Review of the Loss Prevention Committee Functions & Future Procedures.
Sally Bulicek from Gallagher Bassett Services, Inc. spoke to the LPC Committee about
the progress of the LPC Committee, the goals/objectives for the New Year, and
commended the LPC Committee for their hard work. She gave handouts to the LPC
Committee regarding The Full Circle Program, and The Functions and Responsibilities
of a Safety Committee.
o The Committee began to discuss recurring safety issues, training topics, and ideas for
the future direction of LPC as brought forth by Committee members and Sally. The
Committee agreed to meet again in a special meeting to discuss these topics at length.
The special meeting will be April 21St at 2:00 p.m.

S-7-10 MIKE WAGNER— STREET DEPARTMENT 08/18/10
Description of Incident: No damage to the trailer per Police Report. Mary Stauton of
1441 Volkamer Trail was driving her 1999 Buick Century north bound on Volkamer Trail
and drove up the ramp of the trailer with the right side front tire and the car fell off the
trailer causing a flat tire on her car. This happened when she tried to go between our
truck and trailer which was parked on the side of the street.
Supervisor's Comments & Recommendations: Ms. Stauton was in a hurry and did
not stop to let a school bus pass through as she tried to make it between the bus and
parked trailer.

P-36 OFC. RICHARD MEYERS — POLICE DEPT. 11/02/11
Description of Incident: While attending training on November 2, Officer Meyers
discovered that the right rear back-up lens on Squad #246 was cracked. There was no
indication that the damage occurred at that location, and he was unaware as to how the
damage happened. The damage to the bumper is old damage that had been previously
reported.
Supervisor's Comments & Recommendations: Either the vehicle was struck by
another object, or the vehicle backed into an object. There is no way to determine this
with absolute certainty.

S-8-10 BOB COOKE — STREET DEPT. 11/08/11
Description of Incident: Bob Cooke discovered a vehicle with a broken windshield
and reported it.
Supervisor's Comments & Recommendations: A stone could have flung off a tire.
This was not reported at the time the accident happened; Bob discovered the vehicle
with the windshield broken.

P-37-10 THOMAS KREJCZYK — POLICE DEPT. 11/23/10
Description of Incident: Ofc. Krejczyk was backing Squad #264 out of its assigned
parking spot when the driver's side tail light made contact with a support beam, causing
damage to the tail light.
Supervisor's Comments & Recommendations: When the crash occurred, Ofc.
Krejczyk misjudged the distance of the support beam when he was backing out of the
parking spot.

P-38-10 DEBRA LUKASIAK— POLICE DEPT. 12/01/10
Description of Incident: Debbie was turning to go up the ramp of the garage and cut
the corner too sharp. The rear quarter panel and rear door were damaged.
Supervisor's Comments & Recommendations: Debbie was advised that she needs
to be more careful when driving vehicles in the garage.

5-9-10 TODD GORE — STREET DEPT. 12/03/10
Description of Incident: Todd was backing up #718 into the truck's parking spot when
his foot slipped off the brake pedal and backed into the fire extinguisher and the box
that is mounted to the I-Beam.
Supervisor's Comments & Recommendations: Employee must make sure his foot is
placed on the center of the brake pedal.

P-39,10 OFC. MIKE MCINTYRE — POLICE DEPT. 12/20/10
Description of Incident: Officer McIntyre was pulling into the Public Administration
parking garage — spot #257, and struck the electrical box mounted on the column.
Supervisor's Comments & Recommendations: Driver should check parking space
for obstructions and avoid driving close to objects.

S-10-10 KEITH CONLEY — PUBLIC WORKS 12/21/10
Description of Incident: Keith was plowing slush from the center lane going west
bound on Pratt Blvd. when an east bound truck's windshield was sprayed with icy slush
from the Village truck's plow, causing the damage on the other truck.
Supervisor's Comments & Recommendations: Employee must make sure that the
plow is angled completely to the right side when plowing.

P-40-10 10/28 — POLICE DEPARTMENT 12/24/10
Description of Incident: The Officer had his/her right hand bitten by a subject while
trying to take him into custody.
Supervisor's Comments & Recommendations: Due to the close proximity, the
Officer must engage the subject while taking them into custody and there is no way to
prevent such an incident.

