SUBJECT: Loss Prevention Committee Meeting Minutes — June 16, 2011
The meeting was called to order at 2:35 P.M. at the Village Hall.
Present: David Preissig, Chairman
Dave Miller, Fire Chief
Keith Briggs, Commander, Police Department
Diane Hunter, Finance Department Secretary
Joe Carey, Assistant to the Village Manager
Karen Turner, Loss Prevention Secretary

New Business:
• The LPC discussed the new"Pop Up Screen"added to the Job Injury Data entry screen.
It is working well and has enhanced the quality of the report. The LPC gave their input
to Diane for more changes and additions to the form; putting it in chronological order,
adding a section for the employee to sign that he read, and agrees or disagrees with the
report.
• Diane Hunter discussed the Safety National website that is a source of useful
information for a newsletter (personal as well as workplace related information). Joe
Carey suggested adding some of that information to the weekly Status & Information
Report.
• Comdr. Keith Briggs informed the LPC that evacuation maps in the new building need to
be revised. Dave Miller will supply copies of the building plans to the Fire Department.
Per Chief Miller, new weather radios will be given each department soon.
• Comdr. Keith Briggs informed the LPC that 18 police officers who have had accidents
began attending NIPSTA Simulator Driver Training last September of 2010. There will
also be a Train-the-Trainer, hands-on driver training at Alexian Field in October of 2011.
• There was some discussion regarding padding for the pillars and electric boxes in the
Village garage, as well as installing a mirror due to the number of accidents to vehicles.

Description of Incident: While excavating to repair a 12 foot water main valve, the
operator snagged the UNMARKED fiber optic cable.
Supervisor's Comments & Recommendations: Anytime any excavation is performed,
the excavators rely on all utilities to be properly marked so as not to cause damage.

F-11-11/ DAN SCHNEIDER — FIRE DEPARTMENT 05/17/11

Description of Incident: A key was inserted into a door keyhole. The door opened but
they could not get the key out before the door opened all the way. The key was bent and
unusable.
Supervisor's Comments & Recommendations: At times it may be necessary to hold the
door to slow the speed at which it opens. This may give enough time for key removal.
Care must be taken when removing the key because it is possible to have fingers pinched
in the door.

W-04-11/ JOSH FARMER — UTILITIES/WATER 05/17/11

Description of Incident: While excavating to repair a water main leak, the operator cut
through an unmarked street light cable.
Supervisor's Comments & Recommendations: If utilities are not properly marked, this
type of accident will continue.
LPC Comments & Recommendations: Concur. Good job of documentation on the
report.

F-12-11 11/09/11 — FIRE DEPARTMENT 5/24111
Description of Incident: The Firefighter/Paramedic was diagnosed with "left thumb strain"
when moving a patient from a home on a stretcher.
Supervisor's Comments & Recommendations: Employees need to look for alternate
means to remove a patient from a home to allow more room for the stretcher if space is
tight.
LPC Comments &Recommendations: Concur. It is possible the injury could have been
prevented.
P-13-11/ OFC. MICHAEL JEDLINK — POLICE DEPT. 05/25/11

Description of Incident: Ofc. Jedlink was conducting a traffic stop and instructed the
driver to pull onto Wood Dale Road. When Ofc. Jedlink pulled behind the vehicle, it
stopped abruptly, and the he struck the rear left bumper of the vehicle with the front right
bumper of his Squad car.
Supervisor's Comments & Recommendations: The Officer needs to have better
spacing between vehicles as well as being more attentive while driving.

P-14-11 11/10 — POLICE DEPARTMENT 05/26/11
Description of Incident: The employee walked from the door (nearest the Police
Department Records counter)towards the stairway in order to go down to the Police lobby.
As the employee approached the landing area at the top of the stairs the heel of his/her
shoe caught one of the grooves on the landing causing employee to stumble.
Supervisor's Comments & Recommendations: This incident may have been prevented
if the employee used more caution in his/her foot placement.

P-15-11/ OFC. MICHAEL PATRAS —'POLICE'DEPT. 05/28/11

Description of Incident: The Officer was backing up from parking stall#233 in the garage
at 901 Wellington Avenue and struck a support pillar with the passenger side of the Squad
car.
Supervisor's Comments & Recommendations: The Officer needs to pay closer
attention to his surroundings when operating a motor vehicle.

F3-11/ DEPUTY CHIEF S. MILLER— FIRE DEPT, 05120/11

Description of Incident: The Deputy Chief discovered damage to the right front bumper
of car #153. The car was parked outside the Village Hall in the public parking lot and he
was unaware of when the damage occurred.
Supervisor's Comments & Recommendations: The recommendation would be to
park the vehicle in a remote/isolated area in the future.
LPC Comments & Recommendations: Concur
P-16-11 11/11 — POLICE DEPARTMENT 06/1/11
Description of Incident: The Officer was investigating a possible suicidal subject who
attempted to close his front door on the Officer. The Officer stopped the door with his/her
right hand and the offender struck him/her in the center corner (tear duct area) of the left
eye with a cordless house phone.
Supervisor's Comments & Recommendations: There were no actions taken by the
Officer which could have prevented this occurrence.

P-17-11 11/12 -POLICE DEPARTMENT 06/01/11
Description of Incident: The Officer was checking the wooded area near Salt Creek for
evidence of a theft when some type of debris or insect entered his left eye, causing
redness, sharp stinging, and irritation.
Supervisor's Comments& Recommendations: The wooded area was freshly mowed by
large equipment, and this may have created more debris in the air.
