Present: David Preissig, Chairman Dave Miller, Fire Chief John Nowacki, Commander-Police Department Pam Kuc, Housing Inspector Diane Hunter, Finance Department Secretary Karen Turner, Loss Prevention Secretary Absent: Scott Miller, Deputy Fire Chief The minutes were approved for the meeting of April 21, 2011. Review Current Reports: • 10 Tabled Reports: • None Old Business: . None New Business: • Discussed the"Pop Up Screen"that will appear before the Job Injury Data Entry screens appear. Diane will work with Janet Bean to make revisions for review. • Discussed the LPC functions &future procedures. Loss Prevention Committee Minutes for May 19, 2011 Page 2 of 7 • Agreed that to improve communication with the LPC, it would be beneficial to include the Assistant Village Manager as part of the Committee. • Discussed a quarterly Loss Prevention newsletter that will be geared to safety at work. Diane Hunter will begin drafting a newsletter for the committee to review. • Discussed the need for training employees in the field regarding injury prevention and CPR. • Discussed the need for a tornado/evacuation plan and drill. Dave P.will coordinate this through Public Works. • Discussed the EGV Safety Manual — it should be on the Intranet, but needs to be reviewed,.updated, revised, and approved beforehand. Supervisor's Comments & Recommendations: Relocate snow to other areas. LPC Comments & Recommendations: Concur P,09 11% T1/08 POLICE PEPARTMENT.- 3/24/11 s1�1%08 Description of Incident: Officer was on an accident scene where a light pole was down across two lanes of traffic. The Officer helped moved the lamp post from the roadway and then felt pain in the back and knees. Supervisor's Comments & Recommendations: In the future, before exerting full strength in moving an object, the Officer needs to access the weight to determine if assistance is needed. LPC Comments & Recommendations: Concur S,x06 11/ ROBERT COOKE,= PW INFRASTRUCTURE 3/25/11 A-1`7 13 Description of Incident: The accident was caused by a motorist improperly backing out of a driveway and contacting the subject vehicle. Description of Incident: The vehicle was parked at 1142 W. Devon (with the emergency lights on) when a woman in her vehicle backed out of a parking stall and hit the suburban. COMDR -CHUCK WALSH :- Description of Incident: An unknown visitor wrote in black ink on the bench in the Police Department lobby. Supervisor's Comments & Recommendations: The area needs to be monitored via camera and personnel working the police desk. Supervisor's Comments & Recommendations: Employee needs to ensure all mounting brackets work properly. LPC Comments & Recommendations: Concur P 11 1.11 OFC X111 TINA, DREDGE POLICE DEPT 05/06111 v I mY N S y f L= Description of Incident: Officer Dredge was driving on Longboat Dr. when a baseball struck the front hood of her Squad #243, causing a dent on the hood. Description of Incident: The Officers radio fell out of the holder and struck the floor, causing the molded plastic tab (that connects the microphone to the base) to break. Supervisor's Comments & Recommendations: Personal equipment needs to be inspected for wear and tear. If worn out, the Officer needs to notify a Supervisor. LPC Comments & Recommendations: Concur
