6. CONSENT AGENDA
A) Consideration of a request (attached) from
Alexian Brothers Medical Center to waive the 1993-1994
business license fee for general retail sales and film
processing in their outpatient pharmacy in the amount _
of $100. 00.
(It has been the past practice of the Village
Board to waive license fees for other not-for-profit
agencies.
(A report from the Director of Finance is
attached. )
B) Consideration to increase the contract with Lo Verde_
Construction for roadway restoration in the amount of
$3,950 for a total amount not to exceed $29,514 .
(The Village Board awarded a contract to Lo `I
Verde Construction to perform sanitary sewer repairs
in conjunction with the enclosure of ,the drainage
ditch on November 24, 1992 . This contract was . carried
over into the current fiscal year for completion
(The $3,950 increase covers repairs for the
roadway excavation or pavement shifting that was
performed at three points of sanitary sewer along the

westbound lane of Elk Grove Boulevard. This work was
unforeseeable at the time of the original contract .
(Sufficient funds are available for this
increase.
(A report from the Director of Public Works is
attached. )
C) Consideration of a request from First Baptist
Church, 590 Tonne Road, to waive the $10 electrical
permit fee and $25 sign permit fee for a permitted
electrical sign.
(It has been the past practice of the Village
Board to waive the permit fees for not-for-profit
agencies.
(The Director of. Engineering recommends
approval of the request . )
D) Consideration to award a purchase contract to the
lowest responsive and responsible bidder under the
Illinois State Joint Purchasing Agreement to Miles
Chevrolet to furnish ten (10) patrol vehicles and
three (3) unmarked vehicles in an amount not to exceed
$192, 020.58 .
(The low bid State contract provides for a total
cost per vehicle of $14, 713 . 38 for patrol squads and
$14 ,962 .26 for unmarked squads. This price is better
than the bid prices obtained from the Northwest
Municipal Conference.
(Sufficient funds are budgeted and available for
. this expenditure.
(Reports from thePurchasingAgent, Chief of
Police and Director of Public Works recommending-
approval, are attached. )

E) Consideration of the following:
-- to waive the formal bidding process; and
-- to award a contract to Wirfs Industries,
McHenry, Illinois, to perform repairs on, Aerial
Ladder Unit- #138 in an amount not to exceed
$9,500.
(During a recent inspection of the Fire
Department truck, the ladder was found to contain
stress cracks. In addition, major repairs are needed
on the tiller suspension and steering components. The
repairs are necessary to restore the structural
integrity of the unit . %
(Due to the limited amount of vendors capable of
performing this work, two estimates were obtained by
Public Works Fleet Services. Wirfs has performed
similar repairs in Niles, IL and Wisconsin. In

addition, the truck is currently out of service and
unable to respond to service calls.
(Finally, it is recommended to perform the
repairs on the existing unit as a replacement unit is
estimated to cost $425,000.
(Sufficient funds are available for transfer in
the budget for this expenditure .
(A report from the Fire Chief recommending
approval is attached. )
F) Consideration of a request (attached) from the Elk
Grove Park District to waive the $25 alarm license fee
for the period 1 /1 /94 through 12/31 /94 for the
following Park District facilities:
Al Hattendorf Senior Citizen Center
Al Hattendorf-Recreation Center
Elk Grove Historical Society Museum
Elk Grove Park District - Administrative Offices
Elk Grove Park District Warehouse
Elk Grove Teen Center
Elk Grove TV Studio
Fox Run Golf Course
for a total amount of $200. 00
(It has been the past practice of the Village
Board to waive the license fees for other governmental
agencies.
(A report from the Director of Finance is
attached. )
G) Consideration of a request (attached) from School.
District 54 to waive the $25 alarm license fee for the
period 111194 through 12131194 for the following
schools:
Adlai Stevensen Elementary School
Margaret Mead Junior High School
for a total amount of $50. 00
(It has been the past practice of the Village
Board to waive the license fees for other governmental
agencies.
(A report from the Director of Finance is
attached. )
H) Consideration of a request (attached) from PRP Wine
International , 1701 Howard Street, Suite C, to hold a
warehouse sale on Saturday, December 18, from 10 a .m.
to 4 p.m.
(Village Board approval was granted to PRP last /

