Consideration of a request from Fraternal Order of Police, located at 901 Wellington Avenue, Elk Grove Village,to waive the FY 2016-17 business license fees in the amount of$50. (The Fraternal Order of Police is respectfully requesting the Business License fee in the amount of$50 to be waived for the one vending machine that is located at Village Hall. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Finance recommends approval.) b. Consideration of a request from Queen of the Rosary School, located at 690 Elk Grove Blvd.,to waive the FY 2016-17 business license fees in the amount of$50. (The Queen of the Rosary School is respectfully requesting the Business License fee in the amount of$50 to be waived for one vending machine that is located at 690 W. Elk Grove Blvd. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Finance recommends approval.) c. Consideration of a request from Elk Grove Park District, located at 1000 Wellington Pagel of 8 Avenue,to waive the FY 2016-17 business license fee for the June 12 Art in the Park event in the amount of$135. (The Elk Grove Park District is respectfully requesting their Business License fees in the amount of$135 to be waived for Temporary General Retail License and Food Caterers License for the June 12, 2016 Art in the Park event. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Finance recommends approval.) d. Consideration of a request from Prince of Peace United Methodist Church, located at 1400 Arlington Heights Road, to waive the FY 2016-17 business license fees in the amount of $325. (The Prince Of Peace Church is respectfully requesting the Business License fee for 2016- 17 in the amount of$325 to be waived for their preschool located at 1400 S. Arlington Heights Road in Elk Grove. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Finance recommends approval.) e. Consideration of a request from Elk Grove Township Community Day Care Center, located at 711 Chelmsford,to waive the FY 2016-17 business license fees in the amount of$475. (Elk Grove Township is respectfully requesting their Business License fees for 2016-17 in the amount of$475 to be waived for the Community Day Care Center, one vending machine and their Food Processor license to be used at 711 Chelmsford in Elk Grove. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Finance recommends approval.) f. Consideration to award a purchase contract off the National IPA cooperative purchase agreement with Canon Solutions America of Schaumburg, IL to provide replacement copy machines in the amount of$29,485. (The Village's budget contains funding to replace the copiers in the Village Clerk's Office, Community Development Department, and the centrally located Mail Room. (These copiers range in age from 7-10 years and have each processed between .5 million and 1.5 million copies. (The pricing provided by the recommended vendor reflects the National IPA cooperative purchase agreement with all discounts and credits applied. (The National IPA Program utilizes the County of DuPage Contract No. CP-002-13 dated October 1, 2013. (The Director of Information Technology recommends approval.) g. Consideration to award a contract with C.C. Carthage, Inc. of Lake Zurich, IL to Furnish Page 2 of 8 and Deliver Sand and Gravel in an amount not to exceed$36,640.90 from the General Fund and the Water-Sewer Fund. (On Tuesday, May 10, 2016,the Village opened sealed bids for the Furnish and Deliver Sand and Gravel contract. Four(4) contractors obtained bid documents and three (3)bids were submitted. (This contract provides for the supply and delivery of various types of gravels and sands to Village facilities. (The initial term of the contract is from June 1, 2016 through April 30, 2017,with the option of four(4) annual renewals in one-year increments through April 30, 2021. (The lowest responsive and responsible bid was received from C.C. Carthage, Inc. of Lake Zurich, IL in the amount of$36,640.90. (Funds for the contract have been allocated in the Public Works General Fund and Water- Sewer Fund. (The Director of Public Works recommends approval.) h. Consideration of a request from the Illinois Tollway, 2700 Ogden Avenue, Downers Grove, IL to waive building and engineering permit fees in the amount of$6,235 for work associated with the temporary construction trailer at the Illinois Department of Transportation's yard at 1101 Biesterfield Road. