(JOHN F. KENNEDY/ELK GROVE BLVD) NOW, THEREFORE, BE IT RESOLVED by the President and Board of Trustees of the Village of Elk Grove Village, Counties of Cook and DuPage, State of Illinois as follows: Section 1: That the Village President be and is hereby authorized to sign the attached documents marked: "AGREEMENT FOR CONSULTANT SERVICES BETWEEN THE VILLAGE OF ELK GROVE VILLAGE AND TRITON CONSULTING ENGINEERS, LTD." said agreement to retain Triton Consulting Engineers to provide design services in accordance with their proposal dated September 9, 1993 and supplemental proposal dated September 23, 1993 at a cost plus fixed fee not to exceed $58,941.00 a copy of which is attached hereto and made a part hereof as if fully set forth and the Village Clerk is authorized to attest said documents upon the signature of the Village President. Morrison Boulevard from Arlington Heights Road to Elk Grove Boulevard, and Elk Grove Boulevard from Arlington Heights Road to Tonne Road, a distance of approximately 8650 lineal feet, in the Village of Elk Grove Village, Cook County, Illinois . AGREEMENT FOR CONSULTANT SERVICES Route: John F. Kennedy/Rev. Morrison Boulevard Arlington Heights Road to Elk Grove Boulevard Elk Grove Boulevard Arlington Heights Road to Tonne Road County: Cook This is an AGREEMENT between the VILLAGE OF ELK GROVE VILLAGE, COOK COUNTY, acting through its President and/or Village Board, hereinafter referred to as the "VILLAGE" , and Triton Consulting Engineers, Ltd. , 830 North Meacham Road, Schaumburg, Illinois 60173 , a firm of consulting engineers, hereinafter referred to as the "CONSULTANT" . The VILLAGE proposes to engage the CONSULTANT to furnish certain professional services in connection with the preparation of" Design Plans in connection with the proposed improvement of John F. Kennedy Boulevard from Arlington Heights Road to Elk Grove Boulevard, and Elk Grove Boulevard from Arlington Heights Road to Tonne Road, a distance of approximately 8650 lineal feet in the Village of Elk Grove Village, Cook County, Illinois, which work is hereinafter referred to as the "PROJECT" . The CONSULTANT, in signing this AGREEMENT, certifies that he (she) has no financial or other interests in the outcome of this project . 1 Under penalties of perjury, the CONSULTANT certifies that 36-3595778 is their correct Federal Taxpayer Identification Number and they are doing business as a Corporation. SCOPE OF WORK The CONSULTANT agrees to provide professional services as described in Exhibit D, Proposal to Furnish Professional Engineering Services, attached hereto and made part of this AGREEMENT: 1 .1 The CONSULTANT will provide a complete field survey, including topography, cross sections, utilities and drainage. 1.2 The CONSULTANT will prepare Contract Plans, Specifications, and Engineer' s Estimate of Construction Cost for the project . 1 .3 The CONSULTANT shall contact the utility companies and coordinate the required utility adjustments . The design and plans for utility adjustments will be prepared by the respective utility company. 1 .4 The CONSULTANT shall furnish the VILLAGE, the following documents, plans and copies thereof : a) Preliminary Design Studies : 4 copies b) Prefinal Contract Plans and Documents : 4 sets of prints c) Final Contract Plans : Reproducible and 20 sets of prints d) Special Provisions : Original and 20 copies e) Estimate of Time : Original f) Estimate of Cost : Original with prices and 1 copy without prices typed on forms (8 1/2" x 11" ) 3 2 . The VILLAGE agrees to furnish the following items : All presently available survey data, adjacent development plans, standard specifications and details adopted by the VILLAGE, and other available data useful to the work being done by the CONSULTANT. 3 . Prosect Schedule The CONSULTANT shall commence work on the preparation of preliminary design studies, contract plans and documents within five (5) calendar days after date of authority to proceed. The work will be completed and delivered to the VILLAGE within the following period: Contract Plans and Documents - 150 calendar days after notice to proceed. 4 . Prime Compensation 4 . 1 Design and preparation of contract plans and documents : Subject to an Upper Limit of Total compensation of $58, 941, including the Fixed Fee, the CONSULTANT shall receive as full payment for completing the design work required of him under this AGREEMENT, reimbursement of his eligible actual costs plus a Fixed Fee of $6, 875 . The maximum retainage for the PROJECT shall be $1, 768 . The attached Exhibits A, B, C and D are made part of this AGREEMENT. 4 4 . 2 The prime compensation may be increased or decreased by subsequent agreement between the contractual and approving parties if there is a major change in the scope, character or complexity of the work. Prime - Compensation and Specific Rates for Extra Work may by adjusted if the duration of the work in any phase extends more than six months beyond the date of completion, anticipated for March, 1994 , due to no fault of the CONSULTANT. 