6. CONSENT AGENDA
a. Consideration of a request from the Prairie Lake School to waive fees for a food dealer
license for the resale of food to families of the school in the pro -rated amount of $50.
(The Prairie Lake School plans to operate a resale of food to families of the school as soon
as the permit is approved.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.
(A report from the Director of Finance, recommending approval, is attached.)
b. Consideration of a request from Elk Grove Park District to waive permit fees in the
amount of $750.00 for partial storm sewer remediation at Hampshire Park, 1632-1635
Gibson Drive.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.
(The Director of Community Development recommends approval.)
c. Consideration to award a professional service contract to Ayers Associates Inc. of
Madison, WI for aerial orthophotography and planimetric mapping updates as part of the
GIS Consortium purchasing agreement in the amount of $26,740 from the Water/Sewer
Fund.
(Elk Grove Village has been a member of the GIS Consortium since 2008.
(The GIS Consortium (GISC) is a group of local communities that have worked together to
develop a cost-effective solution for implementing geographic information systems (GIS)

and related technologies.
(One of those cost-effective solutions is to purchase aerial photography/mapping in bulk
through the Consortium's 34 municipalities.
(The GIS Consortium Board of Directors appointed a Base Mapping Committee to initiate
a selection process for a photogrammetric mapping consultant, and Ayers Associates was
the selected vendor.
(As part of the pricing agreement, Ayers Associates uses economies of scale to provide
their services at a reduced price.
(As part of this year's mapping program, Ayers Associates will produce updated aerial
imagery of the Village and perform planimetric imagery updates within the Elk Grove
Village community.
(The Assistant Village Manager recommends approval.)
d. Consideration to award a purchase contract to Sentinel Technologies, Inc. of Downers
Grove, IL to provide forty (40) replacement Cisco access points for an amount not to
exceed $27,653.
(Wireless access points are used for a variety of reasons at the Village Hall, Public Safety
complex, and Fire Station 7.
(The access points serving these locations will be 10 years old next month. Since being
purchased they've been surpassed by two generations of newer, faster access points.
(Replacing these access points with current, state-of-the-art technology will both increase
employee productivity and eliminate the risk of older equipment failing.
(The IT staff requested quotes for new access points from three Cisco -certified vendors.
Sentinel's price reflects the lowest of the three quotes received.
(The Director of Information Technology recommends approval.)
e. Consideration to award a contract with Emeric Facilities Services of Waukegan, IL for the
Cleaning and Custodial Services contract in the amount of $38,741.76 from the General
Fund.
(On Thursday, March 14, 2019, the Village opened sealed bids for Cleaning and Custodial
Services at various Village facilities. Seven (7) contractors obtained bids and six (6) bids
were submitted.
(This contract provides for routine cleaning services including the supply of products.
(The initial term of the contract is from May 1, 2019 through April 30, 2020, with the
option of four (4) annual renewals in one-year increments through April 30, 2024.
(The lowest responsive and responsible bid was received from Emeric Facilities Services
of Waukegan, IL in the amount $38,741.76.
(Funds for the project have been allocated in the Public Works General Fund.
(The Director of Public Works recommends approval.)
f. Consideration to award a purchase contract to Sentinel Technologies, Inc. of Downers
Grove, IL to provide five (5) 48-port Cisco 9300 switches and associated licenses for an
amount not to exceed $41,619.
(Just over ten years ago the IT staff installed Cisco switches at key location points

throughout the Village to enable employees to work more efficiently and
productively. Later this year these switches will reach the age where they're no longer be
supported by Cisco.
(Two years ago the IT staff began staggering the replacement of our older Cisco switches
in an effort to level out the expense associated with continuing to use vendor -supported
equipment.
(IT staff requested quotes from three Cisco partners for the five (5) switches we currently
require. Sentinel's price reflects the lowest of the three quotes received.
(The Director of Information Technology recommends approval.)
g. Consideration to award a service contract through the U.S. Communities Government
Purchasing Alliance with Cintas Corporation of Schaumburg, IL for the Uniform, Linen,
and Floor Mat Services Contract in an amount not to exceed $85,000.
(The Department of Public Works recommends awarding a service contract to Cintas
Corporation of Schaumburg, IL for the Uniform, Linen and Floor Mate Services contract.
(The Uniform, Linen and Floor Mate Services contract pricing has been established by the
U.S. Communities joint purchasing cooperative and offered from Cintas Corporation of
Schaumburg, IL.
(The contract provides uniforms for Public Works Department staff and linen and floor
mat services at the Charles J. Zettek Municipal Complex and two (2) Public Works
Department facilities.
(Adequate funds are budgeted in the Public Works Department Budget.
(The Director of Public Works recommends approval.)
h. Consideration to approve the third quarter financial report ending January 31, 2019.
(As part of the new financial software system, staff has created quarterly financial reports
comparing year to date numbers to the prior year.
(The Director of Finance recommends approval.)
i. Consideration to commit to the purchase of Bulk Rock Salt through the State of Illinois
Joint Purchasing Contract in the amount of 4,000 tons for the FY 2019-2020 winter season.
(On October 23, 2018, the Village Board authorized the purchase of 2,400 tons of bulk
rock salt through the Illinois Central Management Services (CMS) Joint Purchasing
Program with the option to increase or decrease the order by twenty percent (20%), or 480
tons.
(This winter season the Village will purchase 2,880 tons of road salt, or one hundred
twenty percent (120%) of the awarded amount.
(Illinois (CMS) is currently preparing bids for the FY 2019-20 Bulk Rock Salt Joint
Purchasing Program.
(The Village intends to purchase 4,000 tons of Bulk Road Salt in FY 2020.
(The contract allows for the purchase of a minimum of eighty percent (80%) and a
maximum of one hundred and twenty percent (120%) of the awarded amount.
(This purchase option provides flexibility in the event that snowfall totals were below
average and provides sufficient quantity to ensure safe roadways during a severe winter.
(Adequate funds have been budgeted for this project in the FY 2020 budget.

