The meeting was called to order at 8:35 A.M. at the Village Hall.

• LPC needs Scott Bernholdt to supply more information regarding how the injury
occurred regarding the On-the-Job Injury Report SR-2.

• The LPC suggested that Randy Block investigate a weed tool on the market for
purchase in regards to the On-the-Job Injury Report LF-1.
• There have been several instances of damage to the Public Works garage chain link
fence in the last few years. A suggestion was made to install a security camera.

Description of Incident: While moving gradall from upper cab, the lower cab's mirror
brushed up along a tree branch and the side mirror was broken.
Supervisor's Comments & Recommendations: Be more aware of your
surroundings.

S-4 MARK ANGELLOTTI — PW— INFRASTR. 05/06/10
Description of Incident: As the employee was clearing debris from the storm grate with
the backhoe, the backhoe began to slide/shift down the embankment. To stop the slide,
the employee lowered the bucket to the ground. This action had the desired effect in
stopping the movement of the machine; however the employee's knee contacted the
rear glass panel which instantly shattered two of the three glass panels. The impact of
the employee's knee on the glass was moderate, but the employee declined medical
treatment.
Supervisor's Comments & Recommendations: The grade of the embankment where
the backhoe was set up is minimal, and the employee properly applied his knowledge
and experience in setting up the backhoe to accomplish the assigned task. Movement
of the machine, when in operation, is not uncommon and is to be expected. The
employee properly applied his operating skills to stop the movement of the machine.
Use of other equipment may be an alternative for the task in the future.

F-8 FIRE DEPARTMENT- 10/10 05/12/10
Description of Incident: The employee was in full turn out gear working in an
extrication drill. Firefighter was using the spreaders (approx. 55 Ib. tool) for several
minutes and when the firefighter finished the task, he/she sat down on the bumper of
another car and advised me that he/she was not feeling well. The employee was
relocated to the side of E7 and A7, and A8 crews were requested to assist with
treatment.
Supervisor's Comments & Recommendations: The employee was not feeling well
prior to work that day. While working through a demanding drill he became over
exerted and should have advised me that he/she was not feeling well earlier in the drill.

W-4 JOSH FARMER — UTILITIES 05/20/10
Description of Incident: As truck #822 was entering the garage, the employee closed
the door, making contact with the tarp cover.
Supervisor's Comments & Recommendations: Employees that are closing the door
need to make sure there is no vehicle approaching the entrance or waiting to enter.
LPC Comments & Recommendations: If the automatic reversal is not in operation — it
needs to be repaired; and if the door does not have this, it needs to be installed.
F-9 FIRE DEPARTMENT — 10/11 05/20/10
Description of Incident: Employee was stepping up into the ambulance while on a
call. He/she felt a pop in their pelvis and felt pain in the thigh and hip.
Supervisor's Comments & Recommendations: Unknown, there was no apparent
cause for the accident. I am unable to explain how this injury could have been avoided.
Stepping up into the ambulance from the side door is a normal practice. Employee
states he/she used the hand rail and there were no unsafe conditions to the ground or
his/her footing that caused any risk.

P-14 10/12 - POLICE DEPT. 05/28/10
Description of Incident: While the Officer was at Hollywood Kennels, a cat he/she
transported, got out of the village trap during the transfer to the cage and fell on the
floor. The officer picked the cat up, and it scratched his/her hands and arms a total of
six (6) times while putting the cat in the kennel cage.
Supervisor's Comments & Recommendations: The Sergeant spoke with the Officer
and counseled him/her about wearing the department provided protective gear when a
situation calls for it. He/she indicated that they understood, and would take the
necessary precautionary steps in the future to eliminate this type of mistake.

FS-4 DEREK STURGULEWSKI — FLEET SVCS. 05/28710
Description of Incident: Derek had to prop the hood open on car#803 because the
hood gas shocks went bad. The hood needed to be open to pressure wash the engine
before he performed his service and repairs. While pressure washing, the wand hose
caught the prop and the hood came down onto the fallen prop chipping a 1" x 1" piece
of plastic out of the hood valance.
Supervisor's Comments & Recommendations: More care should be taken working
around a temporary prop rod or have another person hold the hood up while working
under those circumstances.

