Consideration of a request to renew a purchase contract with Beverly Materials of Hoffman Estates, IL to furnish and deliver gravel and sand in the amount of$26,843.50, of which $26.131.75 is from the Water-Sewer Fund and $711.75 is from the General Fund. (This contract was competitively bid in 2012. (The contract provides for an option of four(4) annual renewals. (Beverly Materials has satisfactorily provided sand and gravel materials for the Village. (The contract period is fi-om May I, 2014 through April 30, 2015. (The contract amount reflects a 0.5% increase in the Consumer Price1ndex. f0 (A report firom the Director of Public Works, recommending approval, is attached). s b. Consideration of a request to renewal a professional service contract to Lechner& Sons of Mount Prospect, IL for uniform, linen, and floor mat rental services is the amount of $28,070 from the General Fund. j (This contract was competitively bid in 2012 and provides for a two-year initial term with the option of three (3) additional annual renewals. (Lechner & Sons has performed satisfactorily throughout this contract. (The contract period is May 1, 2014 through April 30, 2015. (The contract amount reflects a 0.5% increase in the Consumer Price Index. (A report from the Director of Public Works, recommending approval, is attached). Page 1 of 11 c. Consideration of a request to renew a custodial service contract with Crystal Maintenance Services of Mount Prospect, IL to provide custodial services in the amount of$35,500. (This contract was competitively bid in 2010 and provided an option of four(4) annual renewals through April 30, 2015. (Crystal Maintenance Services has satisfactorily provided custodial services for the Village. (The contract period is from May I, 2014 through April 30, 2015. (The contract amount reflects a 0.5% increase in the Consumer Price Index. (A report from the Director of Public Works, recommending approval, is attached). d. Consideration to award a professional service contract to Johnny Rockets Fireworks Display Corporation of Chicago, IL to design and produce fireworks displays in an amount not to exceed $40,000. (This renewal is year three of a five-year contract with Johnny Rockets. (The contract locks in rates at$40.000 through 2016. (The Village administers three fireworks displays each year including a display at RotaryFest(the Saturday before Father's Day), Fourth of July, and a display at the Tree Lighting Ceremony (Friday after Thanksgiving). (The company's performance over the past twelve years has been satisfactory. (The contract cost will be offset each year depending on the contribution of funds by residents through utility billing for the July 4th display.) _ e. Consideration to award a contract with Fleck's Landscaping, Inc. of Wheeling, IL, for the Turf and Landscape Sed Maintenance—Business Park Contract in the amount of $45,210 from the Industrial/Commercial Revitalization Fund. (On Thursday, March 27, 2014, the Village opened sealed bids for the 2014 Turf and Landscape Sed Maintenance—Business Park contract. Three (3) contractors obtained bid documents and submitted bids_ (The contract provides for turf mowing, landscape bed maintenance, fertilizing services, and irrigation system maintenance. (The initial term of the contract is from April 15, 2014 through February 28, 2015 with the Option for four (4) annual renewals in one (1) year increments through February 28, 2019. (The lowest responsive and responsible bid was received from Fleck's Landscaping, Inc.. of Wheeling, IL, in the amount of$ 45,210. (Funds for the project have been allocated in the Industrial/Commercial Revitalization Fund. (A report from the Director of Public Works, recommending approval, is attached.) f Consideration to award a professional service contract to Sungard HTE of Lake Mary, Fl- for Lfor support and maintenance ofour business software applications in the amount of r $46.126.71. (Sungard has acquired HTE, but their various applications remain well integrated and continue to support a large majority of our requirements. (The Village has utilized HTE software since 1990. (Ongoing software support from LITE is critical to the Village's financial operations. Page 2 of 11 (Most of the requested funds of$40,626 will provide product support and program updates for the nine (9) HTE applications currently utilized by the Village. (The remaining $5,500 portion of the requested funds will provide maintenance and support of the custom modifications to the HTE software required by the Village