4 .3 In all instances where the VILLAGE shall request specific services of the CONSULTANT, the CONSULTANT prior to commitment shall indicate to the VILLAGE whether the work shall be considered "extra work" . When requested by the VILLAGE, the CONSULTANT shall prepare and submit a cost estimate for such extra work. DESCRIPTION OF FIRM The principals of Triton Consulting Engineers, Ltd. represent over sixty years of experience as professional consulting engineers in the field of municipal, urban transportation, and various other civil engineering disciplines . Engineering services performed by the firm include complete transportation studies; pedestrian and parking investigations; traffic impact studies ; design and land acquisition surveys; storm water drainage and hydraulic design; utility systems design; pavement analysis and design; traffic signal and lighting design; structural design; and construction layout, inspection and material testing. It is our practice to maintain a close liaison with our clients during the course of the project . We feel a close work relationship with our client together with a thorough understanding of the project will expedite the successful completion of the proposed improvement . The firm has a staff of 25 individuals which include registered professional engineers, registered structural engineers, surveyors, design engineers, technicians, and draftsmen. The offices for Triton Consulting Engineers, Ltd. are located at 830 North Meacham Road in Schaumburg, Illinois . -1- II . EXPERIENCE OF FIRM The staff of Triton Consulting Engineers, Ltd. has extensive experience in providing each of the following three engineering phases for municipalities : development of design studies and planning reports (Phase I) ; preparation of contract plans, specifications and documents (Phase II) ; and construction engineering and resident inspection (Phase III) . We provide our clients with comprehensive engineering services based on our experience in the following areas : • Roadway Design Railroad Engineering Construction Services • Structural Engineering Environmental Studies • Surveying • Hydraulics and Hydrology Transit Facilities • Bridge Design . Transportation Planning Permit Preparation Traffic Signalization • Right-of-Way Acquisition Traffic Studies • Roadway Lighting Utility Systems Our staff has successfully completed numerous Phase I design studies including alignment and cross section alternative analysis; traffic volume studies ; and presentation of project studies to the public through public informational meetings and formal public hearings . -2- Our staff has developed Phase II contract plans, specifications and documents for various street and highway projects similar to the J.F. Kennedy/Elk Grove Boulevard project . These projects have included pavement widening and resurfacing; pavement reconstruction; hydraulic and structural design; watermain and storm sewer design; lighting design; right-of-way acquisition; pavement marking; signing; and landscaping . Our staff has also assisted municipalities in the coordination of contract bidding and awarding of projects . Triton Consulting Engineers, Ltd. has qualified staff available to perform construction engineering and resident inspection services (Phase III) for the project . Construction services include construction staking and layout, resident inspection, documentation, quantity measurements and record keeping. We are prequalified by the Illinois Department of Transportation to provide the engineering services required for this project and have included our IDOT approved overhead and burden rates in Appendix A. We have included in Appendix A a detailed description of similar projects completed in the last three years along with a list of client references . -3- III . PROJECT STAFF We feel that the timely and successful completion of an engineering assignment is 'dependent upon the continuity of our design staff and their familiarity with the project requirements . We propose the following staff assignments for this project : Principal-In-Charge - Vito P . Sammarco, P.E. Project Manager - Charles J. Stenzel, P.E . Project Engineer - Dana L. Milne Design Engineer - Daniel J. Loftus Surveyor - Jeffrey Conrad Mr. Sammarco has extensive experience in F.A.U.S . , IDOT and municipal improvements in the northwest suburban area.. He has directed the preparation of feasibility studies, project reports and contract plans . Mr. Stenzel was selected as Project Manger based on his experience on municipal transportation projects and his past involvement on similar roadway, intersection, and utility projects . He is familiar with the policies and standards of the Village and has designed and coordinated other roadway improvements similar to the J. F. Kennedy/Elk Grove Boulevard corridor improvements . -4- Ms . Milne was selected as project Engineer based on her experience in all phases of State, County, and municipal engineering improvements • Ms . Milne has been responsible for the preparation of numerous project reports and contract plans and is familiar with current design policies and procedures . Ms . Milne has conducted public meetings as well as Village Board presentations . Mr. Loftus would serve as Design Engineer for the project . He has knowledge in the preparation of Phase I and II engineering for State and municipal projects