A RESOLUTION AUTHORIZING THE MAYOR AND VILLAGE CLERK TO
EXECUTE A FINANCIAL ASSISTANCE AGREEMENT BETWEEN THE VILLAGE
OF ELK GROVE VILLAGE AND THE ILLINOIS ENVIRONMENTAL PROTECTION
AGENCY (IEPA) FOR THE SALT CREEK STREAMBANK STABILIZATION
PROJECT)
NOW, THEREFORE, BE IT RESOLVED by the Mayor and Board of Trustees of the
Village of Elk Grove Village, Counties of Cook and DuPage, State of Illinois as follows:
Section 1: That the Mayor be and is hereby authorized to sign the attached documents
marked:
Financial Assistance Agreement
AMENDMENT
Illinois Environmental Protection Agency
Bureau of Water
a copy of which is attached hereto and made a part hereof as if fully set forth and the Village
Clerk is authorized to attest said documents upon the signature of the Mayor.
Section 2: That this Resolution shall be in full force and effect from and after its
passage and approval according to law.

The period of performance covered by this agreement shall be from November 1, 2003 through
September 30, 2006. No services will be paid which are performed prior to or subsequent to
this performance period.

Project Description/Scope of Work
SALT CREEK STREAMBANK STABILIZATION — ELK GROVE VILLAGE
The end date of the Agreement Period is changed from September 30, 2005 to September 30,
2006, ,
The ProjectDescription/Scope of Work Is amended to reflect the following:
In Item 3, thelast two sentences are deleted in their entirety and replaced with "The Recipient
shall complete the installation and construction of the streambank stabilization practices by April
1, 2006. Photographic slides and prints documenting Design implementation shall be
completed and submitted by the Recipient to the Illinois EPA by May 1, 2006.
In Item 4, the last two sentences are deleted in their entirety and replaced with "Two (2) copies
of the draft report shall be completed and submitted by the Recipient to the Illinois EPA by
August 1, 2006. The final report shall be completed and three (3) paper copies and one (1)
electronic copy submitted by the Recipient to the Illinois EPA by September 30, 2006.
In Item 7, the last two sentences are deleted in their entirety and replaced with "A draft of all O
& M Plans shall be completed and submitted by the Recipient to the Illinois EPA by April 1,
2006. The final O & M Plans shall all be completed and submitted by the Recipient to the
Illinois EPA by August 1, 2006.