P-41-10 OFC. MIGUEL MOSQUEDA— POLICE DEPT. 12/27/10
Description of Incident: Officer. Mosqueda was backing the Squad out of a parking
stall and struck a support beam with the left front side of the vehicle.
Supervisor's Comments & Recommendations: The Officer should clearly check
objects prior to backing up the vehicle.

P-1-11 OFFICER RUSSELL SULLIVAN — POLICE DEPT. 01/03/11
Description of Incident: While in the Municipal Complex parking garage, Officer
Sullivan was removing the battery from the radar gun when he accidentally dropped the
radar gun. The gun fell to the cement ground which caused damage to the radar's
antenna.
Supervisor's Comments & Recommendations: To prevent a similar incident, Officer
Sullivan has to take extra care/caution when removing the battery from the radar gun.

SGT. JENSEN — POLICE DEPT.
Description of Incident: A juvenile was being processed for consumption of alcohol by
a minor and was placed in Interview Room #1. While inside the room, the juvenile
damaged a plastic sign that was posted on the south wall. The sign indicates to people
that if they need medical attention, they are to advise an Officer.
Supervisor's Comments & Recommendations: Required by law, signage must be
posted. Recommendation that the sign be secured with more screws, and that Officers
monitor the Interview Room via camera. Commander Nowacki will review hand cuff
rings in the Interview Room.

PUBLIC WORKS
Description of Incident: A force was applied to the overhead door roller track that
caused it to bend in two places, and dislodge from its normal mounted position. I did
not see how the door came to be in this condition, and therefore cannot conclusively
determine how the damage occurred.
Supervisor's Comments & Recommendations: Without discussing the incident with
the person responsible for the damage, I cannot pinpoint the breakdown in the
sequence of events that led to the damage. I will defer any recommendations until that
information is made known to me.

P-03-11 OFC. PATRICK EVANS — POLICE'DEPT. 01/14/11
Description of Incident: Officer Evans was stopped in traffic in the left lane eastbound
on Landmeier Rd. near Higgins Rd. when he decided to perform a u-turn to westbound
Landmeier Rd. and was struck by a moving eastbound vehicle in the left turn lane next
to him.
Supervisor's Comments & Recommendations: Officer Evans needs to pay closer
attention to his surroundings before maneuvering his vehicle in such a manner that
could potentially cause him to be involved in a crash.

P-04-1'1 OFC. JOHNNELL WILLIAMS — POLICE DEPT. 01/16/11
Description of Incident: Officer Williams was backing out of parking space #230 with
Squad #230 when a drainage grate shifted upwards and punctured the undercarriage of
the vehicle.
Supervisor's Comments & Recommendations: The drainage grates have a locking
mechanism for keeping the grates in place that needs to be engaged.
LPC Comments & Recommendations: Concur. The Committee discussed having
periodic safety checks in the garage.

P-05-11 OFC. MIGUEL MOSQUEDA— POLICE DEPT. 01/18/11
Description of Incident: Officer Mosqueda inadvertently placed a Department workbook,
mini cassette recorder, and an Intoxilyzer S-D2 (PBT) on the trunk of his squad as he was
preparing to begin his tour of duty. Officer Mosqueda then drove his squad out of the
Municipal Garage while those items remained on the trunk. Apparently when Officer
Mosqueda turned onto Commercial Drive from the Police Department drive, the items fell
off the trunk and onto the roadway. It is believed those items were then damaged by other
traffic running over them.
Supervisor's Comments & Recommendations: Officer Mosqueda cannot utilize the
exterior of a Squad to temporarily rest his equipment on it. He should utilize the interior of
the Squad which would better protect the equipment.

P-06-11 11/01 — POLICE DEPARTMENT 01/26/11
Description of Incident: The Officer, while being assigned to a truck theft surveillance,
was walking in a parking lot to check a registration on a vehicle parked in the lot. While
the Officer was walking in the lot, he/she slipped on ice. As a result the Officer landed
on his/her head and shoulder.
Supervisor's Comments & Recommendations: Supervisor does not see any actions
that the Officer could have done to prevent this accident.
LPC Comments & Recommendations: The Committee discussed the importance of
employees wearing safety shoes (grooved, rubber soles) and that they should be
mandatory to try to prevent falls.

