Consideration of a request from Alexian Brothers Medical Center, located at 800 Biesterfield Road to waive permit fees for the installation of the sign for the Hospice Residence at 901 Martha Street in the amount of$34.00. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (A report from the Director of Community Development,recommending approval,is attached.) b. Consideration of a request from the E'lk Grove Village Park District to waive building permit fees for the removal and replacement of five (5) light poles at Olmstead Park, located at 920 Cypress Lane, in the amount of$195.00. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (A report from the Director of Community Development, recommending approval, is attached.) c. Consideration of a request(attached) from Rodriguez Dance Theatre to waive fees for a Class A Liquor License and a Food Dealer License for the purpose of a fundraising event Page 1 of 7 to be held on October 11, 2013 at St. Nicholas Episcopal Church located at 1072 Ridge Avenue. (The Liquor License is $2,900 and the Food Dealer License is $100 for a total fee waiver of$3,000. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (A report from the Village Clerk, recommending approval, is attached.) d. Consideration to award a professional service contract to the lowest responsive and responsible bidder Denler& Sons, Inc. of Mokena, IL for the 2013 Crack Sealing Contract in the amount of$41,718.40. (On Thursday, August 29, 2013,the Village opened sealed bids for the 2013 Crack Sealing Contract. A total of four(4) contractors obtained bid documents, with three (3) submitting bids on the project. (The lowest responsive and responsible bid was received from Denler& Sons, Inc. of Mokena, IL, in the amount of$41,718.40. (The project consists of routing and filling cracks in the asphalt pavement on Lively Boulevard, from Higgins Road to Landnreier Road, and from Pratt Boulevard to Devon Avenue. The section of Lively Boulevard from L,andmeicr Road to Pratt Boulevard was completed under contract in 2012. (Adequate funds are available in the General Fund. (A report from the Director of Public Works, recommending approval, is attached.) e. Consideration to award a professional services contract to Plante Moran of Southfield, MI for consulting services in the selection of a replacement Enterprise Resource Planning (ERP) system in an amount not to exceed $67,850. (The Village's current ERP system is more than 15 years old and in need of replacement. (The replacement of the ERP will provide several enhancements to the Village's operations including improvements in customer service by streamlining business processes, allowing for faster management decisions, and improving workflow. (Additionally, the new software will more easily interface with the many third party applications the Village requires, and it will reduce the dependency on custorn developed applications required to run the organization. (A cross departmental team was established to oversee the vendor evaluation and selection for the ERP consultants. (The Village received a total of six (6) responses to our request for proposals. Of the six, two(2) vendors were selected to make on-site presentations. (Plante Moran was selected as our vendor of choice based on the breadth and depth of their knowledge about ERP vendor offerings as well as their broad experience in assisting other communities with similar project's. (The firm has been in business for 88 years and has consulted on over 75 LRP engagements in the past 5 years. They regularly engage ERP vendors in discussions regarding their current and future plans and have over 30 active client projects in their -- ----Page 2 of 7— ------ ---..�----— Government Technology Consulting Service. (Some of their current and recent projects for local communities inchzde providing ERP consulting services for the Villages of Woodridge, Mount Prospect and Northbrook. (A report from the Director of Information Technology, recommending approval, is attached.) f. Consideration to award a professional services contract to Peters & Associates of Oakbrook Terrace, I1, in an amount not to exceed $141,480 for advanced technical support of the Village's computer networks during Fiscal Year 2014. (The Information Technology Department needs to supplement its Network Engineer skill set. Based on the current hourly rate, the requested amount would provide three days per week of Network langineer support for the remainder of FY 2014. (Peters & Associates has provided high-quality implementation and support services to the Village for over 5 years. (As in the past, the requested support will only be used and billed on an as-needed basis. (A report from the Director of Information Technology, recommending approval, is attached.) g. Consideration to increase a professional service contract with Schroeder& Schroeder, Inc. of Skokie, 1L for the 2013 Village-Wide Concrete Replacement Program by $105,000 from $337,052 to $442,052. (A contract for the 2013 Village-Wide Concrete Replacement Program was approved on February 26, 2013. (This contract provides for rehabilitation of the concrete sidewalk and curb along miscellaneous Village roadways, replacement of concrete items disturbed in sewer and watermain repairs, and establishes pricing for the Private Participation Concrete Replacement Program (PPCRP). (A substantial increase in watermain breaks this past year subsequently resulted in necessary restoration