CONSENT AGENDA A) Consideration of the following: To approve the final pay estimate to Gerardi Sewer and Water for the Phase I Drainage Improvements Project in an amount of $12,858.21; and To approve the Balancing Change Order for the Phase I Drainage Improvements project for a net decrease of $25, 644 .51, from the contract price. (Gerardi Sewer and Water completed the project at a total cost of $182, 742.19, which is $25,644 .51, under the contract price.. (A report from the Director of Engineering and Community Development, recommending approval, is attached. ) B) Consideration to increase a contract awarded to Donohue and Associates on May 12, 1992, for the Fire Administration Headquarters Floodproofing Project in an amount not to exceed $22,803.50, for a total contract price not. to exceed $215,000. Agenda for VB Me•ng 1-12-93 • -2- (The original contract amount was $192,196.50. The increase is needed to cover unanticipated work and materials involved with the project such as the replacement and installation of electrical service lines, iron piping, catch basins, clay filling and electrical modifications among others. - (A report from the Director of Engineering & Community Development detailing the added costs and recommending approval is attached. ) C) Consideration of a request from Alexian Brothers Medical Center (attached) to donate three (3) polaroid cameras to the Elk Grove Fire Department to document victim injuries encountered by Village Paramedic staff. (A camera will be provided for each of our three (3) ambulances. Film will be provided at no cost to the Village at the Emergency Department of the hospital . (The photographs, taken at the scene of the incident, will assist hospital emergency response personnel to assess patient needs and methods of treatment. These pictures will be included as part of the patients ' records. (The Fire Chief recommends approval of this request) . D) Consideration to grant an extension to the 15-mile residency requirement for an additional 3-month period, until May 12, 1993, for Firefighter Roger A. Ferguson. E) Consideration to authorize Patrol Officers Frank Vrchota and Patrick Hayes to attend Basic Recruit Training to be held in Chicago, IL, at the Chicago Police Training Academy, from January 4 to April 8, 1993, at a cost not to exceed $2,608 per officer and at a total cost not to exceed $5,216. (The police officers were hired on December 28. The openings were available at the Chicago Police Academy for training beginning January 4 . Authorization to attend the training academy was allowed subject to Village Board approval on January 12. ) Agenda for VB Metng 1-12-93 -3- F) Consideration to increase the FY 1992-93 Budget for the Department of Engineering and Community Development in the amount of $30,836, for payment to Triton Consulting Engineers, LTD. , under the supplemental agreement approved by the Village Board on July 14, 1992, for the Elmhurst road project . (The Village will be reimbursed 100% for these costs by the Illinois Department of Transportation. (A report from the Director of Engineering and Community Development recommending approval is attached. ) G) Consideration to extend the professional service contract with Integrated Technology Group (ITG) to supply professional data processing services for an additional 15-day period at a cost not to exceed $10,000. (The revised contract will be available for the January 26 Village Board agenda. ) H) Consideration to approve the following monthly rates for all levels of participation in the HMO Illinois Medical Health Care Plan and the Humana Health Chicago Medical and Dental Health Care Plans for the period January 1, 1993 to April 30, 1993. (The Humana Health Chicago rates represent a 5. 7% increase over the 1992 calendar year. (The Humana Dental Plan rates at the employee only and the employee +1 dependent levels- remain the same as the previous year. The employee +2 or more dependent rate is decreased 42% over the previous year. Agenda for VB Me O ng 1-12-93 • -4- (The premiums for the Village 's Self-Insurance Health Plan is effective from May 1 to April 30 (fiscal year) . The approval of the rates for the HMOs will place them on a fiscal year basis as well . (A report from the Personnel Office was previously distributed. ) I) Consideration to adopt RESOLUTION NO. 1-93 determining the appropriateness for class 6B status pursuant to the Cook County Real Property Classification Ordinance as amended January 1 , 1990 for certain real estate owned by Pearson I Inc. /Bruce Offset