Present: Jack Gray, Chairman Diane Hunter, Finance Department Secretary Scott Miller, Deputy Fire Chief John Nowacki, Commander-Police Department David Preissig, Deputy Director of Infrastructure Karen Turner, Loss Prevention Secretary The minutes were approved for the meeting of March 18, 2010. Old Business: • Deputy Chief Scott Miller informed the LPC that the large Elk Grove Village resident j that needed a portable lift, is now deceased. New Business: i Jack Gray stated that we need to schedule Driver Refresher courses soon, as there are 42 Police employees, and 25 employees from other departments on the current list. Sgt. While making the turn, the right side of the truck brushed the snow bank at the curb line. There must have been ice under the snow, because the snow pile broke the plastic running board and put a minor crease in the door. Tony states that because there was a car turning left onto Plum Grove and the snow was not plowed all the way to the curb, it was tight getting the truck through the turn. Tony did not realize there was any damage until he got back to the shop and inspected the truck. Supervisor's Comments & Recommendations: Warn the employee to avoid driving through an unsafe situation or condition. LPC Comments & Recommendations: Concur P=6 POLICE DEPARTMENT 110/06". :03/12/10 Description of Incident: Employee was in training at the Police Training Institute. During the training he went into a double knee kneeling position and felt pain in his right knee. He immediately reported the injury to the Tactical Training Center manager, Scott Rhoten. Although sore, employee continued his training, however as the day went on it appeared to get worse. Supervisor's Comments & Recommendations: This is an incident that is unavoidable; however when the injury occurred he should have stopped the training. This can prevent further injury. Description of Incident: Greg pulled retired Police Squad #224 into the Fleet shop to begin a recycle conversion of this vehicle to go into another department. While driving it in, he noticed that the front windshield had a crack in it. This vehicle has been stored outside in the Fleet parking lot for about the last 30 days. Supervisor's Comments & Recommendations: None LPC Comments & Recommendations: Concur -F-6: FIRE..DEPARTMENT:' 10%07 03/25/10 Description of Incident: While removing the stretcher from the rear of the ambulance with a patient on it, employee caught his left forearm in the load wheel assembly that pinched his skin and caused a slight abrasion and hematoma to the medial forearm. Supervisor's Comments & Recommendations: Employee has been made aware to watch where his arms and hands are when removing the stretcher from the ambulance. LPC Comments & Recommendations: Concur P-7 POLICE DEPARTMENT —10/08 04/03/10 Description of Incident: The Officer, while engaged in a traffic stop with emergency lights activated, had his vehicle struck in the rear by another vehicle. The Officer received back pain and was transported to ABMC for medical examination. Supervisor's Comments & Recommendations: The Officer did not cause the accident. The Officer made a proper traffic stop and had the offending vehicle stopped in the curb lane. A second vehicle arrived minutes later and drove directly into the rear of the police vehicle which had the emergency lights activated. This accident was unavoidable by the Officer. TED MALEK — POLICE DEPT. 04/03/10 Description of Incident: Officer-Malek had stopped a vehicle for a violation. While sitting in his vehicle checking the driver's license status, he had the emergency lights activated. His vehicle was then struck in the rear by another vehicle. Supervisor's Comments & Recommendations: The accident could not have been avoided by the Officer. Officer Malek made a traffic stop to the curb lane. Officer Malek had all his emergency lights activated. The offending vehicle failed to change lanes and struck the rear of Squad 232. LPC Comments & Recommendations: Concur W-3 JOSHUA'FARMER UTILITIESIWATER: 04/09/10 Description of Incident: While excavating to replace a 6" water main valve for the Corrinithia line, unmarked gas service was severed by the back-hoe operator. Supervisor's Comments & Recommendations: None LPC Comments & Recommendations: Concur F-7 LT. MICHAEL SHARP — FIRE DEPT. 04/09/10 Description of Incident: Lost/missing equipment. Supervisor's Comments & Recommendations: Upon noticing missing or lost equipment, personnel should inform their immediate supervisor and/or supervisors. They need to further investigate, report it to the proper authority, and complete a Supervisor Investigative Report if necessary. LPC Comments & Recommendations: The equipment was eventually returned. I Loss Prevention Committee Minutes for May 20, 2010 Page 6of8 P-9 OFC.