CONSENT AGENDA A) Consideration of a request (attached) from Alexian Brothers Medical Center to waive the $100 business license fee for the retail sales conducted in its outpatient pharmacy. (It has been the past practice of the Village Board to waive the business license fees for not-for-profit agencies. (A report from the Finance Director recommending approval is attached. ) B) Consideration to award a purchase contract to the lowest responsive and responsible bidder, Gem Construction, Wood Dale, IL, for the removal and replacement of sidewalks, driveway aprons, curb and gutter under the Village 's Private Participation Concrete Replacement Program at a per unit cost not to exceed the following: Agenda for VB M16-ting 6-14-94 -2- Utast Pricing. Approximately $20,000 of the work bid is for the Village; the remaining $169,235 is estimated work for residents and businesses which will be collected prior to commencement of the replacement work. (To date, about 300 residents and businesses have expressed an interest in the program. The cost for the program is somewhat higher this year due to increases in the price of concrete over the past year. (A report from the Director of Public Works recommending approval is attached. ) C) Consideration to award a contract to Patrick Agenda for VB M ing 6-14-94 -3- Engineering, Inc. , Glen Ellyn, IL, to study the HVAC system in the Police Department area at a cost not to exceed $6,500. (The current HVAC system has deteriorated to the point where attention is required each day to ensure proper operation. Equipment failure occurs frequently especially during the warm months. This causes the building to become too cold or too warm. This study will evaluate the current system and give recommendations for improvement and/or replacement . (Six consultant firms were sent requests for proposal . Of the four firms that responded Patrick Engineering was found to be the most qualified based upon experience, knowledge and understanding of the project. (Sufficient Funds were included in the budget for this expenditure as well as the replacement of the BVAC system. (A report from the Public Works Director, recommending approval is attached. ) D) Consideration of the following: -- to waive the formal bidding process; and -- to award a sole source maintenance contract to Motorola, Inc. , Schaumburg, IL, for the maintenance of the KDT (Computer-sided dispatch) equipment used by the Fire and Police Departments in an amount not to exceed $10,256.40. (Motorola is the only source for spare parts, repairs, and technical assistance for the mobile data terminals (KDT Units) in the Police and Fire vehicles. (Reports from the Purchasing Agent, Police Chief and Fire Chief, recommending approval, are attached. ) E) Consideration to extend a maintenance contract with Aldridge Electric to provide maintenance of Village-owned traffic signals and overhead street lights for a period of May 1 , 1994 to April 30, 1995 in the amount of $6,454 . 80. (The low bid contract awarded to Aldridge Electric last year included two (2) one-year options to extend the contract. Based upon the excellent service provided by the company, it is recommended that the first one-year option be accepted by the Village. The cost for the option is the same as last year. This contract will be in effect from May 1 , 1994 until April 30, 1995. (The contract covers the maintenance of Village- owned traffic signals at six intersections. In addition, the contract includes the maintenance of Agenda for VB Muting 6-14-94 -4- twenty-seven overhead street lights on Touhy and Elmhurst Roads. (Sufficient funds are included in the 1994-95 Fiscal Year Budget for this expenditure. (A report from the Purchasing Agent and Director of Public Works recommending approval is attached. F) Consideration to concur with the previous authorization of the Village Manager to allow a one day private church tent party at Puskar Tools, 2301 Estes on Sunday, June 5, 1994 . (A report on this item was distributed previously. ) G) Consideration of the following: -- to waive the formal bidding process; and, -- to renew a service contract with Chicago Communications Service, Inc. , to maintain Motorola radio equipment for the Police, Fire and Public Works Department in the amount of $22,863. 52. (Chicago Communications has maintained the Village 's radio contract since 1984 . Chicago Communications maintains the equipment at 75% of Motorola 's pricing while offering the same contract terms and conditions. (The contract price reflects a 3% increase over last fiscal year which was the same price as the 92-93 fiscal year. (Sufficient funds are budgeted and available for this contract. (Reports from the Purchasing Agent, Fire Chief, Police Chief and Director of Public Works, recommending approval are attached. ) H) Consideration to reject all bids submitted by P.R. Streich and Sons for a sump pump system for the truck rack pit located at the Fleet Services Facility, 1635 Biesterfield Road. (Bids were requested for an explosion proof pump at the Fleet Services Facility. There is a budgeted amount of $10,000 for this purchase. P. R. Streich & Sons was the only company to submit a bid. This bid exceeded the amount budgeted by $2,850. (Upon analyzing this bid, it was found that a surface-mounted pump could be used in lieu of the explosion proof model originally bid. (Based on the high bid price, sole bidder, and possible alternatives, it is recommended that the Village Board reject the bid in order to re-define Agenda for VB Ming 6-14-94 -5- specifications and rebid the sump pump system at a later date. (Reports from the Purchasing Agent and the Director of Public Works recommending rejection are attached. I) Consideration of the following: -- to waive the formal bidding process; and -- to award a contract to Systems and Analysis Group, Inc. (S.E.A. Group) of Schaumburg, IL, for the purchase and installation of Level 5 cable runs, upgrade of the Novell 3. 