for a total contract amount of$46,126.71. (A report fi"om the Director of Information Technology, recommending approval, is attached.) g. Consideration to award a contract for furnishing Hot-Mix Asphalt Materials with Arrow Road Construction Company of Mount Prospect, IL for the 2014 MPT Hot-Mix Asphalt Material Purchase Contract in the amount of$57,931.30 from the Motor Fuel Tax Fund. (On Thursday, March 27, 2014, the Village opened sealed bids for the 2014 MFT Hot-Mix Asphalt Material Purchase Contract. Three (3) vendors obtained bid documents and three (3) submitted bids. (This contract provides asphalt materials used by the Department of Public Works for residential and business park street patching operations. (The lowest responsive and responsible bid was received from Arrow Road Construction Company, of Mount Prospect, IL. This bid is approximately 3.45 percent lower than the Engineer's Estimate of$60,000. (This contractor has previously supplied asphalt materials to the Village and has successfully completed several projects for the Village including last year's MFT program. (It is recommended that a contract for the 2014 Hot-Mix Asphalt Material Purchase Contract be awarded to Arrow Road Construction Company in the amount not to exceed $57,931.30. (Sufficient funds for this contract are allocated in the FY 2015 budget through the Motor Fuel Tax (MFT) Fund. (A report from the Director of Public Works, recommending approval, is attached.) h. Consideration to award a professional service contract to the lowest responsive and responsible bidder Dynamic Heating & Piping Company of Crestwood, IL for the Replacement of Rooftop Units at the Municipal Complex (901 Wellington Avenue) in the amount of$59,400 from the Capital Projects Fund. (On Thursday, March 27, 2014, the Village opened sealed bids for the Replacement of Rooftop HVAC Units for the Village Hall portion of the Municipal Complex (901 Wellington Avenue). (A total of six (6) contractors obtained bid documents, with three (3) submitting bids on the project. (The lowest responsive and responsible bid was received from Dynamic Heating & Piping Company of Crestwood, IL in the amount of$ 59,400. (The project will include the removal and replacement of,rooftop HVAC units and connection to the building automation system. (Adequate funds are available in the Capital Projects Fund. (A report from the Director of Public Works, recommending approval, is attached.) i. Consideration to award a construction contract to the lowest responsive and responsible bidder, Globe Construction Inc. of Addison, IL for the 2014 Private Participation Concrete Replacement Program (PPCRP) in an amount not to exceed $109,837, of which Page 3 of 11 $99,837 is from the PPCRP (self-funded), $5,000 is from the Water & Sewer Fund and $5,000 is from the Corporate Fund. (On March 27, 2014, the Village opened sealed bids for the 2014 Private Participation Concrete Replacement Program. (A total of sixteen (16) contractors obtained bid documents and eleven (1 l) contractors submitted bids. (The lowest responsive and responsible bid was received from Globe Construction Inc. of Addison, IL in the amount of$109,837. (Adequate funds are budgeted in the Private Participation Concrete Replacement Program, Water & Sewer Fund and Corporate Fund. (A report from the Director of Community Development, recommending approval, is attached.) j. Consideration of a request to award a maintenance contract with Lyons & Pinner Electric Company, Inc. of LaGrange, IL, for the Residential Streetlight Maintenance Contract in the amount of$109,985.20. (On Thursday, March 20, 2014, the Village opened sealed bids for the Residential Streetlight Maintenance Contract. Four (4) contractors obtained bid documents and three (3) submitted bids. (The contract provides for the routine monthly maintenance of the Village's residential streetlights and funding for non-routine/emergency work. A cost for group re-lamping was also solicited as part of the proposal. (The initial term of the contract is from May 1, 2014 through April 30, 2015 with the option for four(4) annual renewals in one (1) year increments through April 30, 2019. (The lowest responsive and responsible bid was received front Lyons & Pinner Electric Company, Inc. of LaGrange, IL in the amount of$ 109,985.20. (Funds for this contract have been allocated in the FY 2015 Public Works Department budget. (A report from the Director of Public Works, recommending approval, is attached.) k. Consideration to award a professional service