a. Consideration of a request from St. Julian Eymard Catholic Church, located at 601
Biesterfield Road,to waive permit fees for parking lot maintenance in the amount of$25.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.
(The Director of Community Development recommends approval.)
b. Consideration of a request from the Salvation Army, located at 1000 Pratt Blvd.,to
waive permit fees for parking lot maintenance in the amount of$99.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.
(The Director of Community Development recommends approval.)
c. Consideration of a request from the Alexian Brothers Medical Center, 800 Biesterfield
Road,to waive engineering permit fees to remove and replace sidewalk in the amount of
$180.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.

d. Consideration of a request from the Veterans of Foreign Wars,located at 400 E. Devon
Avenue,to waive permit fees for parking lot maintenance in the amount of$421.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.
(The Director of Community Development recommends approval.)
e. Consideration of a request from the Elk Grove Park District to waive permit fees in the
amount of$495 for parking lot maintenance at Al Hattendorf Community Center located
at 225 E. Elk Grove Blvd.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.
(The Director of Community Development recommends approval.)
f. Consideration to authorize payment of$2,000 in dues to the Illinois Municipal League for
Fiscal Year 2017-2018.
(The Illinois Municipal League(IML)represents municipalities on legislation pending
before the General Assembly.
(In addition,the Village dues payment includes subscriptions to the Illinois Municipal
Review,a monthly publication that contains articles on programs and findings of member
municipalities.
(Sufficient funds are budgeted and available for this purpose in the General Fund.)
g. Consideration of a request from the Lutheran Church of the Holy Spirit, located at 150
Lions Drive,to waive building permit fees to construct an addition to the chancel and
modify the congregation area in the amount of$6,211.00.
(It has been past practice of the Village Board to grant fee waivers for governmental and
non-profit organizations.
(The Director of Community Development recommends approval.)
h. Consideration to waive formal bidding procedures and award a purchase contract to
Verdin Company of Cincinnati, OH to furnish and install a replacement clock mechanism
and sound equipment in the amount of$23,510.00 from the Grove Mall TIF Fund.
(The Village solicited proposals from two(2)firms that provide these types of clock
mechanisms and sound systems. Evaluation of the proposals ranked the proposal from the
Verdin Company as more responsive and and a better integration into the existing
Clocktower components that will remain.
(Adequate funds are available in the Grove Mall TIF Fund.
(The Director of Public Works recommends approval.)
i. Consideration of the following:

To increase the Cable Television Fund budget in the amount of$38,000;and
To award a contract to the lowest responsive and responsible bidder VSA, Inc of Buffalo
Grove, IL for the purchase and installation of a three(3) Sony PTZ camera package in an
amount not to exceed$37,411 from the Cable Television Fund.
(The cameras in Room E of Village Hall,used for coverage of Committee of the Whole
meetings,have experienced catastrophic failure.
(These units are 27 years old and replacement parts are completely unavailable due to the
manufacturer going out of business.
(The old units can be replaced with the same model as are currently used in the Council
Chambers, Sony BRC-Z700 PTZ HD cameras.
(In order to replace these units,on November 14,2016 the Village opened sealed bids for
the purchase of a three(3) Sony PTZ camera package.
(A total of two(2)contractors submitted bids.
(The lowest responsive and responsible bid was received from VSA, Inc.of Buffalo Grove,
IL
(The Cable Production Coordinator recommends approval.)
j. Consideration to award a professional services contract to GOVTEMPUSA, LLC of
Northbrook,IL for office support services for the period from January 1,2017 through
December 31, 2017 in an amount not to exceed$43,680.
(The requested amount would provide 3 -4 days per week of clerical and customer service
support during phase four of implementing the utility billing module.
(The requested support will only be used and billed on an as-needed basis.
(The Director of Finance recommends approval.)
k. Consideration to award a purchase contract through the State of Illinois Central
Management Services to CDS Office Technologies of Itasca, IL for the purchase of six (6)
Panasonic Toughbooks for fire apparatus and ambulances in the amount not to exceed
$28,500 from the General Fund.
(The Fire Department has budgeted for the replacement of all apparatus and ambulance
laptops,as part of the Village's IT equipment replacement cycle.
(Field testing was completed on laptop computers provided by various manufacturers.
(The Fire Department is requesting to purchase six (6)fully-rugged Panasonic Toughbook
model#CF-20 for the patient care area of the ambulances.
(The Panasonic Toughbooks are available with pricing established by the State of Illinois
Central Management Services Joint Purchasing Contract#CMS5848520.
(Adequate funds are budgeted and available in the General Fund.
(The Fire Chief recommends approval.)
1. Consideration of the following:
• To adopt Ordinance 3471 (attached)authorizing the Levy and Collection of Taxes
for the Corporate and Municipal purposes of the Village of Elk Grove Village for
the Fiscal Year beginning May 1, 2017 and ending April 30,2018;