of concrete sidewalk, aprons, and curbs. (An increase in sidewalk identified from field observations and resident requests as having excessive differential settlements resulted in additional corrective work. (It is recommended that the contract with Schroeder & Schroeder, Inc. be increased by $105,000 to a total amount of$442,052. (Sufficient funds for this increase are allocated in the Water& Sewer Fund in the amount of$40,000, and sufficient funds in the amount of$65,000 are available in the 2013 Motor Fuel 'fax (MFT) Fund. (A report from the Director of Public Works, recommending approval, is attached) h_ Consideration to grant a variation from the Municipal Code, Section 8-1213-1AD, Easements, to permit the construction of a shed which will encroach three feet(3') into the ten-foot (10') public utility easement at 59 Woodcrest Lane. (The property owner is seeking a variation to construct a shed within a public utility easement. Comcast, AT&T,Nicor, and ComEd have written letters (attached) granting permission to encroach upon the easement. Page 3 of 7 (The Community Development Department has Geld checked this location for Village- owned utilities in the easement. There are no Village utility conflicts in the easement. (A report fi-om the Director of Community Development, recommending approval, is attached.) i. Consideration to adopt Ordinance No. 3350 to amend the Village Code of the Village of Elk Grove Village, "rifle 6, Section 6-4-5-3 A. Prohibited and Restricted Arcas: No Parking Streets and Areas: (Martha Street) as it pertains to parking restrictions on the west side of Martha Street south of Biesterfield Road. (This Ordinance amends Section 6-4-5-3 A of the local code to ban parking on the west side of Martha Street south of Biesterfield Road. (A report from the Chief of Police, recommending approval, is attached.) J. Consideration to adopt Resolution No. 45-13 (attached) determining the appropriateness for Class 6B status pursuant to the Cook County Real Property Classification Ordinance as amended February 15, 2006 for certain real estate located at 800 Morse Avenue. (The Law Offices of Liston &Tsantilis, on behalf of Ella Holdings, LLC, (Applicant) is seeking a Cook County Class 6B tax exemption for property located at 800 Morse Avenue. (The Applicant intends on purchasing the building and leasing it to a related entity called Perfection Plating. (The subject property consists of an approximately 39,520 square foot building. (Perfection Plating is a manufacturer and distributor of industrial metal plating. Due to business expansion, Perfection Plating intends to occupy the building and utilize it for warehousing and distribution. (The Applicant plans to create up to ten new full-time jobs and two new part-time jobs. (Perfection Plating is currently headquartered in Elk Grove Village at 775 Morse Avenue and also operates at 1501-1502 Morse Avenue. Perfection Plating currently employs 125 full-time people in the Village. (The Applicant has allotted $300,000 to refurbish the building including but not limited to the office area, warehouse area, lighting system, restrooms, exteriors, and parking lot. (Eligibility requirements for 6B status are new construction, substantial renovation or buildings that have been vacant for a period of time. This site qualifies as it involves occupation of an abandoned building for less than twenty-four(24) months continuous vacancy with a purchase for value and special circumstances. (Property approved for Class 613 status allows the owner of the property to have the assessment level lowered for a period of twelve years. The assessment schedule is 10% of fair market value for ten years then 15% in the eleventh year and 20% in the twelfth year. Industrial property is currently assessed at 25% of fair market value in Cook County. (A report from the Director of Business Development and Marketing, recommending approval, is attached.) k. Consideration to adopt Resolution No. 46-13 (attached) determining the appropriateness Por Class 6B status pursuant to the Cook County Real Property Classification Ordinance as amended February 15, 2006 for certain real estate located at 1601 Tonne Road. (Applicant) is seeking a Cook County Class 6B tax exemption for property located at 1601 Tonne Road. (The Applicant intends on purchasing the building to utilize it for the design and manufacture of signs for public transportation systems. (The subject property consists of an approximately 23,700 square foot building. (Hanover Displays Inc. is currently located in Elk Grove Village at 354, 356, and 358 Lively Boulevard. Due to business expansion,the Applicant is seeking to consolidate all three locations in Elk Grove Village. (Hanover Displays Inc. has allotted $800,000 to refurbish the building including but not limited to the roof, floors, HVAC system, plumbing, electrical, windows, and doors. (Eligibility requirements for 6B status are new construction, substantial renovation or buildings that have been vacant for a period of time. This site qualifies as it involves occupation of an abandoned building for less than twenty-four (24) months continuous vacancy with a purchase for value and special circumstances. (Property approved for Class 613 status allows the owner of the property to have the assessment level lowered for a period of twelve years. The assessment schedule is 10% of fair market value for ten years then 15% in the eleventh year and 20% in the twelfth year.
