The meeting was called to order at 8:34 R.M. at the Village Hall.

Old Business:
• Janet Bean is currently working with Diane Hunter on the revisions to the On-the-Job
reports that would enable us to edit and be able to put them in sequential order.
• The committee discussed the underground parking garage incident summary by Diane
Hunter. The total amount of cost for damage in '09, '10 and '11 is $6,595. Deputy Chief
Nowacki stated that the accident report does not warrant the wrapping of the all the
poles in the garage at this time. It appears that the number of accidents has reduced
since '09, and he would like to wait until year-end to see if they reduce further before any
further action is taken (striping the poles and/or electric boxes). John will speak with the
proper individuals as to whether or not the electrical boxes can be relocated/moved in
parking stalls #257 and #242. Police squad cars are not parking in these two parking
stalls.
• There was discussion regarding the speeders in the street in front of the Municipal
building. Pam Kuc inquired if a stop sign could be put up by the underground garage
entrance/exit door.
• The Safety Nation and OSHA websites will be utilized to access safety information
articles for a newsletter. A draft of the newsletter is planned for November 2011. Dave
Preissig will contact Matt Roan regarding adding a weekly safety tip to the Status &
Information weekly report generated from the Village Manager's Office.
• Dave Preissig will contact the Village Manager to discuss the revising of the Elk Grove
Safety Manual and posting it to the Intranet.
New Business:
• The CPR is now complete for all Public Works employees.
• The LP committee discussed the NIPSTA Simulator Driving Training and concluded that
it is not a substitute for the real hands-on training. The LPC was in agreement that there
needs to be training that would take the place of the Driver Refresher Course currently
used. The employees who have had a vehicle accident need to attend a Driver
Refresher Course within a few months after the occurrence. Deputy Chief Nowacki will
contact Tony Konetzke to look into online training available with the state. He will also
check with Sally Bulicek with Gallagher Bassett Services as to what other Villages do
regarding employee vehicle accidents.

. The Tornado/Evacuation plan and drill will be created over the winter. This will be a
joint venture with the Emergency Management (Fire Dept.). Members indicated that it
is not clear to all employees where they need to move to in the event of a tornado.
The evacuation plan is not in the Safety Manual.

F-22/11 11/21 — FIRE DEPARTMENT 8/11/11
Description of Incident: The Firefighter was performing his/her morning rig check on E9
(E118)and was exercising the pre-piped deck gun that sits on the top of this engine raising
it into the high locked position while standing on the top of the pumper. The firefighter
activated the release with one hand while raising up on the base unit with the other hand.
Using his/her legs to assist with the lift, he/she felt a sharp pulling type pain in hi/her lower
back.
Supervisor's Comments & Recommendations: When operating the equipment, the
operator must be sure to properly engage the release lever before lifting it into the
operating position vehicle while making sure his/her feet are on a solid level platform and
using proper lifting technique with his/her legs. The firefighter should ask for assistance if
he/she is not able to accomplish the task on their own.

S-9-11 Non-Employee Related Incident 9/6/11
Description of Incident: As a contractor was installing new duct for Comcast, their
directional boring equipment came in contact with the street light cable and twisted it
around their equipment, pulling the wires out of several poles along that line.
Supervisor's Comments &Recommendations: The precise point of contact is unknown
as the strike was not discovered until the boring equipment was retracted.

Description of Incident: Multiple members began to stow equipment and Outriggers to
prepare the vehicle to respond to a fire alarm dispatch. Lt. Gutzmer attempted to stow
stabilizer ground pad when the rear outrigger began to move inward toward the chassis.
The stainless steel cover of the outrigger pinched the ground plate, causing the cover to
become bent and tourquing the slide tray downward.
Supervisor's Comments & Recommendations: Members should avoid attempting
multiple tasks near the vehicle during operations. Outriggers should be fully stowed
away/secured and visually confirmed by the operator before other personnel performed any
work, access cabinets, or stow other accessory equipment on that side of the vehicle.

P-27-11/ OFC. MIGUEL MOSQUEDA— POLICE DEPT. 8123/11

Description of Incident: Officer Mosqueda was parking Squad #240 in its parking
space when Squad #240 made contact with the support post next to the parking spot,
causing damage to the right rear door and quarter panel.
Supervisor's Comments & Recommendations: Officer Mosqueda needs to slow down
and pay attention to his surroundings when operating a motor vehicle.

Description of Incident: The Officer lifted the tongue on the S.M.A.R.T. trailer and the
hitch release mechanism did not completely disengage from the ball of the hitch. The
upward motion of lifting the trailer and not properly releasing caused a twinge and pull in
the employee's lower back. The Officer felt increasing pain and discomfort to the lower part
of his back.
Supervisor's Comments & Recommendations: The Officer needs to make sure the
hitch release is fully disengaged before trying to remove it from the vehicle.

SR-03-11 11123 —WATER & SEWER 08/31/11
Description of Incident: While installing the electric service for the new salt barn,
employee was working in the drainage ditch line and made contact with poison ivy.
Supervisor's Comments& Recommendations: Employees need to be required to work
in similar areas on how to identify poison ivy.

Description of Incident: Officer Kuzyk was backing up the identification van in the Public
Works garage and backed into the 2007 Elgin Sweeper truck.
Supervisor's Comments & Recommendations: Officer Kuzyk needs to make sure the
area behind his vehicle is clear before backing up in the future.

P-30-11 OFC. MIGUEL'MOSQUEDA-POLICE DEPT. 91311.1

Description of Incident: Officer Mosqueda was proceeding westbound on Greenleaf
from Busse and attempted to gain entry into the Greenleaf Fire House. He was unaware
that the driveway was closed due to construction, and turned left, placing the driver's side
front and rear tires into the 18 inch wide, 20 inch deep groove in the roadway. The rims on
Squad #242 were damaged and the tires were flattened.
Supervisor's Comments & Recommendations: Officer Mosqueda need to be aware
of his surroundings and where his vehicle is going when he is in control of the vehicle.

Description of Incident: Transponder was last used by Officer Struwing on 9/26/11 and
he recalls returning the transponder to the traffic office on that date. On 8/8/11 traffic
officers were using the transponder issued to patrol. The office and all traffic vehicles were
checked and it was not located.
Supervisor's Comments & Recommendations: A formal check-in/out procedure will
be put in place.
LPC Comments & Recommendations: Concur. A tracking system is in place.
