The meeting was called to order at 8:35 A.M. at the Village Hall.

P-28 10/22 — POLICE DEPARTMENT 09/20/10
Description of Incident: The Officer was driving eastbound on the south drive in the
parking lot of Elks Crossing to back up another Officer. As the officer passed the
entrance to the McDonalds, the officer started to turn to the left. The Officer looked
over his/her right shoulder to make sure the private drive was clear of traffic. The
Officer turned her/his head back, seeing the light post, but it was too late to take
corrective measures.
Supervisor's Comments & Recommendations: The Officer must be constantly
aware of his/her surroundings while operating his/her vehicle.

P-29 OFFICER KELLEY SWEENEY— POLICE DEPT. 09/20/10
Description of Incident: The Officer was driving eastbound on the south drive of Elks
Crossing on her way to back up another officer. As she passed the McDonalds
entrance, she began to turn left in the lot while looking over her right shoulder to make
sure nobody was exiting the McDonalds. When she turned around, she saw the light
pole, but was unable to avoid a collision.
Supervisor's Comments & Recommendations: Driver must be aware of her
surroundings while operating a motor vehicle.

S-6 10/23 — STREET 09/24/10
Description of Incident: While shoveling asphalt millings off the roadway, the
employee felt a "pull" or"snap" in his lower back. Upon returning to the Landmeier
facility, the employee reported the incident but declined medical attention in hopes that
it was a slight muscle strain that would get better over the weekend. However, by the
following Monday morning, the employee felt that his condition had worsened and
required medical attention.
Supervisor's Comments & Recommendations: The employee has repeatedly shown
himself to be capable of performing the required action during his/her employment here.
He was using the appropriate tools and the accepted methods when the injury
occurred. None of the other crew members recall seeing the employee performing this
task in an unsafe manner or reported unsafe working conditions.
As the employee was performing a required job task and no unsafe acts or conditions
were identified, it is difficult to determine how future occurrences may be avoided.
Therefore no recommendations or actions are being presented at this time.

SR-3 MATTHEW KECK— UTILITIES 09/24/10
Description of Incident: Matt was on Julie locates, which requires him to lift manhole
covers to see the direction of pipes, he removed the lid, and as he leaned over the radio
fell out of his top pocket into the water vault that had about four feet of standing water.
Supervisor's Comments & Recommendations: Employee needs to take more
precautionary measures. He needs to leave the radio in the truck or on the ground next
to him. This type of radio has no clip.
LPC Comments & Recommendations: Employee does not need to have the radio on
him. However, there are leather cases available (Engineering and Fleet have them) that
clip on the belt. A holster type case is also available (Canine Officers use) that are more
secure.

P-30 OFFICER ERIC PERKINS — POLICE DEPT. 09/26/10
Description of Incident: Officer Perkins was responding to a traffic crash at the
intersection of Rohlwing/Nerge. One of the vehicles involved was reported to have an
unconscious driver still in the vehicle, and the vehicle was continuously doing circles in
the intersection, out of control. When Officer Perkins arrived, he discovered the vehicle
was in fact doing continuous circles in the intersection, with the driver's door open.
Present were numerous pedestrians, on-lookers, and other vehicles. Officer Perkins
placed Squad #233 in the direct path of the out of control vehicle, stopping the car.
Supervisor's Comments & Recommendations: No recommendations. This was an
intentional act in response to a dangerous situation.

P-31 SGT. GARY MIURA — POLICE DEPARTMENT 09/30/10
Description of Incident: The left arm brushed against the telephone causing it to fall
out of the holster and into a toilet bowl.
Supervisor's Comments & Recommendations: The Sergeant's arm accidentally
brushed against the telephone. More caution could have been exercised while at the
toilet.

F-17 10/24— FIRE DEPARTMENT 10111/10
Description of Incident: Sometime between positioning the backboard under the
patient and lifting the patient onto the stretcher, a piece of glass cut the palm of
employee's left hand through his/her latex glove. There was about a % inch laceration.
Employee self-cleaned and bandaged the wound and did not seek medical care.
Supervisor's Comments & Recommendations: Employee came to supervisor after
the call and told the supervisor that he/she had cut his hand on some glass from the
accident. Employee was wearing latex gloves because he/she was involved with patient
care. Employee was informed to watch hand placement and take precaution with the
broken glass. Employee stated that he/she did not need stitches and did not need any
other medical care. There was no fluid contact with the patient.
LPC Comments & Recommendations: Concur
F-18 JORGE SIGLER — FIRE DEPARTMENT 10/12/10
Description of Incident: While at the hospital, the patient was transferred from the cot
to a hospital bed. When removing the stretcher from the patient, the cable unknowingly
became tangled between hospital bed and stretcher causing cable to sever.
Supervisor's Comments & Recommendations: Take time to make sure that all
equipment is put away properly.

P-32 10/27 — POLICE DEPARTMENT 10/22/10
Description of Incident: The Officer advised his/her supervisor that he/she was
engaged in a foot pursuit at 00:37 am. At the completion of his duty, the Officer
discovered that his/her knee had some swelling and stiffness.
Supervisor's Comments & Recommendations: Due to unpredictable situations, the
Officer became involved in a foot pursuit which unfortunately caused his/her right knee
to swell.

P-33 10125— POLICE DEPARTMENT 10/22/10
Description of Incident: While engaged in a foot pursuit, the employee felt a tear in
his/her left hamstring and right calf.
Supervisor's Comments & Recommendations: Unfortunately I don't believe this was
an avoidable injury. As an Officer sits in vehicles for long periods of time, they may be
called on to perform explosive bursts of energy without warning or properly warming up.

P-34 10126 — POLICE DEPARTMENT 10/25/10
Description of Incident: The Officer reported to his/her Supervisor that he/she injured
his/her neck as a result of repeatedly raising and lowering a gurney on a death
investigation call.
Supervisor's Comments & Recommendations: This piece of equipment (gurney)
needs to be replaced with a functioning and more user friendly model.

P-35 OFFICER JEDLINK — POLICE DEPT. 11/04/10
Description of Incident: Officer Jedlink was executing a right turn from northbound
Busse onto eastbound Landmeier when the rear of his squad began to "fishtail" on the
wet pavement. In an attempt to gain control of his unit, he then turned into the skid.
Officer Jedlink's squad then jumped the curb, continued onto the south parkway and
struck a Cook County speed limit sign. As a result of the curb collision, the right
(passenger side) tire/rim was damaged. The collision with the sign post resulted in an
approximately three inch scrape to the left area of the front bumper.
Supervisor's Comments & Recommendations: Officer Jedlink should have
compensated for the adverse road conditions and adjusted his driving accordingly.
