6. CONSENT AGENDA
A) Consideration of a request (attached) from Alexian
Brothers Medical Center to waive permit fees in the amount
of $10,300 for the Cath/PIR Expansion Project.
(It has been past practice of the Village Board to
grant fee waivers for Alexian Brothers Medical Center and
non-profit organizations.
(A report from the Director of Engineering and
Community Development, recommending approval, is
attached. )
B) Consideration to award a professional service contract to
W.J. Electric Inc. , of Bridgeview, IL to furnish, deliver
and install a generator power system at Pump House #11 in
a -total amount not to exceed $55,289, from the Water &
Sewer Fund.
(The new generator will operate two booster pumps, the
SCDA control system and two natural gas fired furnaces.
(Requests For Proposals were solicited from nine (9)
vendors and three (3) responded. It was determined that
W.J. Electric was the most responsive and responsible
bidder.
(W.J. Electric has performed similar types of design
and installation for the Village in the past with
satisfactory results.
(Sufficient funds are budgeted and available for this
purpose.
(Reports from the Director of Public Works and
Director of Finances, recommending approval, are
attached. )

C) Consideration to adopt Ordinance No. 2975 (attached)
granting a Special Use Permit to US Cellular to permit the
installation of an equipment cabinet and the mounting of a
cellular antenna on an existing storage tank on property
located at 2727 East Higgins Road.
(This item was discussed at the January 27, 2004
Village Board meeting and currently appears on the Agenda
under Unfinished Business of the Village Attorney. )
D) Consideration to adopt Ordinance No. 2976 (attached)
amending Sections 6-2-2 of Chapter 2 Vehicle Licenses of
the Village Code by establishing penalties for individuals
purchasing their vehicle licenses after the due date.
(This amendment would provide penalties of 50% for
individuals who have not purchased their vehicle stickers
after July 1" each year. Additionally, seniors and
handicapped would have to pay full price for the stickers
purchased after July 1ib.
(Any person, firm or corporation who has failed to
purchase a vehicle license in any one or more preceding
years prior to the purchase of the current vehicle
license, shall be required to pay all prior delinquencies,
plus penalty if applicable, before being eligible to
purchase the current year's license.
(This item was discussed at the January 14, 2004 JPZ
meeting and again at the January 27, 2004 Village Board
meeting. )

a) ZBA Docket 03-9 - Variation request to construct a two-story
addition that would result in a floor ratio of 0.48 or cover an
area equal to forty-eight percent (48%) of the area of the lot
at 627 Delaware Lane (PH 11-11-03)(Remanded back to ZBA 2-12-
04)
b) ZBA Docket 03-10 - Variation request to enclose an existing
second floor outdoor deck that would result in a floor area
ratio of 0.48 or cover an area equal to forty-eight percent
(48%) of the area of the lot at 1083 Florida Lane (PH 11-11-03)
c) ZBA Docket 04-1 - Variation request for variations to allow the
construction of a detached accessory structure having an area
of 3,500 sq. feet with a length in excess of 25 feet and a
height of 22 feet located 48 feet from the front property line
at 1500 Touhy Avenue. (PH 2-26-04)

a) Direct the Village Attorney to prepare an Ordinance
amending the Village code as it pertains to Vehicle
stickers.
b) Direct the Village Attorney to prepare an Ordinance
approving a Special Use Petition and approving variations
with respect to the height of a structure and fence at
2727 E. Higgins Road.
c) Direct the Village Attorney to prepare an amendment to
the Dierking Terrace contract.