OFC. TIMOTHY KLEINFELDT
Description of Incident: Officer Kleinfeldt dropped a vehicle off at the Meacham
Public Works for service. A Public Works employee found damage after washing the
vehicle. Officer Kleinfeldt stated that the vehicle was very dirty, and he didn't see
damage when checking the Squad at the start of his shift. The Public Works employee
stated the damage appeared to be old.
Supervisor's Comments & Recommendations: The unknown driver could have
avoided this by being aware of the surroundings of his Squad.

OFFICER MCINTYRE
Description of Incident: Subject was handcuffed to the steel chair which has a
handcuff ring attached to the underside of the chair. The subject used the metal chair
as leverage bending both rings of the handcuffs as he tried to break free. The cuffs
were damaged so severely they needed to be cut off the chair with bold cutters.
Supervisor's Comments & Recommendations: The seats in the interview room need
to be replaced with a chair design that does not have the small ''/z" steel rim that can be
used for leverage. A second solution is to put a bar on the wall so the prisoner could be
cuffed to it. Third, if these rooms had the ability to be monitored via audio, possibly the
I/C Officer would have heard noise of the metal on metal as he bent the cuffs.

FIRE DEPARTMENT
Description of Incident: During the Snow Blizzard, the apparatus door (A10) was
opened for (A10) to back into the station, after a call. The wind was blowing large
amounts of snow into the building. The battery chargers (near the front bay door) were
covered by snow, in a matter of seconds. The optimizer charger/analyzer (800 radio
batteries) started to arc inside of the machine. There were four (4) or five (5) large arcs
before we could unplug the machine.
Supervisor's Comments & Recommendations: If possible, move the charging units,
or protect them from weather conditions.

Description of Incident: While clearing snow, plow drivers pushed the snow to the
corners of the parking lot. The snow moved some portions of the wall.

familiar with the area ask someone who might know before starting.

S-02-11 SCOTT SANTILLE — STREET DEPT. 02/02/11
Description of Incident: During the blizzard, Scott was plowing high, drifting snow off
the road when the snow got under the right front fender causing the fender to break.
Supervisor's Comments & Recommendations: The front-end loader was not
available to be used due to the historic snowfall totals.

S-03-11 JEFF REYNOLDS — PUBLIC WORKS 02/02/11
Description of Incident: While snow plowing, Public Works employee hit a large tree limb
that had fallen and was buried in the snow.
Supervisor's Comments & Recommendations: The tree limb was buried and the
employee was unable to see it before striking it, causing damage to the vehicle. The
accident was unavoidable; this occurred during the February snow blizzard.

S-04-11 JOSH FARMER—WATER DEPT. 02/03/11
Description of Incident: During the blizzard cleanup while pushing snow off the street,
the front end loader hit the rail-road track as it passed over the crossing.
Supervisor's Comments & Recommendations: The snow was very deep; therefore it
was hard to see the rail. The employee needed to go slower to feel the rail at the
crossing.

BS-03-11 11102 — BUILDING MAINTENANCE 02/07/11
Description of Incident: When the employee was locking the door leading from room F to
room E, the door was not locking. When the employee pulled the door closed to lock it, the
door handle came off. The employee lost his/her balance and fell backwards to the floor,
landing on his/her buttocks and right wrist.
Supervisor's Comments & Recommendations: The employee must have proper
footing and balance.

F-02-11 11/4— FIRE DEPARTMENT 02/09/11
Description of Incident: A8, E8, and ER staff members were transferring a very heavy
patient (approx 400 lbs.) from the ambulance stretcher to a bed in the ABMC ER. The
stretcher and bed were placed next to each other. Prior to moving the patient, the
firefighter stated that he/she wanted to lower the stretcher. I asked, "why"and the firefighter
replied, "because he/she did not want to hurt his/her back". I told the firefighter"don't hurt
your back", and the stretcher was lowered. The patient was moved, and afterwards the
firefighter grimaced as if he/she were in pain. I asked the firefighter if he/she was injured
and he/she repeatedly denied injury stating he/she was OK. E8 returned in service and A8
remained to complete their report. On our next call, the firefighter approached me and
advised me that his/her back now felt tight in the lower thoracic area and that he/she
wanted to document the injury in case it became worse. He/she also stated that he/she did
not want to be examined or treated. The firefighter remained on duty.
Supervisor's Comments & Recommendations: The firefighter was advised to use
proper positioning and lifting technique and to report injuries as soon as possible.