F-10 DANIEL SCHNEIDER— FIRE DEPT. 06/11/10
Description of Incident: While backing up and turning, the right front diamond plate
behind the bumper clipped the concrete light standard along the fencing in the Public
Works lot. A rear spotter was being utilized at the time, and it was in view of the driver
but not of the obstacle.
Supervisor's Comments & Recommendations: Anytime a vehicle is backing up, the
driver needs to utilize two (2) additional personnel to spot rig. One should be located at the
back, and one in the front, watching all parts of the apparatus and possible obstacles. All
this has been covered in SOG 220 (vehicle backing policy).

P-15 OFC. JASON GOTTWALD — POLICE DEPT. 06/20/10
Description of Incident: As Officer Gottwald turned into the assigned parking stall, the
front left corner of the front bumper came into contact with the support beam.
Supervisor's Comments & Recommendations: Use extreme caution pulling into the
very narrow parking place.

P-16 OFC. STEVEN PRINDLE — POLICE DEPT. 06/24/10
Description of Incident: Officer Prindle was proceeding southbound on Elmhurst
Road just north of Lunt Avenue in Police Squad #238. He observed a possible
overweight truck proceeding in the opposite direction. He executed a U-turn and the
right, front tire and rim of (Police Squad #238) struck the curb on the east side of the
street next to the right northbound lane of Elmhurst Road just north of Lunt Avenue.
Supervisor's Comments & Recommendations: Officer Prindle must use better
judgement and make sure there is enough room to safely execute a U-turn or any other
traffic maneuver when going to stop a violator.

LF-1 WATER & SEWER — 10/14 06/28/10
Description of Incident: The employee was weeding the live roof system on the roof
of the Village Hall. The constant bending over and stretching caused back muscle pain
and discomfort. He/she notified supervisor at lunch time (11 a.m. that day) that he/she
did not feel it necessary to see the doctor at that time, and would like to "wait and see if
it gets better on its own". Follow-up: Employee felt that condition was not improving
and went to their private doctor on 7/2/10 for medical attention, and was released
without restrictions and will obtain a signed Return to Work Authorization from their
doctor.
Supervisor's Comments & Recommendations: Employee has completed this same
work in the past without incident, which involves continual bending, squatting, and
standing while removing clover from the live roof plantings. Employees will be advised
to stretch lightly beforehand and be mindful of reach, posture, and position.

P-17 POLICE DEPT. - 10/1307/04/10
Description of Incident: The Officer was processing a theft scene for evidence. The
Officer was climbing over pallets of product in a trailer while holding the ET camera. As
the Officer stepped off a box, he either stepped onto another small box that gave way or
he/she slipped off the edge of the pallet. The Officer was not absolutely certain.
However, when doing so, his/her left knee twisted causing slight pain. The officer
stated he/she did not wish to seek medical treatment thinking he/she had strained or
pulled a muscle.
Supervisor's Comments & Recommendations: In the future, the Officer should
remove the equipment he/she is holding, set it aside, or pass it to an assisting Officer.
This will free the Officer's hands to assist him/her in maneuvering over obstacles when
his/her vision is obstructed. Also if possible, look at the area that will be used for his/her
footing to make sure it is safe.

P-18 POLICE DEPARTMENT— 10/15 07/09/10
Description of Incident: The Officer was sliding into the driver's seat of Squad #226
when he/she struck his/her right knee on the computer/docking station. His/her knee
became sore and slightly bruised.
Supervisor's Comments & Recommendations: The Officer was not familiar with the
setup of the computer inside this Squad car. He/she is now very familiar with this setup
and this type of accident should be avoided in the future.

F-11 FIRE DEPARTMENT— 10/16 07/10/10
Description of Incident: While extricating a patient, the patient was acting combative
when he/she grabbed at him, and their bloodied finger entered firefighter's mouth, his/her
finger exiting firefighter's mouth in a "fish hook"manner. The patient was removed from the
vehicle and into an ambulance. The firefighter then told me of the exposure.
Supervisor's Comments & Recommendations: Extrication was difficult as patient
was suspended in her seat. A lot of communication was being done by the crew
through talking. The probability of another exposure of this manner is unlikely.

P-19 REBECCA MCGREAL — POLICE DEPT. 07/13/10
Description of Incident: Officer McGreal was backing Squad #236 into a parking stall
at Thorntons Gas Station located at 2310 York Road, Elk Grove Village, IL when she
struck a portion of the building that extended outward towards the parking lot. Squad
#236 suffered a minor scratch on the rear bumper, and a small amount of paint/cinder
block was chipped off from the building. The store cashier, Adram Kulam was notified.
Supervisor's Comments & Recommendations: Officer McGreal was reminded to
use caution when backing into parking stalls, specifically stalls that are near buildings.

