ZETTEK MUNICIPAL BUILDING, 901 WELLINGTON AVENUE ON TUESDAY APRIL 9,2013 Mayor Craig B. Johnson called the meeting to order at 5:23 p.m. 1. ROLL CALL: Present: Mayor: Craig B. Johnson; Trustees: Nancy J. Czarnik, Patton L. Feichter, Jeffrey C. Franke, Samuel L. Lissner, James P. Petri, Christine K. Prochno Also Present: Village Manager Raymond R. Rummel; Village Attorney George B. Knickerbocker; and Village Clerk Judith M. Keegan A quorum was present. 2. PLEDGE OF ALLEGIANCE /INVOCATION—The Invocation was delivered by Jerry Szostak, Queen of the Rosary Church. Mayor Johnson requested that all Village Trustees stay after the Board meeting to discuss homeland security issues. 3. APPROVAL OF MINUTES Motion to approve the minutes of the Regular Village Board Meeting held on March 26, 2013 as submitted and dispense with the reading. REPORT OF THE MAYOR AND BOARD OF TRUSTEES Elk Grove Business Park - Mayor Johnson reported that the first quarter vacancy rate in the business park has dropped to 8.6%. The vacancy rate was at 9.8% at the end of 2012 and 12.26% at the beginning of 2011. Approximately one half million square feet of space has been occupied so far this year. A new teardown and rebuild is taking place at Nicholas Boulevard and Lunt Avenue. The following are some of the new businesses that recently moved to Elk Grove Village: Creative Werks, IMS Buhrke-Olson and CHEP Aerospace. Additional teardown and redevelopment in the Village include Duke Development at 1300 Busse Road, Bridge Development at 2475 Touhy Avenue, Panattoni Construction Inc. at 1925 Busse Road, Prologis at 2299 Busse Road and CenterPoint at 1601 Busse Road. Road Construction—Mayor Johnson gave warning of upcoming road construction projects starting in the near future. Notice of construction is in the Village Newsletter, on the Village website, on Channel 6 and notification will be hand-delivered to residents and businesses located along the affected roads. Projects include the rehabilitation of Biesterfield Road from Arlington Heights Road to Martha Street, Elk Grove Boulevard and JF Kennedy Boulevard Crosswalk Rehabilitation and Arlington Heights Road Reconstruction from Higgins Road to Brantwood Avenue. Several residential streets are also going to be resurfaced. The Village also just received word that the Devon Avenue Bridge over the highway is going to be worked on. 1 VBM 4-9-13 Reappointments of Board and Commission Members—Mayor Johnson asked for Village Board concurrence of the following reappointments: Fire & Police Commission—Michael P. MOVED: Trustee Feichter SECONDED: Trustee Petri AYES: Trustees Feichter, Franke, Lissner, Petri, Prochno, Czamik NAYES: None MOTION CARRIED Mayor Johnson thanked everyone on the Boards and Commissions for volunteering their time to the Community. Trustee Czamik noted the Spring Banners are on display. Trustee Czarnik stated Packages 4 Patriots is sponsored by the Lutheran Church of the Holy Spirit. Packages 4 Patriots meets on the third Tuesday of every month to put together packages to mail to military personnel. Tyler Juckett, currently stationed in South Korea, received a package and sent a note expressing his thanks for the package and stated how heartwarming it is to receive random acts of kindness from back home.. Trustee Czarnik stated there are four$1,000 scholarships being offered from the Frank J. Czamik Memorial Scholarship Fund. Applications are available at Elk Grove High School, Conant High School, the Park District Office and the Pavilion. The scholarships are available for Elk Grove Village residents graduating from high school this year and going on to college. Applications are due May 10, 2013. Trustee Lissner congratulated Trustee Prochno, Trustee Feichter, Trustee Franke and Mayor Johnson on being re-elected. Mayor Johnson noted with the exception of Trustee Franke joining the Board when Trustee Dill left, that the Village Board has been together for eighteen years. Trustee Feichter stated students from Mead Junior High attended the Community Character Coalition meeting last Tuesday. The students were part of the Rotary Club. The group is called Interact. The students presented a video and also talked about the work they had done throughout the past year. The Community Character Coalition was so impressed with what they had accomplished that they would like them to become members of the Coalition. 