including roadway reconstruction, hydraulic analysis, drainage, detention and right-of-way. Resumes for key personnel proposed for assignment to the project are included in Appendix B . our current staff guarantees sufficient personnel for assignment to the project ensuring the timely completion of the project . Testing Service Corporation would be subcontracted to perform pavement cores if requested by the Village . -5- IV. PROJECT APPROACH The staff of Triton Consulting Engineers, Ltd. has extensive experience in the planning, design, and construction assistance for roadway improvements . We feel the timely and successful completion of the proposed improvement may be achieved through the proper coordination of the project with the Village, residents of the area, and other impacted agencies . We will ensure the necessary project coordination through overseeing all aspects of the project administration in addition to the detailed design engineering services for the various project phases . We have reviewed the Request for Proposal dated August 17, 1993 ; met with the Village on August 27, 1993 ; and have investigated the site of the roadway improvements . The proposal includes engineering services for the preparation of contract plans, specifications, bid documents, construction estimate of costs, and associated permit documentation for the John F . Kennedy/Rev. Morrison Boulevard, corridor from Arlington Heights Road to Elk Grove Boulevard and the Elk Grove Boulevard corridor from Arlington Heights Road to Tonne Road. The total project length is approximately 8, 650 lineal feet . -6- SCOPE OF ENGINEERING SERVICES 1 . Project Coordination and Data Collection Preliminary conference with Village staff to discuss goals and objectives of the project as well as establish schedule for completion. - Obtain available data from the village including utility atlases, aerial mapping, recent drainage improvements and permit information. - Perform peak hour and/or pedestrian traffic counts at the following intersections to determine the requirement for traffic control measures : - John F. Kennedy Boulevard and Brantwood Avenue - Rev. Morrison Boulevard and Elk Grove Boulevard - Arlington Heights Road and Elk Grove Boulevard - Elk Grove Boulevard and Ridge Avenue - Elk Grove Boulevard and Tonne Avenue - Meet with Village Staff to review comments on the preliminary design studies, final plans, and final contract documents . 2 . Field Survey - Obtain local field survey data - Perform field survey to identify existing conditions including topography, cross sections, driveways, right- of-way and drainage . - Plot design survey at 1" = 50' scale. 3 . Soils Investigation (as required) Conduct soils investigations to identify pavement condition, soils stability, and subgrade conditions . Pavement cores would be reviewed to identify pavement replacement requirements . - Prepare soil investigation report to identify subsurface needs, pavement replacement limits, and pavement design requirements . -7- 4 . Preliminary Design Studies - Develop proposed roadway profiles and horizontal alignment and typical section for the project with respect to geometric modifications, drainage, and driveway grades . - Prepare supplemental data for the Illinois Department of Transportation/Division of Water Resources (IDOT/DOWR) permit submitted for the Rev. Morrison Boulevard and Elk Grove Boulevard median drainage ditch enclosure project . - Evaluate traffic data to determine the need for intersection traffic control measures including traffic signs, traffic signals, and pedestrian crosswalks . - Analyze construction staging and maintenance of traffic to minimize local impacts and ensure construction safety. - Perform analysis of existing drainage patterns, to identify storm sewer replacement needs to provide storage for the change in impervious area created by widening the routes . - Review and discuss the results of the preliminary design studies with the Village. 5 . Public Meetings (as required) Prepare exhibits for Village Board or public meeting. Presentation of preferred improvement plan to Village Board and/or the area residents . 6 . Preparation of Contract Plans and Documents It is anticipated that the following sheets will be included in the contract plans : Description a . Title Sheet 1 b . Index of Sheets/Plan Notes 1 C . Prepare bidding documents and special provisions based on Village standards . IDOT and Village highway and utility standard drawings will be included in special provisions . 7 . Preparation of Final Contract Documents and Bidding Assistance - Based on comments received from the Village, preparation of final contract plans, specifications and documents . Provide 20 sets of final contract plans and documents to the Village for bidding purpose . Prepare bid tabulations, perform reference checks, and make recommendation of selected bidder. S . Enclosed Drainage System Perform analysis of existing drainage patterns along Elk Grove Boulevard from Arlington Heights Road to Rev. Morrison Boulevard to determine storage in existing ditch system. - Design enclosed storm sewer system to replace the existing paved ditch drainage system. - Identify trees to be transplanted due to the enclosure of the ditch system. - Incorporate the drainage design in the contract plans for the project . 