Company, 1099 Greenleaf Avenue, Elk Grove Village, IL. (Pearson I Inc. /Bruce Offset Company plans to build an addition of 32,000 square feet for the expansion of their commercial printing business. The property tax incentive allows a reduction from 36% of fair market value to 16% for an eight (8) year period. (The construction of the addition will create 40 full-time and 5 part-time jobs and provide an estimated $9,000 per year in sales taxes to the Village. (Attached is a report from the Economic Development Assistant and a letter from Pearson I Inc. /Bruce Offset Company requesting the class 6B status. ) J) Consideration to approve ORDINANCE NO. 2281 granting a certificate of authority to AAA 303 Cab Association, Inc. to permit the issuance of a public passenger vehicle dispatcher license in the Village of Elk Grove Village. (The Public Vehicle Commission held a public hearing on AAA 303 Cab Association 's request for a public passenger vehicle dispatcher license on December 8, 1992. This item was discussed at the December 15 Committee of the Whole Meeting) . K) Consideration to adopt RESOLUTION NO. 2-93 (attached) establishing fees for public hearings held before all Village Boards and Commissions as established by the Village of Elk Grove Village. (The resolution establishes filing fees and court reporter deposit amount for all public hearings, excluding public hearings related to annexations and zoning issues and zoning appeals. The fees for the items that are excluded were approved by Resolution 8-92. Agenda for VB Meeting 1-12-93 -5- (This Resolution will establish a fee schedule for public hearings heard by the Vehicle Commission, Liquor Commission or other possible hearings before other Boards or Commissions. The new Municipal Code requires these fees to be established by Resolution. In the past, these fees were published directly in the Municipal Code. The fee schedule represents no change from the fees assessed in the past. (A report from the Village Clerk recommending approval is attached. ) L) Consideration to approve ORDINANCE NO. 2282 amending the restriction on the use of water for outdoor purposes for Village on the Lake. (The amendment allows Village on the Lake to use water outside, Monday through Friday, from 9 a.m. to 1 p.m. when the Village 's watering restrictions are in effect. These hours are currently applied to our industrial and commercial users. (This item was discussed at the 12115192 Committee of the Whole Meeting. ) M) Consideration to award a professional service contract to Alexian Brothers Medical Center to administer Hepatitis B inoculations to 99 Police Department personnel at a total cost not to exceed $15,000. (On October 13, 1992, the Village Board increased the Police Department budget to administer Hepatitis B inoculations for sworn police officers as required under the Federally mandated Bloodborne Pathogen Act. At that time, it was expected that the cost to administer the inoculations would be $18,000. Since then the cost to administer the inoculations decreased $30 per person for a total cost of $15,000. (ABMC is recommended to initially administer the inoculations to the entire Department as their location is convenient for Officers and ABMC has provided quality services in the past. Any future inoculations for new employees will be administered by the Community Health Nurse. Agenda for VB Me•ng 1-12-93 • -6- REGULAR AGENDA 7. Consideration of requests (attached) from the Elk Grove Park District, School District #54 and School District #59 to waive the 1993 Alarm User License Fees as follows: Park District - $200: Al Hattendorf Sr. Citizen Al Hattendorf Recreation Center Elk Grove TV Studio Elk Grove Teen Center Elk Grove Park District (main office) Elk Grove Historical Society Museum Fox Run Golf Course Elk Grove Park District Warehouse School District 54 - $75: Adolph Link School Adalai Stevenson School Margaret Mead School School District 59 - $150: Admiral Byrd School Clearmont School Grove Jr. High School Ridge School Rupley School Salt Creek School (It has been the past practice of the Village Board to waive license fees for governmental agencies. (A report from the Director of Finance is attached. ) 8. Consideration to approve ORDINANCE NO. 2283 amending Ordinance No. 1714 pertaining to the development, landscaping and screening and approving the site plan for property located at the northwest corner of Meacham Road and Biesterfield Road, Elk Grove Village. (This item is under the Unfinished Business (Village Attorney) section of the agenda.