contract with Brancato Landscaping, Inc., of Elk Grove Village, IL for the Parkway Turf Restoration - Hydroseeding Contract in the amount not to exceed $124,000. (On Thursday, March 27, 2014,, the Village opened sealed bids for the Parkway Turf Restoration - Hydroseeding Contract. Four (4) contractors obtained bid documents and three (3) submitted bids. (The contract provides for the landscape services including site preparation, hydroseeding, and watering required to restore parkway turf areas damaged by tree removals or Village utility repairs. (The initial term of the contract is from April 8, 2014 through March 30, 2015 with the option for four(4) annual renewals in one (1) year increments through March 30, 2019. (The lowest responsive and responsible bid was received from Brancato Landscaping, 901 Nicholas Boulevard, Elk Grove Village, 1L in the amount of$ 124,000. (Funds for the project have been allocated in the FY 2015 budget among Corporate Account, Recycling Fund, and the Water & Sewer Fuld. Consideration of a request to renew a maintenance contract with Lyons & Pinner Electric Company, Inc. of LaGrange, IL for annual Arterial and Business Park Lighting maintenance in the amount of$129,886.05 from the ICRC Fund. (On Apt-if 10, 2012 the Village Board awarded a contract to Lyons & Pinner Electric Company, Inc. of LaGrange, IL for maintenance of the Village's arterial and business park street lights. (The contract provided for four(4) annual renewals. (The contract period is May 1, 2014 through April 30, 2015. (The contract amount reflects a 0.5% increase in the Consumer Price Index and a modification to the monthly unit price for routine maintenance due to a significant increase in the number of JULIE locales required since inception of the original contract. (Lyons & Pinner has provided excellent service to the Village throughout this contract. (A report from the Director of Public Works, recommending approval, is attached.) in. Consideration of request to award a purchase contract to Physio-Control, Inc., of Redmond, WA For the purchase of six (6) Lifepak 15 Defibrillators in the amount of $160,278.60. (The Fire Department has received a quote from Physio-Control, Inc., of Redmond, WA for the purchase of six (6) Lifepak 15 Defibrillators. (The quote provides for a substantial discount based on the number of units to be purchased, as well as a trade-in allowance for our current equipment. (The new models function similarly to our current Lifepak 12 equipment, and fire department personnel are trained on the use of these defibrillators so no additional training will be needed to place the new units in service. (This purchase covers the first two (2) years of a planned three (3) year replacement of all of the Fire Department defibrillators. (Current models will not be supported by the manufacturer as of January 2016. (Adequate funds are available in the FY 2015 Fire Department General Fund, pending the carryover of funds from the current year budget to cover the cost of three (3) of the units. 1 (A report from the Fire Chief, recommending approval, is attached.) n. Consideration to award a professional service contract with Arrow Road Construction Company of Mount Prospect. IL for the 2014 MFT Hot-Mix Asphalt Resurfacing Contract in the amount of$859,148.30 from the Motor Fuel Tax Fund. (On Thursday, March 27, 2014, the Village opened sealed bids for the 2014 MFT Hot-Mix Asphalt Resurfacing Contract. Six (6) contactors obtained bid documents and five (5) submitted bids. (This contract provides miffing, patching, and asphalt resurfacing on 4.7 miles of Village streets. (The lowest responsive and responsible bid was received from Arrow Road Construction Company, of Mount Prospect, IL. This bid is approximately 3.15 percent lower than the Engineer's Estimate of$887,136. (This contactor has completed several projects for the Village including last year's MFT Page 5 of 11 'r program. (It is recommended that a contract for the 2014 Hot-Mix Asphalt Resurfacing Contract be awarded to Arrow Road Construction Company in the amount not to exceed $859,148.30. (Sufficient funds for this contract are allocated in the FY 2015 budget through the Motor Fuel Tax (MFT) Fund. t (A report from the Director of Public Works, recommending approval, is attached.) o. Consideration to award a professional service contract with Mondi Construction, Inc. of Roselle, IL,. for the 2014 Village-Wide Concrete Replacement Program in the amount of $229,900.40 from the Motor Fuel Tax Fund and $89,000 from the