2 VBM 4-9-13 The Community Character Coalition student recognition program will be taking place at Village Hall on May 14, 2013. Exhibits will be on display before the Village Board meeting and then students will be recognized at the Board meeting. Trustee Prochno volunteered at the Park District and Community Character Coalition's Celebration of Cultures on Friday, April 5, 2013. There were over sixteen tables representing different activities. Trustee Prochno worked the table representing the Italian Sister Cities. They had an indoor bocce ball game. Many cultural events were represented. It was an informative and entertaining evening. The Elk Grove Parks Foundation is conducting a fundraiser April 15th to 19th called"Fit to Give." For a$10.00 donation to the Elk Grove Parks Foundation you can participate in all the fitness and wellness "Fit to Give"programs and activities throughout that week. Included are all group exercise and cycling classes, all yoga and pilates classes,use of the indoor track, workshops, screenings and special classes. Registration can be done on-line or at the Park District. All proceeds support the Elk Grove Parks Foundation's scholarship program which provides funding for individuals who otherwise could not afford vital park district programs such as preschool, before and after care, and swim lessons. Trustee Prochno is a member of the Lioness Club. The Lioness Club, Lions Club and Leo Club are all charitable social organizations that help to make the community better. Lioness Club members enhance communities, acquire new skills, cultivate lasting friendships, meet challenges head on and have a part in changing people's lives. Members pride themselves in helping support those who are sight or sound impaired. Membership in the Elk Grove Village Lioness Club is open to anyone who is interested in helping their community and having fun with new and old friends. Monthly meetings are held on the second Monday of each month at 6:45 p.m. at the Black Steer Restaurant, 1180 W. Devon Avenue. Trustee Franke announced the thirteenth CERT class will be starting on April 19, 2013. Class will be held Friday the 19th, Saturday the 20th and will finish up the following Saturday, April 27, 2013. Anyone interested in signing up for the class should contact Officer Carl Sabo. 5. APPROVAL OF WARRANT Motion to authorize payment of invoices as shown on the March 31, 2013 Accounts Payable Warrant in the amount of$563,280.69. SunGard Public Sector, Inc., Lake Mary FL—Authorization to award a professional service contract for support and maintenance of our HTE,software applications and custom program modifications for the period from May 1, 2013 through April 30, 2014 in an amount not to exceed $44,944. F. Brancato Landscaping, Inc. Elk Grove, IL—Authorization to award a landscape services contract to the lowest responsive and responsible bidder for the 2013 Parkway Turf Restoration in the amount of$133,000. G. J. Congdon Sewer Service, Inc., Carol Stream, IL—Authorization to award a professional services contract to the lowest responsive and responsible bidder for the Kent Avenue and Devon Avenue Watermain Improvement project in the amount of$285,393.50 from the Water/Sewer Fund. H. Avalon Petroleum, Tinley Park, IL—Authorization to renew a gasoline and diesel fuel delivery.contract for one year in the amount of$493,000. I. Authorization of the following: • To award a purchase contract through the State of Illinois CMS Joint Purchasing Contract to the lowest determined bidder, Wright Automotive of Hillsboro, IL for the purchase of 7 Dodge Avenger inspector vehicles and 2 Ford Explorer vehicles to be paid from the FY 2013-14 budget at a total cost of$173,496 from the Capital Replacement Fund; and • To award a purchase contract from the Suburban Purchasing Cooperative SPC Joint Purchasing Contract to the lowest determined bidder, Currie Motors Ford of Frankfort, IL, for the purchase of 2 Ford Interceptor Sport Utility Vehicles to be paid from the FY 2013-14 budget at a total cost of$58,758 from the Capital Replacement Fund. Health Insurance - Authorize the Village Manager to execute contracts renewing the Village's health insurance programs as of May 1, 2013: • To execute a contract with Blue Cross/Blue Shield to provide medical services and to establish the Monthly Premium Rates for the Blue Cross/Blue Shield Self-Insured Medical PPO as follows: Participation Level Monthly Premium Total Employee Only $950.55 Employee Plus One Dependent $1,996.23 .Employee Plus Two Dependents $2,171.92 Medicare Supplement Rates One Person $556.63 Two Persons $1,113.26 • To execute an agreement with Blue Cross/Blue Shield for claims administration at a cost of$70.78 per participant, per month, $261,603 annually; • To execute a Blue CrossBlue Shield High Deductible Plan to provide medical services and to establish the Monthly Premium Rates for the Blue Cross/Blue Shield Self-Insured Medical PPO as follows: Participation Level Monthly Premium Total Employee Only $762.21 Employee Plus One Dependent $1,600,67 Employee Plus Two Dependents $1,741.55 Medicare Supplement Rates One Person $446.35 Two Persons $892.70 • To execute an agreement with Blue Cross/Blue Shield for claims administration at a cost of$70.78 per participant, per month; • To execute a contract with HMO Illinois to provide base plan HMO medical benefits at the following monthly rates: Participation Level Monthly Premium Total Employee Only $654.27 Employee Plus One Dependent $1,263.02 Employee Plus Two Dependents $1,939,36 Medicare Supplement Rates One Person $527.85 Two Persons $1,055,70 • To renew a contract with Guardian Dental HMO to provide HMO Dental benefits at the following monthly premium rates: Participation Level Monthly Premium Total Employee Only $32.30 Employee Plus One Dependent $60.04 Employee Plus Two Dependents $90.37 • To establish the following Monthly Premium Rates for the Village's Self-Insured Dental Plan: Participation Level Monthly Premium Total Employee Only $72.28 Employee Plus One Dependent $151.78 Employee Plus Two Dependents $190.26 5 VBM 4-9-13 • To renew an agreement with HealthSmart (formerly American Administrative Group) to administer the Village's Self-Insured Dental Plan at a cost not to exceed $5.20 per participant,per month, for Dental Claims at $19,219 annually; • To renew a contract with Symetra Financial to provide Specific Stop Loss Re-Insurance at a cost not to exceed $935,294; • To renew the Flexible Spending Plan administered by HealthSmart at a cost of$4.65 per participant,per month, adding $1.25 for a debit card; • To renew a contract with Gallagher Benefit Services, Inc., who receive 3% of the HMO premium from Blue Cross/Blue Shield, $6 per employee, per month of the Blue Cross/Blue Shield Illinois PPO Admin Fee and a graded percentage on premium volume from Hartford. Limitation of Liability has changed from an unlimited amount to the aggregate liability under this Agreement, if any, of either party to the other for claimed losses or damages shall not exceed $20,000,000. This provision applies to the fullest extent permitted by applicable law. Such change is still well above the market; and • To renew the COBRA Administration by HealthSmart at a cost of$1.75 per participant, per month. M. RECYCLING & WASTE COMMITTEE—CHAIRMAN FEICHTER—On Saturday, June 22, 2013 the Village will be hosting a document destruction event and electronic recycling event at Fire Station 8 on Oakton Street between the hours of 9:00 a.m. and noon. 12. INFORMATION COMMITTEE - TRUSTEE PROCHNO—No Report 13. INDUSTRIAL/COMMERCIAL REVITALIZATION COMMISSION - TRUSTEES LISSNER/PETRI—No Report 14. YOUTH COMMITTEE - TRUSTEE FRANKE - The Youth Committee will be awarding three $1,000 scholarships. One will be awarded by the Parks Foundation and two will be awarded by the Village. Applications are available at the high schools, Village Hall, Pavilion and Park District Office. Applicants must be a resident of Elk Grove Village and attending College or Trade School in the fall. Applications are due May 3, 2013. The recipients will be announced on May 28, 2013. Mayor Johnson encouraged everyone to apply. The Rotary Club offered a$500 scholarship for Junior College attendance and no one applied for it. 15. PARADE COMMITTEE•-TRUSTEE CZARNIK-The annual Hometown Parade is going to be on Saturday, June 15, 2013. The parade theme is "Sports on the Boulevard." Parade applications were mailed and responses are being received. The 2013 parade Grand Marshal is Don Kerstin. Don supports many charitable efforts with his time, talent and good works. Sponsors for this Year's parade are Grove Memorial Chapel, Chicago Offset Printing, Elk Grove Chamber of Commerce, Jerry Castle & Son Hi-Lift Inc., and A&H Plumbing. The next meeting of the Parade Committee is the first Monday in May at 5:30 p.m. 16. PLAN COMMISSION- VILLAGE MANAGER—No Report 17. ZONING BOARD OF APPEALS -VILLAGE MANAGER—No Report 18.