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.) i. Consideration to award a service contract to JG Uniforms of Chicago Illinois for the purchase of police officer uniforms in an amount not to exceed $45,000. (The Police Department has purchased uniforms and equipment from Apparel Sewn Right (ASR/Kale Uniforms, Inc.)for the last twenty years. (In June 2015,the Village received notice that ASR/Kale Uniform, Inc. was closing. JG Uniforms agreed to fulfill all orders under the contract with ASR/Kale maintaining the same terms. The Village Board approved this contract on August 18, 2015. (For the 2017 budget JG Uniforms is requesting a 5%increase in its prices, as there has been no increase in uniform prices in the last 5 years. This increase is reasonable and the department would like to continue to utilize JG Uniforms. (Some of the benefits provided by JG include:they offer hand measured and sewn ballistic vest carriers;they have the ability to work with several manufacturers of uniform specialty items which hold costs down; they provide uniforms to approximately 100 other police departments which allows JG to provide discounts due to high volume and also provides acceptable delivery expectations as most of the uniform items are in stock; they possess all of the Department's specialty embroidery set ups; and they do not charge for delivery or restocking fees. (For the above reasons I am requesting that the Village Board waive the bidding process and award a service contract to JG Uniforms for the 2017 fiscal year not to exceed $45,000. Consideration to concur with prior authorization and waive the formal bidding procedures and award a purchase contract to Mauritzon, Inc. of Chicago, IL for a 14 oz. Flame Resistant Athletic Field Tarp in the amount of$48,640.00 from the General Fund. (The Village solicited quotes from three (3)area tarp manufacture for 128,000 square feet of 14 oz. flame resistant vinyl coated polyester tarp to cover the Elk Grove Village High School Football Field during Village firework shows. (Mauritzon, Inc. of Chicago, IL submitted the lowest quote. (Adequate funds are available in the General Fund. (The Director of Public Works recommends approval.) k. Consideration to award a purchase contract to Currie Motors of Frankfort, IL in the amount of$155,156 for the purchase of five(5) Ford Utility Interceptors. (The Police Department is replacing three units due to mileage and age and one hybrid unit due to age. The Fire Department is replacing one unit due to mileage and age. (The Ford Utility Interceptors are available through the Suburban Purchasing Cooperative (SPC)Joint Purchasing Contract from the lowest determined bidder, Currie Motors of Frankfort, IL. (Adequate funds are budgeted in the General Fund. (The Director of Public Works recommends approval.) 1. Consideration to award a purchase contract through the Suburban Purchasing Cooperative (SPC)Joint Purchasing Contract to Foster Coach Sales of Sterling, IL for the purchase of one (1) Ford F-550 ambulance with custom Horton Body in an amount not to exceed $210,000 from the Capital Replacement Fund. (The Fire Department is replacing one ambulance due to age and excessive engine hours. (The Ford F-550 ambulance is available through the Suburban Purchasing Cooperative (SPC)Joint Purchasing Contract from the lowest determined bidder, Foster Coach Sales of Sterling, IL. (Adequate funds are budgeted in the Capital Replacement Fund. (The Fire Chief recommends approval.) m. Consideration to award a professional service contract to Swanson Contracting Co., Inc. of Alsip, IL for the improvement of seven(7)railroad crossings located on Touhy Ave., Estes Ave., Greenleaf Ave., Lunt Ave., Morse Ave., Mark St. and Carl Blvd. in the amount of$632,469.20 from the BLF Fund. (The Village requested a proposal from the rail spur line operator, Chicago Terminal Railroad, for improvement of seven separate crossings with the Business Park, Touhy Ave., Estes Ave., Greenleaf Ave., Lunt Ave.,Morse Ave., Mark St.and Carl Blvd. (The rail spur line operator, Chicago Terminal Railroad, has provided the Village with an estimated cost proposal from their construction company, Swanson Contracting Co., Inc., for the improvement of the crossings. Page 4 of 8 (The Director of Public Works recommends approval.) n. Consideration to award a professional service contract with Alliance Contractors, Inc. of Woodstock, IL for the 2016 Crosswalk Rehabilitation Contract in the amount of $335,448.70 from the Residential Enhancement Fund. (On Tuesday, May 10, 2016,the Village opened sealed bids for the 2016 Crosswalk Rehabilitation Contract. (Five(5) contractors obtained bid documents and three (3) submitted bids as shown on the attached bid tabulation of contractors' bid prices. (The lowest responsive and