F-12 LT. PETER GUTZMER— FIRE DEPT. 07/15/10
Description of Incident: The shore line ejected from the vehicle upon start-up while
responding to an Automatic Fire Alarm. The shore line plug failed to clear away from
the vehicle before the vehicle moved forward to respond. The plug was crushed under
the vehicles tires.
Supervisor's Comments & Recommendations: Re-educate staff on the need to
have all shore lines (for vehicles after ejecting) to hang freely from the ceiling to avoid
impact or damage when vehicles move from their parking area. Apparatus operators
must visualize the power cord prior to moving a fire vehicle.

P-20 POLICE DEPARTMENT - 10/17 07/20/10
Description of Incident: An Ambulance call (10-11497) for an unresponsive subject not
breathing was given CPR by an employee for approximately 10 minutes. The employee,
while conducting follow up at ABMC ER, passed out while talking with Village paramedics.
The ER Dr. Gordon advised that the employee over exerted himself/herself during life
saving efforts and suffered from heat exhaustion.
Supervisor's Comments & Recommendations: Employee must remain hydrated on
sunny days and pace self in order to not over exert himself.

W-5 ANGEL LOPEZ — PW-UTILITIES 07/22/10
Description of Incident: After dumping material at a job site, the tailgate could not be
returned to its original position. See attached employee statement.
Supervisor's Comments & Recommendations: Since the pintle hitches on all
vehicles are removable, they should only be placed into the receiver when the vehicle
needs to pull a trailer.

P-21 Reported By: Gary Miura 07/26/10
POLICE DEPARTMENT
Description of Incident: At 1740 hours I was notified by the I/C Operator, Officer
Moujouros, that the prisoner in cell#7 was hitting the outer covering for the cell camera with
his fists. Upon arrival, it was observed that the camera appeared to have been pulled from
the ceiling mount, and was hanging by the wiring. Police report CDC# 10-11877.
Supervisor's Comments & Recommendations: Due to the fact that there are
disgruntled prisoners detained in our jail, there are no recommendations as how to
prevent future occurrences.

P-22 OFC. JOHN WILLIAMS — POLICE DEPT. 07/28/10
Description of Incident: Officer was closing out his car. He rolled up the driver's
window and as he exited the car, he noticed the window was cracked. It is unknown
how the damage occurred.

F-13 ROBERT STECKI — FIRE DEPT. 08/01/10
Description of Incident: Station 7 was dispatched for an accident on northbound 53/south
of Higgins. The accident was found at northbound 53/north of Higgins. Upon our arrival
numerous vehicles were involved in the accident. The supervisor and Battalion 2 pulled up
on the scene in the far right lane of northbound 53. While managing the incident, a
northbound vehicle struck the left rear corner of#157 while the vehicle was parked on the
scene. (Vehicle was occupied). At this point I requested Battalion 5 from Schaumburg to
assume Command. Northwest Central Dispatch was notified including the Illinois State
Police. Fire #2838 driver of the vehicle that struck the Suburban was not paying attention
to the conditions of Rt. 53.
Supervisor's Comments & Recommendations: More aggressive lane or road
closures to prevent damage to property or people.

P-23 OFFICER M. MCGREAL — POLICE DEPT. 08/01/10
Description of Incident: While attempting to pull into a parking space, the Officer
misjudged his clearance and struck the vehicle parked in the adjacent space. The front
right portion of the police bumper struck the rear left tire of the parked vehicle.
Supervisor's Comments & Recommendations: In the future, the Officer should pay
more attention to his surroundings when operating a village vehicle.

SR-2 WATER & SEWER- 10/18 08/06/10
Description of Incident: While flushing and pulling a hose, the employee felt the left ring
finger pop.
Supervisor's Comments & Recommendations: This is something we do on a regular
basis. Injury is unpreventable.

P-24 Reported by: Officer Brandi Mathis 08/06/10
POLICE DEPARTMENT
Description of Incident: On 8/7/10, Officer Mathis observed damage to the front, right tire
and rim while she was inspecting her vehicle. A check of the driver log indicated Officer
Vrchota was the last person to drive the vehicle on 8-6-10. The damage was fresh as
concrete dust fell from the valve stem while checking the damage. I asked Officer Vrchota
if he struck anything on 8/6/10 and he said he did not. I checked videos from the two (2)
days and did not observe any unusual movements while the vehicle was in motion.
Supervisor's Comments & Recommendations: Be more careful when driving.

