 .10/03_-"FIRE DEPARTMENT 02/19/10 Description of Incident: As we were responding to an ambulance call, employee was putting his pants and boots in the driver's compartment of Ambulance 121. He had put them in the compartment and a strap fell out as he was closing the door, he reached down to put the strap back in as the door closed. The spring closer on the door slammed the door closed and hit him in the side of the face. Supervisor's Comments & Recommendations: Slow down when performing tasks and concentrate on safety. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for March 18, 2010 Page 9 of 10 P-5 THOMAS DANAHY— POLICE.'DEPARTMENT 02/20/10 Description of Incident: While Officer Danahy was pulling on the exterior rear passenger door handle of Squad 254, the door handle broke off in his hand. He said he did not apply unnecessary force to the handle when this incident occurred and was simply opening the door to retrieve some equipment. Officer Danahy could not offer any explanation as to how this happened. Officer Torres witnessed this incident and stated that when Officer Danahy attempted to open the door, the handle broke off in his hand. Officer Torres also stated that he was on the opposite side of the vehicle when it happened. Supervisor's Comments & Recommendations: No recommendations made due to the fact that this incident appears to be accidental. LPC Comments & Recommendations: Concur W-2 "16/04'-WATER &'SEWER 02/23/10 Description of Incident: While hydro-excavating for utility locations, a foreign object shot up underneath employee's prescription eyeglasses causing irritation. Supervisor's Comments & Recommendations: Whenever using high pressure air and/or water, caution should be taken to make sure eyes are completely shielded from flying foreign objects. LPC Comments & Recommendations: When employee is hydro-excavating (using high pressure air and/or water), safety goggles must be worn. Loss Prevention Committee Minutes for March 18, 2010 Page 10 of 10 P-4 10/05 — POLICE DEPARTMENT 02/23/10 Description of Incident: As the Officer was removing a deceased subject from a transport van, the wheels on the gurney did not lock. When the full weight of the body came off the van floor it dropped to the lowest position on the gurney. Supervisor's Comments & Recommendations: Training has been completed by all sworn officers on the correct use of this equipment within the past 18 months. However, this equipment is rarely used and I believe having the instructions attached to the cart would allow officers to refresh their memory of its proper operation. Additionally, if officers are not comfortable with the operation of equipment and directions are absent, it is incumbent on them to seek assistance. LPC Comments & Recommendations: Concur E-2 MICHAEL DELFAVERO - ENGINEERING 03/04/10 Description of Incident: While parking a Village vehicle at a construction site, the vehicle hit snow bank. Supervisor's Comments & Recommendations: Employee should exercise more care when parking a Village vehicle. LPC Comments & Recommendations: Driver Refresher Course