Water& Sewer Fund for a total amount of$318,900. (On Thursday, March 27, 2014, the Village opened sealed bids for the 2014 Village-Wide Concrete Replacement Contract. Ten (10) contractors obtained bid documents and five (5) submitted bids. (This contract provides for rehabilitating concrete sidewalk and curbs along miscellaneous Village roadways as part of the street resurfacing program, and restores concrete items disturbed in sewer and wateimain repairs. (The lowest responsive and responsible bid was received from Mondi Construction, Inc., of Roselle, IL. (This bid is approximately 7.64 percent lower than the Engineer's Estimate of$345,264. (This contractor satisfactorily completed similar concrete construction projects for various municipalities in the past year including Downers Grove and Buffalo Grove. (It is recommended that a contract for the 2014 Village-wide Concrete Replacement Contract be awarded to Mondi Construction, Inc., in the amount not to exceed $318,900. (Sufficient funds for this contract are allocated in the FY 2015 budget through the Motor Fuel Tax (MPT) Fund ($229,900.40) and the Water & Sewer Fund ($89,000) for a total amount of$318,900. (A report front the Director of Public Works, recommending approval, is attached.) p. Consideration to award a furnish and delivery contract with Avalon Petroleum, Inc. of Kankakee, IL for gasoline and diesel fuel in an amount not to exceed $474,000. (On Thursday, March 27, 2014, the Village opened sealed bids for the Gasoline and Diesel Fuel Supply Contract. Five (5) contractors obtained bid documents and four (4) submitted bids. (The contract provides tot- furnishing and delivery of 87-octane regular unleaded gasoline, No. 2 grade ultra-low sulfur diesel fuel, and winter blend ultra-low sulfur diesel fuel to three (3) Village sites. (The initial term of tile contract is from May 1, 2014 through April 30, 2015 with the } option for four(4) annual renewals in one (1) year increments through April 30, 2019. (The lowest responsive and responsible bid was received from Avalon Petroleum Company, Inc., of Kankakee, IL. (This vendor's fixed margin added to the indexed fuel prices is the lowest of the four(4) responsive bidders. (Funds for gasoline and diesel fuel are adequately budgeted among the various Village departments. Costs are allocated based on actual usage. (A report from the Director of Public Works, recommending approval, is attached.) Page 6 of 11 y. Consideration of the following items for Fiscal Year 2014-15: i • To authorize a 2% market adjustment to non-union step and specific rate plan positions; • To authorize a 2% market adjustment to the merit pay plan positions; • To continue the eligibility range for defined merit employees who are within 66% of s the pay range, funded at 2%; • To adopt Ordinance No. 3375 amending wages by 2% for part-time positions of Custodian, Porter, Health Code Enforcement Officer, Fire Inspector, and Water Meter Reader; Temporary Part-Time Administrative interns and Summer Seasonal Employees; and School Crossing Guards of the Village of Elk Grove Village; and • To adopt Ordinance No. 3376 (attached) amending the Position Classification and Salary Plans: the Classification Plans: and Salary Range for, Merit Pay Plan Positions, Step Pay Plan Positions, and Specific Pay Plan Positions of the Village Code of the Village of Elk Grove Village. Consideration to adopt Ordinance No. 3377 (attached) amending Chapter 22 `Single Family Rental Unit Business License" by establishing licensing provisions for all residential rental wt its and provisions for crime free housing. (One of the primary components of the amendment involves the licensing and inspection ofall rental units in the Village, both single family and multi-family, as well as the rental of individual condos and townhomes. (The other component of the amendment would be the creation of the Crime Free Rental Housing provisions, which are intended to protect, preserve, and promote the health, safety and welfare through the reduction, control and prevention of criminal and nuisance Page 9 of 11 activities. Consideration to increase the contract awarded to Water Well Solutions of Illinois Division, LLC of Elburn, IL to rehabilitate Emergency Well 14 (998 Devon Avenue) in the amount of$56,806 from the Water& Sewer Fund. (On August 20, 2014 the Village Board awarded a contract to the lowest responsive and responsible bidder, Water Well Solutions Illinois Division, LLC of Elburn, IL in the amount of$188,782 to rehabilitate the Emergency Well 