BS-3 WATER & SEWER — 10/19 08/12/10
Description of Incident: While taking apart an EM fire light, and after disconnecting the
power cord when pulling the cord through the light housing, the wires popped out and
struck the right eye.
Supervisor's Comments & Recommendations: Safety glasses should be worn while
performing these job tasks.

P-25 OFFICER RUSSELL SULLIVAN — POLICE DEPT. 08/16/10
Description of Incident: Officer Sullivan was backing from a parking space in the parking
lot of Motel 6. He backed into a guard rail, damaging the rear-passenger side bumper
cover/rear tail light assembly.
Supervisor's Comments & Recommendations: Use more caution.

FIRE DEPARTMENT
Description of Incident: Engine #8's cell phone was discovered missing on Saturday,
August 14, 2010. Crews checked all vehicles and shifts the last three days, and the cell
phone still was not found.
Supervisor's Comments & Recommendations: If budget allows, have a phone in
each vehicle so they do not have to be switched. If not, purchase larger phones that
are easier to see and keep track of.

F-15 FIRE DEPARTMENT— 10/20 08/28/10
Description of Incident: The Firefighter was the engine officer and first in company to
a house fire at 690 Ruskin. Firefighter performed tasks such as pulling the 200 ft. pre-
connect hose, entering a garage charged with smoke to extinguish a fire. The firefighter
also navigated through the home, full of smoke. Later the firefighter and his crew
performed an overhaul. After the fire, the firefighter was assigned investigator duties
and stayed on the scene. Per the Ambulance Report, the firefighter complained to
personnel on the scene that he/she was having chest pain. Per the Fire Report: After
returning from a house fire, the firefighter advised a coworker that he was still
experiencing chest pain. Treatment was started, an incident number was punched, and
E7 assisted A7 with ALS care. A7 transported to ABMC.
Supervisor's Comments & Recommendations: The firefighter experienced cardiac
chest pain during an unavoidable hazardous condition of firefighting operations.

FIRE DEPARTMENT
Description of Incident: While raising a main aerial ladder to the roof of the building
during initial fire attack/suppression/ventilation operations, the "foot pad" on the
underside of the ladder basket hit/snagged the I-beam face of the building at the roof
line, causing the "foot pad" to shear off.
Supervisor's Comments & Recommendations: Initial operational control of the aerial
device can be given to the turntable position, but only to the extent necessary to get the
aerial ladder out of the vehicle cradle (driving storage position), to an elevation that can
safely clear the vehicle and its components. Turntable operator should then only
observe for potential hazards (poles/wires, etc.) and provide safety cut off, while
operational control (extending/elevating/rotating) of the ladder should then rest with the
operators located in the basket.

P-26 10/21 - POLICE DEPARTMENT 9/2/10
Description of Incident: Officer was operating a department-issued vehicle (#251) and
was enroute to a surveillance in Alsip, IL from his residence on 9/2/10. At approximately
6:35 a.m., the Officer was stopped in traffic when a vehicle struck the rear of the Officer's
vehicle, pushing his vehicle forward where the front of his vehicle struck the rear of the
vehicle in front of him. Officer was transported by ambulance from the crash scene to
Advocate Good Shepherd Hospital where he was treated for his/her injury.
Supervisor's Comments & Recommendations: This was an unavoidable accident
due to the defective brakes of another person's vehicle. I have no recommendation to
the employee as to how to prevent a similar occurrence.

P-27 THOMAS STURGULEWSKI — POLICE DEPT. 9/2/10
Description of Incident: Thomas Sturgulewski was operating a department vehicle
enroute to conduct a surveillance regarding a pending investigation. While stopped on
Cary Road, he was struck from behind by a Nissan Altima driven by Belgica Cardoso. The
impact pushed Investigator Sturgulewski's vehicle forward, resulting in it striking a
Chevrolet Silverado which was stopped in front of his vehicle. The Silverado was operated
by John Garozzo of 202 Lake Shore Dr., Cary, IL. John Gerozzo did not indicate that he
was injured.
Supervisor's Comments & Recommendations: No recommendation, employee not
at fault and had no control over the actions of the other motorist who struck him.

Description of Incident: A Tanker truck filling with water most likely struck the building
while pulling away - too close to the building.
Supervisor's Comments & Recommendations: Possible install a protective bollard
and/or install cameras to catch the offenders.