Description of Incident: Firefighter discovered a cracked passenger side door panel
on Al 0; it appears that something hit it.

FIRE DEPARTMENT
Description of Incident: During a morning apparatus check, Lt. Peterson found the grab
handle for roll-up door, first lower compartment, officer's side (compartment for EMS
equipment&extinguishers) bent. The previous shifts reported no incidents where that side
of the apparatus may have contacted anything to cause the damage to the handle. No
other damage was found.
Supervisor's Comments & Recommendations: The cause of the damage unknown.
A report is being done for documentation.

F-05-11 MIKE RODGERS - FIRE DEPARTMENT 02/18/11
Description of Incident: While washing the right side of the vehicle, Lt. Sharp noticed two
vertical scratches on the roll up door for the compartment above the rear wheels. These
scratches were most likely caused by an air bag that was loaded into the compartment with
its valve stem pointing upward. When the door was raised, it probably scrapped against
that valve stem.
Supervisor's Comments & Recommendations: Always try to use caution when
loading compartments. In this compartment, make sure the valve stem is pointing down
or remove the air bag and relocate it to another compartment.

F-06-11 11/05 — FIRE DEPARTMENT 02128/11
Description of Incident: During the course of patient treatment, the patient became
extremely combative. As our personnel were trying to treat and subdue the patient, he
intentionally injured the firefighter's hand.
Supervisor's Comments & Recommendations: Additional manpower may have
prevented the injury.

BS-02-11 11103 — BUILDING MAINTENANCE 02105/11
Description of Incident: While pull starting a snow blower, the employee felt a twinge in
his right elbow.
Supervisor's Comments & Recommendations: The employee possibly used two (2)
hands when starting a pull start unit.
LPC Comments & Recommendations: Recommendation that an electric start for snow
blowers be purchased.

P-09-11 11/06 — POLICE DEPARTMENT 02/28/11
Description of Incident: The Officer was initiating a traffic stop at Nerge and
Bonaventure when the driver of the target vehicle came to an abrupt stop in the middle
of the roadway, In response, the Officer quickly applied his/her brakes, at which time
the rifle (in the vehicle) was projected out of the rack, striking him/her in the back of the
head. The Officer advised that the Officer had checked the rack prior to the beginning of
his/her tour of duty and found it to be secure. The Officer further related that other than
a headache, he/she was not injured. The Officer refused medical attention, and there
was no visible indication of trauma.
Supervisor's Comments & Recommendations: The location of the rifle rack, which is
immediately behind and above the driver's head, should be reviewed.
LPC Comments & Recommendations: This is the second time this kind of incident has
been reviewed. Recommendation that new locks be installed in Squad cars.
F-07-11 JORGE SIGLER — FIRE DEPARTMENT 3/3/11
Description of Incident: Per Firefighter Sigler: While loading equipment on the stretcher,
the monitor's external housing hit the hand rail on the stretcher and cracked one of the
bags off the case.
Supervisor's Comments & Recommendations: Firefighter/Paramedics must be more
careful with carrying and placement of EMS equipment. In the future, secure Lif-Pak
slowly and carefully to the stretcher. Possibly purchase a larger one.

W-01-11 11/07 — PUBLIC WORKS-WATER/SEWER 03/09/11
Description of Incident: While exercising residential water service shut off at 1564
Nevada, the extension that the employee was using for more leverage slipped off the end
of the buffalo box key, hype-extending employee's right elbow.
Supervisor's Comments & Recommendations: Find some way to temporarily put
extensions on the buffalo-box key to exercise the hard to turn round ways. Also look
into obtaining a buffalo-box with longer handles.