9 . Traffic Signal Modernization Prepare temporary signal plan, permanent signal plan, cable plan, sequence of operations, summary of quantities and contract specifications for the modernization of the Elk Grove Boulevard/Arlington Heights Road traffic signal . Investigate need for pedestrian signals for Elk Grove Boulevard at the Arlington Heights Road intersection. -9- Incorporate traffic signal plans and specifications into the contract plans and documents .. Prepare final contract plans and documents based on comments received from the Village and the Cook County Highway Department . Assist the Village in obtaining the necessary permits for the traffic signal work from the Cook County Highway Department and the Cook County Forest Preserve District . Assist the Village in obtaining funding participation for the traffic signal improvements from the Cook County Highway Department . -10- V. PROJECT SCHEDULE AND BUDGET Triton Consulting Engineers, Ltd. has the experience and manpower required to efficiently complete the project to meet the Village' s schedule . Based on a September 20, 1993 Notice to Proceed, the schedule provides .for the completion of Preliminary Design by November 15 , 1993 , and Final Design by February 15 , 1994 . This will provide for a contract letting in March of 1994 . A project schedule is included in Appendix D. The estimated manhours to complete the engineering services as herein outlined are 1238 manhours . We propose to perform the necessary professional consulting engineering services for the project on a cost plus fixed fee basis with an upper limit of compensation of $58, 941 . The details of the manhour estimate and fee determination are shown in Appendix D. The following design services would be considered extra work, per our discussions with the Village, and have not been included in the Cost Estimate for Consultant Services : 1 . N. 11,586 - September 24, 1993 On August 26, 1993, a site trip was made to observe field conditions and determine the equipment necessary to perform the boring and sampling work. It was noted that construction is taking place along the interior median of Rev. Morrison Boulevard and Elk Grove Boulevard for the placement of a large culvert. The construction has closed off a significant portion of the roadway with frequent spoil piles, barricades and other obstructions along the roadways. It will be required to perform the Geotechnical Study for this project after the culvert construction operations are completed and the site Is returned to normal. Your Geotechnical Engineering Study will be performed under the general supervision and review of Mr. Charles C. Luther, P.E., Mr. Michael V. Machalinski, P.E. or Dr. Lawrence A. DuBose, P.E., S.E. They are each graduate Civil Engineers, having at least fifteen years of experience in Geotechnical Engineering with most of their work being in northeast Illinois. The proposed scope of work is to include soil borings for ail areas of pavement widening. Three (3) borings are proposed along the interior median of J. F. Kennedy Boulevard, each to be extended to a minimum depth of 10 feet for sewer improvements. Twelve (12) borings are proposed along the interior median or exterior parkway of Elk Grove Boulevard, to be extended to a minimum depth of 5 feet. Illinois Bearing Ratio tests will be performed on representative subgrade samples from J. F. Kennedy Boulevard and from Elk Grove Boulevard. The site reconnaissance indicates the existing pavements for this project are generally in poor to fair condition. Indications are that the pavement consists of bituminous concrete over aggregate base course. Our cost proposal includes fourteen (14) pavement cores to determine the pavement composition throughout the limits of this project. The 4" diameter cores are to be taken of the pavement and base course materials. No subgrade sampling is to take place at the core locations. The borings and cores will be located in the field using taping procedures based on your site plan. This information along with site observations made by technicians will be included in the Geotechnical Report. Utility clearances will be obtained prior to the start of drilling by contacting JULIE (Joint Utility Locating Information for Excavation), local municipalities and associated agencies. Soil samples will be obtained by auger methods at maximum 2 foot intervals for the full depth of the borings. Representative portions of all samples will be sealed, packaged and transported to our laboratory. Groundwater observations will also be made during drilling. -2- TRITON CONSULTING ENGINEERS P. Field and laboratory test data will be summarized in an engineering report, to include a boring location plan as well as individual boring logs. The report will address anticipated soil and groundwater conditions impacting the roadway improvements, based upon the information obtained from the borings. It will also provide specific recommendations to guide treatment of unstable or unsuitable soil types. Our cost proposal follows, further defining the detailed scope of work.
