CONSENT AGENDA A) Consideration of a request (attached) from Mr. & Mrs. Patzelt, 1733 West Biesterfield Road, for a variation from Section 8-8A-3A1 of the Municipal Code to permit a driveway to be constructed of brick pavers. (The Municipal Code requires driveways in residential areas to be constructed of either concrete or asphalt . (The petitioner is proposing to replace the concrete driveway with brick pavers. (It has been determined that the pavers will be capable of supporting vehicle loads using the driveways. (A report from the Director of Engineering and Community Development, recommending approval , is attached. The Village Board has granted similar requests in the past . B) Consideration to authorize the Plan Commission to hold a public hearing on the petition of Super 8 Motel for a Special Use Permit to construct a 58 room hotel at 2951 Touhy Avenue. (The proposed hotel would be located on industrial zoned property in Elk Grove along Touhy Avenue. Hotels/motels require special use permits to be located on industrial zoned property. Agenda for VB Melling 9-27-94 -2- (The property in question is near the existing Excel Inn on Touhy and is immediately adjacent to the property in the city of Chicago, which is under review for consideration to construct a concrete reprocessing facility. ( No date has been established for the public hearing before the Plan Commission. ) C) Consideration to award a contract to the lowest responsive and responsible bidder, R. W. Hendrickson Company, Des Plaines, IL, for pruning and trimming parkway trees, in an amount not to exceed $20, 760. (Approximately 2,000 parkway trees are to be pruned at a cost of $10. 38 per tree. Hendricksen has performed similar work in the past for the Village. (The work involved in this contract is for high branch trimming. Public Works does not have the equipment to reach the high branches on the mature trees in the Village. (Sufficient funds are budgeted and available for this expenditure. (Reports from the Purchasing Agent and Public Works Director, recommending approval, are attached. ) D) Consideration to award a purchase contract to the lowest responsive and responsible bidder, Akzo Nobel Salt, Inc. , Clarks Summit, PA, for the purchase and delivery of rock salt in an amount of $25. 87 per ton for a total cost not to exceed $82, 784 . (Akzo Nobel Salt, Inc. , was awarded the contract for rock salt under the joint purchase agreement with the State of Illinois. (The Village does not expect any salt shortages this year. There is approximately 1 ,000 tons of salt in our current reserves that will be added to the salt purchased this season. This amount should be sufficient for snow removal operations. (The Village is required to purchase 700 of the amount ordered or 2,240 tons at a price of $57,948. 80. Sufficient funds are budgeted eted and available for 9 this expenditure. (Reports from the Purchasing Agent and Director of Public Works recommending approval, are attached. ) E) Consideration to award a purchase contract to the lowest responsive and responsible bidder, Piper Industries for 10,000 - 18-Gallon Recycling Bins at a cost not to exceed $3 . 40 per bin for a' total contract price of $34, 000. (The Recycling Committee examined the bins e' Agenda for VB Me�ing 9-27-94 it -3- proposed by the lowest bidder and second lowest bidder. The Committee recommended purchasing the bin from Piper Industries, the lowest bidder. (The recycling bins will be delivered to all single-family homes in the Village before November 1 . Information on the Village 's increased recycling program will be included in each bin. (A report from the Recycling Committee was previously distributed. A report from the purchasing agent is attached. ) F) Consideration to adopt RESOLUTION NO. 48-94 (attached) authorizing the Village President to execute an agreement between the Village of Elk Grove and Barton-Aschman Associates Inc. , for traffic engineering services at a cost plus fixed fee of $3, 670. (The agreement provides for traffic engineering services at three intersections : Biesterfield Road/Wise Road and Michigan Lane; Meacham Road and Nerge Road; and, the intersection of the major north/south access aisle of the Mallard Crossing Shopping Center and Nerge Road. (Barton-Aschman provided the lowest quote of $3, 670. Barton-Aschman specializes in traffic feasibility studies and has experience in the analysis and design of traffic signals. (The cost to perform the Meacham Road and Nerge Road intersection and the Mallard Crossing/Nerge Road intersection will be shared equally between the Village and Hamilton Partners. The cost for this portion of the study is $2, 430. As part of the Mallard Crossing Annexation Agreement, a traffic consultant was to be retained 6 months to 1 year after the opening of Mallard Crossing and the Elgin/O 'Hare Expressway to study these intersections. The traffic consultant will determine if any traffic improvements are needed due to these new developments. Hamilton Partners has agreed to pay their share of this study ($1 ,215) . (The study of the Biesterfield/Wise/Michigan intersection was awaiting the completion of the right-hand turn lane from southbound Meacham to westbound Biesterfield. That improvement has been completed. (A report from the Director of Engineering and. CONSENT AGENDA (continued) G) Consideration of the following: -- To waive the formal bidding process; and, -- To award a contract to (ACRS) American Concrete Raising Incorporated, Bensenville, IL to perform slab jacking of the multi-purpose/training room floor in an amount not to exceed $11 ,000. (At the August 9 Village Board meeting the contract for remodeling the Fire Administration building was approved. The contract, however, did not include expenses for the removal and replacement of the multi-purpose/training room concrete floor estimated at $43, 480. In the alternative, slab jacking of the floor will be performed at a significant cost savings of $32, 480. (MTI, the general contractor for this project, solicited a request for proposal for the slab jacking project. ACRI was the only contractor to respond out of 4 contractors. Follow-up calls to the contractors did not result in more responses. (In the Sub-Trades expenditures that were approved by the Village Board on August 9, 1994, $13,280 was approved for concrete work. This amount will be used to cover the $11 ,000 for the slab jacking project .