year for a similar sale. No problems were
encountered. The Director of Engineering and
Community Development recommends approval of the
request . )
I) Consideration to adopt RESOLUTION NO. 72-93
(attached) authorizing the Village President to
execute an amendment agreement between the Village of
Elk Grove Village and the Suburban Bus Division (PACE)
of the Regional Transportation Authority (Dial-A-Ride) .
(This item was _discussed at the 11123193
Committee of the Whole. It amends the PACE share of
the Dial-A-Ride subsidy from $2 .50 to $2 . 00 per rider
effective January 1 , 1994 . )
J) Consideration to adopt RESOLUTION NO. 73-93
(attached) adopting the Village of Elk Grove Village
Safety Manual .
(The Village Safety Manual sets forth general
Village policy for safe work habits.
(It was discussed by the Personnel Committee on
December 1 and recommended for approval by the Village
Board.
(The Safety Manual was distributed previously) .
K) Consideration to adopt ORDINANCE NO. 2334
(attached_) amending Title 3, Chapter 19, Section 6, of
the Village Code of the Village of Elk Grove Village
(Alarm Systems) .
(The ordinance brings the proration of our Alarm
license fees in line with other Village licenses that -
are prorated. Currently the alarm licenses are
pro-rated on a quarterly basis. All other licenses
are prorated on a semi-annual basis. The ordinance
will provide for a consistent policy on prorating fees
by placing alarm license prorations on a semi-annual
basis.

recommending approval, is attached. )
L) Consideration to adopt ORDINANCE No. 2335
(attached) amending the position classification and
salary plan of the Village of Elk Grove Village .
(The ordinance provides for the temporary
appointments of up to 4 firefighters. The temporary
positions will terminate through attrition .
(A report on this item was previously
distributed. )
M) Consideration to adopt ORDINANCE No. 2336
abating a portion of the 1993 Property Tax levy for

General Obligation Bonds, Series 1987, and certain
refunded bonds of Series 1985, and 1989 and 1991 .
(The levy of $606,503 and $543, 575 will be abated
for the portion of Bonds - Series 1985, Series 1989 &
Series 1991 , refunded by the bonds issued in 1992 and
1993 . The levy of $713, 125 for the Bond Series 1987
will be abated by payments received from members of
the High-level Excess Liability Pool (HELP) . The
amount of $483,223 for the library 's portion of
General Obligation Bonds will be abated for debt
service funds which the library reimburses the
Village. )
N) Consideration to adopt ORDINANCE No. 2337, Tax Levy
Ordinance, in accordance with Public Act 82-102 :
Truth in Taxation.
(A report from the Director of Finance was
previously distributed. )
O) Consideration to adopt ORDINANCE No. 2338
(attached) amending Title 3, Chapter 3, Section
3-3-7, number of licenses of the Village Code of the
Village of Elk Grove Village by increasing the
number of Class "D" Liquor Licenses - (Giappo 's
Pizza) .
(The. ordinance allows Giappo.'s Pizza, 642
Meacham, t6 obtain a Class "D" liquor license to sell
beer and wine on the premises. )

A. Pending Action by Village Board
1 . At the 9114193 Village Board meeting an ordinance
amending Day Care Centers was referred to
the JPZ Committee for review. The ordinance amends
Title 3, Chapter 8, Article A, Day Care Centers and
Homes, of the Village Code of the Village of Elk
Grove Village as it pertains to maximum number and
age of children in day care homes.
2 . At the 11123193 Village Board meeting action was
deferred for a sign variation request from Burton
Grove Apartments to allow one 90 sq. ft .
freestanding sign and one 78 sq. ft . monument sign.