11 SFT III software to a 50 user capability, and one additional 24 port Network hub in an amount not to exceed $7,077. (The S.E.A. Group provided excellent performance through the first phase of installation of the Police Department PIMS III network system which included the designing, cabling, and definition of specific equipment requirements. This contract represents the continuance of the phasing in of the PIMS III project. Because of S.E.A. 's performance and familiarity with the project it is advantageous to utilize the same vendor throughout this process. For this reason it is recommended the formal bidding process be waived. (Sufficient funds are budgeted and available for this expense. (Reports from the Purchasing Agent and Police Chief, recommending approval, are attached. ) J) Consideration to award a purchase contract to the lowest responsible and responsive bidder, Mid-County Equipment, Libertyville, IL, for a John Deere, Model 955 utility trailer in an amount not to exceed $12,858, including the trade-in value of a 1979 Dew Eze Utility Tractor at $1 ,000. (Funds are included in the budget for the equipment replacement . (Reports from the Purchasing Agent and Director of Public Works recommending approval is attached. ) K) Consideration of the following: -- to waive the formal bidding process; and, -- to renew a service contract with Decision Data Service, Inc. , of Itasca, IL, to provide Maintenance of the IBM AS/400 system and related computer hardware an amount not to exceed $9,206. 88. Agenda for VB Ming 6-14-94 -6- (The Village Board awarded the original contract for the period May 1 , 1993 to April 30, 1994 with the option to annually extend the contract for (2) one-year periods. The total contract cannot exceed 3 years. (The base contract pricing for each unit of equipment is the same as last year. (A report from the Purchasing Agent, recommending approval, is attached. ) L) Consideration to authorize Police Chief, Fred Engelbrecht, to attend the International Association of Chiefs of Police Annual Conference in Albuquerque, New Mexico, from October 15, 1994 through October 20, 1994 at a cost not to exceed $1 ,218. (Sufficient funds are budgeted and available for this request. ) (By registering now, the Village will receive a discounted registration rate. ) M) Consideration to authorize Deputy Chief Gregory T. Riddle, to attend the Association of Public Safety Communications Officials National Conference in Pittsburgh, Pennsylvania, from August 6, 1994 through August 12, 1994 at a cost not to exceed $1 ,352 .20. (Sufficient funds are budgeted and available for this expenditure. (By registering now, the Village will receive an early registration discount . ) N) Consideration to renew a purchase contract with Cintas Uniforms, Schaumburg, IL for a one-year period from May 1 , 1994 through April 30, " 1995 for Public Works uniforms and accessories in an amount not to exceed $42,000. (Last Fiscal Year the Village Board awarded a three-year contract for Public Works uniforms, subject to annual renewal . The contract amount and accessories is the same as the previous fiscal year. (Cintas Uniforms has contracted with the Village for these services since the mid-eighties. Cintas Uniforms provides uniform service to 52 Public Works employees as well as linen products, floor mats, dust mops, towels and other accessories to Public Works and Fire Departments. Cintas met Village expectations. (Sufficient funds are budgeted and available for this expenditure. (Reports from the Purchasing Agent, Public Works Director, and Fire Chief recommending approval -are attached. Agenda for VB Ming 6-14-94 -7- O) Consideration to award a purchase contract to the lowest responsive and responsible bidder for one 1995 Ford LN8000 truck chassis with truck mounted server cleaner to Sewer Equipment Company of America, Glenview, IL in an amount not to exceed $67,485 . 00, including the trade-in of a 1979 Ford LN800 at a value of $12, 717. (Bid specifications were furnished to six (6) potential bidders of which three bids were received. The Public Works Department reviewed and evaluated the bids and determined that the Sewer Equipment Company of America, Glenview, IL had the lowest responsible and responsive bid. (Specifications required a self-cancelling turn signal steering column which was not included in the bid price. However, this can be installed for an additional $450. 