*MICHAEL MCINTYRE — POLICE DEPT. 04/13/10 Description of Incident: Officer McIntyre was-qualifying on the range with the AR-15. The range officer was having him shoot from a barricaded position using the shooting stall as the barricade. The muzzle flash singed/burned a portion of the carpet, approximately 3" x 2" covering the side of the shooting stall. Supervisor's Comments & Recommendations: At no time should a weapon be fired when the muzzle is in close proximity to the carpeted section of the shooting stall. LPC Comments & Recommendations: Concur -P-1 0 SGT. KEITH WILCOX — POLICEDEPT. 04/16/10 (, eported.BY)- Description of Incident: During routine service, Fleet Services noticed sidewall damage to the front left tire of Squad 237. The damage is not noticeable during normal driver inspection. The damage was observed after the vehicle was lifted for other service. Apparently the tire struck an object in the roadway causing the damage on an j unknown date and time. Supervisor's Comments & Recommendations: The location and time of occurrence is not known. Damage was discovered by Fleet Services after the vehicle was lifted for service. The damaged tire may have been there for.several weeks. Drivers are to be i reminded that if they suspect that a tire may have hit an object in the roadway they need to thoroughly inspect the tire. LPC Comments & Recommendations: Concur Loss Prevention Committee Minutes for May 20, 2010 Page 7 of 8 J .&.2 ROBERT COOKE - PUBLIC WORKS-STREET 04/21/10 Description of Incident: As the operator was attempting to go around a parked vehicle, the strobe/brake light housing on the dump body came into contact with the right rear dump body of Unit 816. Supervisor's Comments & Recommendations: Caution all operators to always be aware of surroundings especially in close quarters. LPC Comments & Recommendations: Driver Refresher Course P-11 OFC. TIM°KLEINFELDT- POLICE:DEPT. .".,04/23/10 Description of Incident: Officer Kleinfeldt was-driving southbound in the parking lot passing a box truck that was slowing down along the grassy area on the east side of the lot. The driver of the box truck made an abrupt right turn into the left front corner of the bumper of Squad 244. Supervisor's Comments & Recommendations: Officer Kleinfeldt should exercise more caution while driving in parking lots. LPC Comments & Recommendations: Driver Refresher Course FS-3 PUBLIC WORKS-FLEET SVCS. — 10/09 04/26/10 Description of Incident: Employee was trying to get a grease fitting on the bucket of end loader #750. After several attempts, he reached for the grease gun tip and the flexible hose blew off the crimped section on the fitting end. The grease that came out under extreme pressure squirted employee in the left eye. Supervisor's Comments & Recommendations: Due to conflicting accounts of what happened leading up to the accident, (from the operator of the failed equipment and the witness) I have to take action on numerous conditions: 1) do not yank the grease hose off of a fitting when it is stuck because of pressure; 2) do not use two hands to over pressurize the grease gun that was meant for single hand operation; 3) do not pull down on the grease hose from the overhead by the flexible hose, possibly weakening the hose; 4) replace all high pressure grease hoses in the shop because some were meant I ' I I Loss Prevention Committee Minutes for May 20, 2010 Page 8 of 8 for hand operated grease guns only, not the overhead type, and some were weak from many years of use. LPC Comments & Recommendations: Concur P-12 OFC. RICHARD MEYERS POLICE DEPT. 04/28/10 Description of Incident: Sometime during the past year, Officer Meyers misplaced his department issued Stinger Flashlight. Although he cannot be certain, Officer Meyers believes that he brought it home but.cannot locate it. Supervisor's Comments & Recommendations: Unused department issued property should be kept in his department locker. LPC Comments & Recommendations: Concur .P-13 S-O.T, EDWARD'HANSEN POLICE'DEPT: 0510:6/:10. Description of Incident: Employee was attempting to maneuver police Squad 227 into a marked parking space at the 901 Wellington Municipal Complex, and while completing the maneuver, the right front bumper area came into contact with the left rear wheel well of a vehicle parked in the parking space directly adjacent. The vehicle struck police Squad 244 that was parked and unoccupied. Supervisor's Comments & Recommendations: Use of better judgment of distance. LPC Comments & Recommendations: Driver Refresher Course