14 (998 Devon). (The project consists of furnishing and installing column pipe, check valves, air lines, cable, motor, rebuilding the pump assembly, and rehabilitating the well. (Water Well Solutions has completed the AlrBurst Rehabilitation process, but that process has generated significantly more sand & rocks than anticipated. The sand bailing process _ has exceeded the estimated hours the contract award was based upon. (The requested increase of$56,806 will increase the total amount for the contract not to exceed $245,588. (Adequate funds for the project are available in the Water& Sewer Fund. (A report from the Deputy Director of Operations - Public Works, recommending approval, is attached.) u. Consideration to adopt Resolution No. 11-14 determining the appropriateness for Class 613 status pursuant to the Cook County Real Property Classification Ordinance as amended July 17, 2013 for certain real estate located at 800 Arthur Avenue. (The Law Offices of Madigan & Getzendanner, on behalf of Prologis (Applicant) is seeking a Cook County Class 613 property tax exemption for property located at 800 Arthur Avenue. (The Applicant is in negotiations with the SPX Corporation to lease the property which has been vacant since August of 2011. (SPX is currently located in Elk Grove Village at 751 Pratt Avenue where they currently employ between 125-150 people. (The company is expanding and relocating their entire operation to 800 Arthur Ave. (Additionally, SPX will relocate its Wood Dale operations to Elk Grove which will bring the total employment to between 145-170 people. (The subject property consists of an approximately 71.841 square foot building. Page 1 of 6 (The Applicant has allotted $2,149,579 to refurbish the building including, updating the existing 13,809 square feet of office to 2014 standards, which will include a new office layout, paint, carpet, and tile. (Additionally, the Applicant intends to construct 15,000 square feet of office/lab that will be used for their engineers for research and development and testing. (Also included will be the installation of new exterior windows on the east of the building to increase natural light along with a new employee entrance. Energy efficient T5 lights will be installed throughout the warehouse, along with installation of air conditioning in the warehouse, and an increase in the power from 400 amps to 1200 amps. (Specific office improvements will include, expanding the break room, constructing a private office with private restroom, constructing two conference rooms. There will be 15 private offices in total. The office lab will have +/- 12 private tech offices, 7 rooms devoted to labs and conference space, and additional restrooms for lab techs and warehouse employees. The entire office area will have finishes in line with a class A office building. There will be new carpet, paint, and tile throughout. (The move is a result of recent business expansion. Johnstone intends on relocating its current six full-time employees to the new location and hire an additional two full-time positions. The Applicant has allotted approximately $35,000 to rehabilitate the building. This includes but is not limited to installation of a new HVAC system, new lighting, and new floors. This also includes construction of 3,500 square foot showroom. The Applicant has also submitted a landscape plan for screening along Lively Boulevard at a cost of$7,000. (According to Prologis (Seller), they have completed over$116,000 worth of work to the building including landscaping and signage in order to generate a sale. (The Applicant intends on purchasing the building and leasing it to a related entity called Ryano Resins. Ryano intends to use the facility for the blending of epoxy resins and the warehousing and distribution of the same to be used in the construction industry for floor and wall coatings. (Ryano Resins currently occupies an approximately 3,000 square foot space in Aurora and because the business has been growing it needs to expand its operations. Ryano Resins intends on relocating its entire operations to Elk Grove Village including its entire workforce of seven with four new jobs to be created shortly after the move. (The building has been 100%vacant since February of 2013. (Since taking ownership, the Applicant has spent $223,400 to refurbish the building making it attractive to a new tenant. (Atlas plans on spending $332,971 on further improvements to the building including both internal and external construction, landscaping, and signage.