responsible bid was received from Alliance Contractors, Inc. of Woodstock, IL. (This contract provides for removal of brick crosswalk pavement at various locations across Village roadways and the replacement with new brick crosswalks or integrally colored concrete crosswalks. (Locations for work proposed in this contract include the intersections at Biesterfield Road and Michigan Lane, Biesterfield Road and Home Avenue,Biesterfield Road and West Glen Trail,Biesterfield Road and Grissom Trail, and Rohlwing Road and White Trail. (Adequate funds are available in the Residential Enhancement Fund. (The Director of Public Works recommends approval.) o. Consideration to adopt Resolution No. 16-16 (attached) authorizing the Mayor to execute a Letter of Receipt stipulating the terms of an agreement for an application seeking a Cook County Class 6B Property Tax Exemption status pursuant to the Cook County Real Property Classification Ordinance as amended February 15, 2006 for certain real estate located at 2300 Arthur Avenue. (The Law Offices of Eugene L. Griffin&Assoc. on behalf of CR Express Inc. (Applicant), is seeking a Cook County Class 6B Property Tax Exemption for property located at 2300 Arthur Avenue. (The Applicant intends on purchasing the building to house its growing airfreight logistics business. (The Applicant is currently located in Des Plaines at 1797 Winthrop Drive. The Applicant currently employs sixty-one people all of which will be relocated to Elk Grove. Additionally,the Applicant intends to add an additional sixty jobs. (The subject property consists of an approximately 109,783 square foot building sitting on approximately 4.7 acres. (The Applicant intends on spending$2 million to refurbish both the interior and exterior of the building. Including:ten new docks on the east side; new parking lots per Village code;new landscaping; new fagade; new internally illuminated signage; new exterior lighting;remove and replace the east dock and drive-in door per Village code; both parking lot entrances shall be widened to 24' for two-way traffic per Village code; remove and replace both parking lot aprons per Village code; the drainage ditch shall be cleared of all trees and invasive brush; restore to a grass lined ditch; all storm structures shall be cleared of all debris and the functionality of the storm sewers shall be verified. (The goal of Class 6B is to attract new industry, stimulate expansion and retention of existing industry and increase employment opportunities. The eligibility requirements for Page 5 of 8 6B status are new construction, substantial renovation or buildings that have been vacant for a period of time. (This site qualifies as it involves occupation of an abandoned building for more than twenty-four(24)months continuous vacancy with a purchase for value, and significant rehabilitation. (Property approved for Class 6B status allows the owner of the property to have the assessment level lowered for a period of twelve years. The assessment schedule is 10%of fair market value for ten years then 15% in the eleventh year and 20% in the twelfth year. Industrial property is currently assessed at 25%of fair market value in Cook County. (The Director of Business Development and Marketing recommends issuing a Letter of Receipt.) p. Consideration to adopt Resolution No. 17-16 (attached)amending Resolution 35-00 and authorizing the Mayor and Village Clerk to execute a First Amendment to the Ground Lease Agreement between the Village of Elk Grove Village and SBA 2012 TC Assets, LLC at property located at 1751 Greenleaf. (This amendment would extend the Ground Lease Agreement for 1751 Greenleaf between SBA 2012 TC Assets, LLC and the Village of Elk Grove an additional four(4), five-year terms at the end of the term expiring September 30, 2025. The new final Ground Lease Agreement expiration date will be on September 30, 2045. (The amendment provides for a renegotiation date of October 1, 2035. (The amendment would also permit SBA 2012 TC Assets, LLC to sublease a portion of their lease area at 1751 Greenleaf to T-Mobile for the purpose of installing cellular equipment and building an equipment shelter. (The amendment would necessitate an addition of 5'x 20' area of ground space on the southeast corner of the leased property, covering a vacated access and utility easement previously used by SBA 2012 TC Assets, LLC. (As part of the amendment, SBA 2012 TC Assets, LLC would pay an additional $1,000 per month in rent with a 4%annual escalator. Additionally, for our consideration they have agreed to provide a one-time payment of$15,000.