00 to the bid price. Sewer Equipment Company of America remains the lowest bidder with that addition. (A 1979 Ford LN800 valued at $12, 717 is included in the bid price which brings the total cost to $67, 845 . This represents a total of $20,155 below the amount budgeted. (Reports from the Purchasing Agent and Director of Public Works, recommending approval, are attached. ) P) Consideration of the following: -- to adopt RESOLUTION NO. 30-94 (attached) authorizing the Village President to execute an agreement between the Village of Elk Grove Village and Christopher B. Burke Engineering, LTD. , to provide engineering services for the FEMA floodplain and floodway map revision at a cost of $18,000; and -- to increase the Department of Engineering and Community Development budget by $18,000 to cover the expenditure of the study. (Several homes developed by Centex Homes and Terrestris Development in the mid eighties were constructed on lots that were elevated out of the floodplain. Since that time the Village in conjunction with these developers has requested numerous times to have the Federal Emergency Management Agency (FEMA) amend the Flood Insurance Rate Maps to remove these lots from the floodplain. The Village was not able to resolve the issue with FEMA until we requested help from our legislators to intervene on our behalf. With that help the Village now has only one more step to complete for a FEMA map revision. Agenda for VB Ming 6-14-94 -8- performed to secure the requested map amendment . Christopher B. Burke will perform the study and prepare the documentation for submittal to FEMA. Burke has successfully obtained FEMA map revisions for other municipalities and has the expertise to perform the work. (Burke will perform the work on a time and materials basis at a total fee of approximately $18,000. Centex Homes has agreed to contribute $5,000 to the Village for this expenditure. Village commitment would be the remaining $13,000. (Since the need for a hydraulic study was not known until recently the study was not budgeted in the 94-95 budget . A budget increase of $IB,000 is, therefore, necessary for the study. ) Q) Consideration to adopt RESOLUTION NO. 31-94 (attached) ascertaining the prevailing rate of wages for workers on public works projects. (The resolution is to be filed with the Illinois Department of Labor and Secretary of State in the month of June each year. ) R) Consideration of the following: -- to adopt ORDINANCE NO. 2371 (attached) repealing RESOLUTION NO. 24-94 authorizing an annexation agreement between the Village of Elk Grove Village and Lexington Homes; and ORDINANCE 2365 annexing certain property to the Village of Elk Grove Village (Lexington Homes) ; -- to adopt RESOLUTION NO. 32-94 (attached) authorizing the Village President to execute an annexation agreement between the Village of Elk Grove Village and Lexington Homes; and -- to adopt ORDINANCE NO. 2372 (attached) annexing certain property to the Village of Elk Grove Village (Lexington Homes) . (This action will reconfirm the annexation given proper notice to the Roselle Fire Protection District . (A report on this item was previously distributed. ) S) Consideration to adopt ORDINANCE NO. 2373 (attached) Accounting Order Ordinance. (At the January 25, 1994 Village Board meeting a resolution holding in abeyance the effective date of Continental 's Cablevision basic tier rates was adopted. That abeyance expires on June 27, 1994 . Agenda for VB Ming 6-14-94 This was done in order to have enough time to review the "cost of service" justification Continental Cable used to determine their basic tier rates. (At the May 24, 1994 Village Board Meeting the Board gave authorization to hire Crowe Chizek to perform this review. Crowe Chizek 's analysis will enable the cable consortium to determine if basic cable rates are acceptable or if they should be "rolled back" based upon FCC standards. A review and decision by the consortium will not be completed by June 27, 1994 . (This ordinance requires Continental to keep an accurate account of financial activity on their proposed basic tier rates effective June 27, 1994 until such a decision is made. ) 7. REGULAR AGENDA A) Consideration of the following request (attached) : -- To allow the Boy Scouts of America to hold a fundraising carnival on June 15 through June 19 in a portion of Wal-Mart parking lot; and -- To waive the following permit fees for the event : -- Food Caterers License. . . . . . . . . $100 -- Business License Fee. . . . . . . . . .$500 -- Electrical Inspection Fee. . . . . $ 25 TOTAL $625 (The carnival is requested to operate between 6 p.m. and 11 p.m. Wednesday and Thursday, 6 p.m. to midnight on Friday, 1 p.m. to midnight on Saturday and 1 p.m. to 10 p.m. on Sunday. No liquor will be sold at the carnival . There will be one food stand that will sell popcorn, corn dogs and soft drinks. (The carnival will use approximately 113 of the Wal-Mart parking lot. The Director of Engineering and Community Development foresees no problem with parking. (Staff recommends granting the request conditional upon the carnival ceasing operations at 11 p.m. or earlier each night of the event . (A report from the Administrative Assistant is attached. ) B) Consideration to adopt ORDINANCE NO. 2374 (attached) designating certain real property located at the southwest corner of Arlington Heights Road and Biesterfield Road as a blight area under Section 8-13-1 Et. Seq. of the Village Code of the Village of Elk Grove Village.