j. Consideration to adopt Ordinance No. 3598 amending Title 8, Chapter 3, of the Village
Code by adding thereto a new Section 8-3K-1: Penalties of the Building Activity Code of
the Municipal Code of the Village of Elk Grove Village.
(This Ordinance amends recently adopted Ordinance 3597 to add Chapter 3K which was
inadvertently omitted.
(The Director of Community Development recommends approval.)
k. Consideration to adopt Resolution No. 16-19 determining the appropriateness for Class 6B
status pursuant to the Cook County Real Property Classification Ordinance as amended
July 27, 2018 for certain real estate located at 901 Chase Ave, Elk Grove Village, Illinois.
(The Law Offices of Liston & Tsantilis, on behalf of their client Duke Realty Limited
Partnership, (Applicant) is seeking a Cook County Class 613 property tax exemption for
property located at 901 Chase Ave.
(The Applicant intends to purchase the existing building and demolish it to construct a new
Class A industrial building. The property currently consist of an 115,729 square foot
building built in 1969.
(The Applicant will invest approximately $21,449,405 to acquire the property and
construct a 154,000 square foot Class A industrial building. Plans propose a building with
thirty two foot ceilings, up to fifteen dock doors, two grade level drive in doors, and one
hundred plus car parking with twenty five trailer parking spots.
(The eligibility requirements for 6B status are new construction, substantial renovation or
buildings that have been vacant for a period of time.
(This site qualifies as it involves new construction.
(Property approved for Class 613 status allows the owner of the property to have the
assessment level lowered for a period of twelve years. The assessment schedule is 10% of
fair market value for ten years then 15% in the eleventh year and 20% in the twelfth year.
Industrial property is currently assessed at 25% of fair market value in Cook County.
(Upon approval, the Director of Business Development and Marketing will issue a Letter
of Receipt to the Applicant. The Letter of Receipt will allow the Applicant to file an
application with Cook County. Final approval is at the discretion of the Village Board and
contingent with the Applicant completing the improvements stated in their application.
(The Director of Business Development and Marketing recommends issuing a Letter of
Receipt.)
1. Consideration to adopt Resolution No. 17-19 determining the appropriateness for Class 613
status pursuant to the Cook County Real Property Classification Ordinance as amended
July 27, 2018 for certain real estate located at 345 Scott Street, Elk Grove Village, Illinois.
(The Law Offices of Sarnoff & Baccash, on behalf of 345 Scott Street LLC (Applicant) is
seeking a Cook County Class 6B property tax exemption for 345 Scott Street.
(The Applicant intends to purchase the property and rehabilitate the same for use by a
related entity called Wookee Inc. Wookee will use the property for distribution,
warehousing, and freight forwarding.
(The subject property consists of an approximately 65,336 square foot site with a roughly
15,000 square foot building. The building will be vacant prior to the purchase. (The

Applicant will invest approximately $175,000 to rehabilitate the subject property.
Improvements will include fagade, landscaping, and signage. Additionally, the applicant
will replace the parking lot drive and dock apron. Other improvements include fixing the
parking lot, and office and warehouse remodeling.
(The Applicant will relocate their operation to the site from a currently leased space in Elk
Grove. Wookee will relocate five jobs and create ten to fifteen jobs in the coming years.
(This site qualifies as it involves a purchase for value with occupancy of less than twenty
four months with special circumstances and significant rehabilitation.
(The eligibility requirements for 613 status are new construction, substantial renovation or
buildings that have been vacant for a period of time.
(This site qualifies as it involves vacancy for less twenty-four (24) months with a purchase
for value, special circumstances, and significant rehabilitation.
(Property approved for Class 6B status allows the owner of the property to have the

annexation of Commonwealth Edison Substation.
Resolution No. 18-19 authorizing the Mayor and Village Clerk to execute an
Annexation Agreement between the Village of Elk Grove Village and
Commonwealth Edison Company of property to be Annexed to the Village;
Ordinance No. 3599 annexing, rezoning and granting a special use permit for
certain territory to the Village of Elk Grove Village and the granting of variations
of the Zoning Ordinance and Village Code (Commonwealth Edison); and
3. Resolution No. 19-19 approving the Final Plat of Subdivision identified as 1500
Midway Subdivision.

6. CONSENT AGENDA
m. Consideration to adopt Ordinance No. 3600 amending Chapter 3 Fire Prevention Code of
Title 5 of the Village Code by amending a provision of the 2012 Code pertaining to open -
flame cooking devices by adding additional exceptions to the regulations.
(This item was discussed at the March 12, 2019 Village Board Meeting.
(The Village Board requested staff to review the section of the Fire Code pertaining to
certain prohibitions for charcoal burners and other open -flame cooking devices.
(It was determined that certain sections be amended to include the type of fire
extinguisher required within ten (10) feet of the cooking device.
(The Fire Chief recommends approval.)
n. Consideration to adopt Ordinance No. 3601 amending Title 3, Chapter 5A, Food Service
and Retail Food Store Sanitation Codes of the Village Code of the Village of Elk Grove
Village.
(This Ordinance formally adopts the Illinois Food Code, which incorporates Chapters 1-7
of the 2017 FDA Food Code. Additionally, the code includes minor amendments and/or
deletions deemed necessary to address conditions specific to Elk Grove Village.
(The amended codes provide standardized regulations which are adopted by local
governments for application within their communities.
(The Director of Community Development recommends approval.)
