A RESOLUTION AUTHORIZING AN AGREEMENT BETWEEN THE VILLAGE OF ELK GROVE
VILLAGE AND AMERICAN TAXI,INC. FOR SENIOR CITIZEN/HANDICAPPED PERSON

NOW, THEREFORE, BE IT RESOLVED by the President and the Board of Trustees
of the Village of Elk Grove Village, Counties of Cook and DuPage, State
of Illinois:
Section 1. That the Village President be and is hereby authorized to
sign the attached documents marked "Agreement for Senior Citizen/Handi-
capped Person Taxicab Service" a copy of which is attached hereto and
made part hereof as if fully set forth and the Village Clerk is author-
ized to attest said documents upon the signature of the Village President.
Section 2. That this resolution shall be in full force- and effect
from and after its passage and approval according to law

AGREEMENT FOR SENIOR CITIZEN/HANDICAPPED PERSON
TAXICAB SERVICE
THIS AGREEMENT entered into this 13th day of July 1982,
by and between the VILLAGE OF ELK GROVE VILLAGE, an Illinois municipal
corporation (herinafter referred as the "Village") and AMERICAN TAXI, INC.
(herinafter referred to as the "Cab Company");
W I T N E S S E T H:
1.) That the Village of Elk Grove Village is an Illinois Home Rule
Municipality and as such is empowered to enact certain regulations and enter
into certain agreements as authorized by the Constitution of the State of
Illinois.
2.) That the Cab Company is duly licensed to provide taxicab service
within the Village.
3.) That the Village has been advised that the senior'citizens/handicapped
persons of the Village are frequently in need of taxicab service, including
trips for necessary medical care and treatment, but that the cost thereof imposes
a financial burden on them.
4.) That the Village in the exercise of its home rule authority is
desirous of subsidizing the taxicab fares normally required of senior citizens/
handicapped persons to afford them the opportunity of transportation at a
reasonable cost to them.
5.) The Cab Company has indicated its intent to fully cooperate in said
subsidy program, subject to such conditions as hereinafter set forth, and has
indicated an ability to serve the senior citizens/handicapped persons at the
fare price contained herein.
NOW, THEREFORE, IT IS HEREBY AGREED by and betweenthe parties hereto as
follows:
DEFINITIONS.
For the purpose of this Agreement, the following definitions shall apply:
"Senior Citizen" shall include all residents of the Village sixty
(60) years of age or older who have registered and received identification
cards from the Village Clerk's Office.
"Handicapped Person" shall mean a handicapped person having a
Medicaid or Medicare card issued by the State of Illinois and/or a
statement from a doctor stating a permanent total disability of a long
term origin, which is expected to continue, and who have registered and
received an identification card from the Village Clerk's Office.
"Trip" shall mean the boarding of a passenger or passengers at one
location and travelling to one destination point, and shall not include
round -trips after short stopovers or trips with more than one stop -off
point.
II.
RATES.
Notwithstanding the normal rates for taxicab service as is set forth in
Section 18.4213 of the Municipal Code of the Village, the Cab Company shall
provide taxicab service to all registered senior citizens/handicapped persons
of the Village at the following rates:
(A). A flat fee of One Dollar and Seventy -Five Cents (1.75) shall be
charged for all senior citizens/handicapped persons for one-way trips within
the Village limits, Fifty Cents ($.50) of which shall be charged to the
passenger and One Dollar and Twenty -Five ($1.25) billed to the Village as
hereinafter provided.
(B). Any trip beyond the Village limits shall be charged at the then
applicable taxicab rates charged for regular service. The total fare shall
be reduced by One Dollar and Twenty -Five ($1.25), which amount shall be billed
to the Village as hereinafter provided.
III.
ELIGIBILITY.
In order to be eligible for the subsidy taxicab program, a senior
citizen/handicapped person as herein defined must register with the Village
Clerk and receive an identification card. The Village ID Card must be
presented to the cab driver upon entering a taxi. No other source of
identification shall be honored by the Cab Company.
IV.
TRIP TICKETS.
The Village will provide the Cab Company with trip tickets, which
tickets shall require the recording by the driver of such information as the

Village deems necessary, plus the signature of the senior citizen/handi-
capped person. The Cab Company shall submit copies of the trip tickets to
the Village on a monthly basis, or at such other time interval as is mutually
agreeable and shall receive One Dollar and Twenty-five cents ($1.25) for each
signed ticket submitted. Failure to have a trip ticket signed by the senior
citizen/handicapped person passenger will preclude payment by the Village.
V.
TIPLE PASSENGERS.
(A). If two or more senior citizens are picked up at the same location
and travel to the same destination point, the fare will be either the flat
rate, if within the Village, or the meter rate, if outside the Village limits,
plus Ten Cents ($.10) per additional passenger. In both instances, the fare
will be reduced by One Dollar and Twenty-five cents ($1.25), which shall be
paid by the Village as hereinabove provided, and only one trip ticket shall
be signed. If two or more senior citizens are travelling together, and
have separate destination points, a separate fare will be charged for each
separate stop, and separate trip tickets must be signed.
(B). If a senior citizen and non -senior citizen travel together, the
subsidy shall not be in effect and the passengers shall pay the regular
meter rate.
(C). If a handicapped person and non -handicapped person travel together
the subsidy shall not be in effect and the passengers shall pay the regular
meter rate.
VI.
DISCONTINUANCE.
The Village shall have the right to discontinue the subsidy program upon
giving thirty (30) day written notice to the Cab Company, advising them of the
Village's intent to cancel such service.
VII.
INSURANCE.
The Cab Company shall make the Village an additional party insured on
each insurance policy in effect.

VIII.
VIOLATIONS.
If the Cab Company violates any of the provisions of this Agreement,
dr fails to render proper service to senior citizens/handicapped persons,
all licenses of such Company shall be subject to suspension or revocation
pursuant to hearing and notice by the Public Vehicle Commission of the
Village.
IN WITNESS WHEREOF the parties hereto set their hands and seals this
