SUBJECT: Loss Prevention Committee Meeting Minutes—August 22, 2012
The meeting was called to order at 8:30 A.M. at the Village Hall.
Present: Joe Carey, Assistant to the Village Manager
Diane Hunter, RiskNVC Assistant
Pam Kuc, Housing Inspector
Scott Miller, Deputy Fire Chief
David Preissig, Deputy Director of Infrastructure, Chairman
John Nowacki, Deputy Chief
Absent: Karen Turner, Loss Prevention Secretary
The minutes were approved for the meeting of May 18, 2012.
Review Incidents:
• 30 reports were reviewed by the Committee
Issues to monitor as apparent in this incident review included:
1. Overhead doors striking vehicles; consider pressure sensors to be included on all new
overhead door installations (6/10/12 Fire incident)
2. First-year seasonal Public Works employees in vehicle accidents, consider driver
training for first-year seasonals (6/29/12 Public Works incident)
• Driver Refresher Training is recommended for:
1. Matthew FioRito, Sergeant, Police (5/15/12 incident)
2. Neal Hunter, Maintenance Worker, Public Works (7/9/12 incident)
Tabled Reports: None
Old Business:
• Employee names are still being used in narratives of the "On-the-Job Injury Report". Please
remind supervisors to keep narratives "anonymous" for ease of printing and reviewing these
reports.

New Business:
• October is Fire Prevention Month. Fire and Public Works may consider an evacuation drill.
• Next newsletter topic will be written by Joe Carey on "Back Injury Prevention".
Next Meeting:
• Wednesday, October 17th, 8:30 a.m., in Room A.
• Special meeting is scheduled for Tuesday, November 6th, at 8:30 a.m. (Location TBD) with
Sally Bulicek of Gallagher Bassett Services, Inc.