• To adopt Ordinance 3472(attached)abating a portion of the 2016 Property Taxes
Levied for certain taxable General Obligation Bonds, Series 2010,of the Village of
Elk Grove Village;and
• To adopt Ordinance 3473 (attached)abating a portion of the 2016 Property Taxes
Levied for certain taxable General Obligation Bonds, Series 2013,of the Village of
Elk Grove Village.
(This will be the tenth consecutive year that the Village Board is holding the line on
property taxes.
(The Library request is up$271,058 or 5.45%from last year.
(The 2016 Property Tax Ordinance in the amount of$25,671,681 includes the
Library. Ordinances abating$2,470,609 are included,thus reducing the net levy to
$23,201,072.
(The Tax Levy Ordinance must be approved by the Mayor&Board of Trustees and filed
by the Village Clerk with the County Clerks of Cook and DuPage Counties on or before
the last Tuesday in December in accordance with State Statutes.
(The Director of Finance recommends approval.)
in. Consideration to adopt Ordinance No. 3474 amending Title 3, Chapter 3, Section 3-3-7
Number of Licenses of the Village Code of the Village of Elk Grove Village by
increasing the number of Class"D" Liquor Licenses.
(This Ordinance increases the Class"D" Liquor Licenses from eight(8)to nine(9).)
n. Consideration of Ordinance No.3475 correcting a scrivener's error nunc pro tune set forth
in Ordinance No. 3253.
(This Ordinance corrects a scrivener's error in Ordinance No. 3253 by using the word
inadmissible instead of admissible.
(The Codifier being fully aware and having been advised that the clear legislative intent of
the Village Board was to have the Notice of Violation remain admissible at the Hearing,
correctly codified that section of the Village Code with the word admissible.)
o. Consideration to adopt Resolution No. 41-16 authorizing the Mayor to execute an
Addendum to the Contract between the Village of Elk Grove and Andres Medical Billing,
Ltd.
(This Resolution approves an Addendum to the Contract between the Village and Andres
Medical Building,Ltd.by adding a Penalty Clause and a Liquidated Damages Clause to
the Contract.)
p. Consideration to adopt Resolution No.42-16(attached)authorizing the Mayor and Village
Clerk to execute an agreement between the Village of Elk Grove Village and the Suburban
Bus Division(PACE)of the Regional Transportation Authority(Dial-a-Ride).
(Estimated expenses for the Dial-a-Ride program have increased by 3.7%compared to

2016 for a total estimated cost of$329,738 in 2016.
(This year's Local Share Agreement includes a$7,500 reduction in the maximum Pace
contribution compared to 2016, for a maximum Pace contribution of$72,000 in 2017.
(Aside from the decrease in the Pace Contribution,the terms of this year's Agreement have
not been significantly modified in comparison to previous years.)
q. Consideration to adopt Resolution No.43-16(attached) authorizing the Maintenance of
Streets and Highways by a Municipality under the Illinois Highway Code and
appropriating$1,330,800 from the Motor Fuel Tax Funds (MFT)for the 2017 Street
Maintenance Program.
(The Capital Improvements Committee approved staff recommendations for the
2017 MFT Street Maintenance Program.
(Approval of the Resolution appropriating Motor Fuel Tax(MFT)funds in the amount of
$1,330,800 to be utilized for the 2017 MFT Street Maintenance Program.
(Upon concurrence by the Board,staff will prepare bid documents and advertise for
bids the 2017 MFT Street Maintenance Program in February.
(The Director of Public Works recommends approval.)
r. Consideration of the following:
• to adopt Resolution No.44-16 authorizing the Mayor to execute a Joint Agreement
between the Illinois Department of Transportation(IDOT) and the Village of Elk
Grove Village, and appropriate funds for construction engineering and construction
match for the resurfacing of Lively Boulevard;and
• to award a professional service contract to Civiltech Engineering of Itasca, IL for
the construction engineering services for the resurfacing project on Lively
Boulevard in the amount of$87,546 from the BLF Fund.
(The Village secured Federal funding for the construction and construction engineering for
the resurfacing of Lively Boulevard from Landmeier Road to Higgins Road.
(Attached is a copy of the Joint Agreement for the Lively Boulevard resurfacing project to
be executed by and between the Illinois Department of Transportation(IDOT) and the
Village of Elk Grove Village.
(The Agreement provides funding for construction and construction engineering for the
proposed roadway construction project.
(A resolution appropriating funds for the full construction engineering and the 20 percent
construction match($258,685 total)must be adopted by the Village as a function of the
execution of this Agreement.
(Civiltech Engineering, Inc. submitted a proposal to provide the necessary construction
engineering services for this project.
(The Village will be responsible for the payment of construction engineering costs and the
State will reimburse the Village for 80%of the total construction engineering costs.
(Adequate funds are available in the BLF Fund.
(The Director of Public Works recommends approval.)
s. Consideration to adopt Resolution No. 45-16 (attached)authorizing the Mayor to execute a

Letter of Receipt stipulating the terms of an agreement for an application seeking a Cook
County Class 6B Property Tax Exemption status pursuant to the Cook County Real
Property Classification Ordinance as amended February 15,2006 for certain real estate
located at 1001 Morse Avenue.
(The Law Offices of Sarnoff&Baccash,on behalf of Wolf Family Enterprises LLC
(Applicant),is seeking a Cook County Class 6B Property Tax Exemption for 1001 Morse
Avenue.
(The Applicant intends to purchase the building and lease it to a related entity called
Chicago Wheel Services Inc.
(Chicago Wheel is a family owned business currently leasing 26,000 square feet of space
at 855 Bonnie Lane in Elk Grove Village. Due to business expansion Chicago Wheel will
occupy the entire building for its operations consisting of aluminum wheel manufacturing,
distribution, and warehousing. They will move their staff of thirty-five to the new building
and look to hire an additional three to five employees.
(The subject property consists of a 37,746 square foot building that is vacant and unused.
(The Applicant intends on spending approximately$434,900 to refurbish the building.
Including: replace drive and dock aprons to Village standards; install new fagade EIFS
with awning,and illuminated sign;replace roof; clean fagade; office buildout;clean and
maintain exterior drainage ditch.
(The goal of Class 6B is to attract new industry,stimulate expansion and retention of
existing industry and increase employment opportunities. The eligibility requirements for
6B status are new construction, substantial renovation or buildings that have been vacant
for a period of time.
(This site qualifies as it involves occupation of an abandoned building for less than twenty-
four(24)months continuous vacancy with a purchase for value, special circumstances and
significant rehabilitation.
(Property approved for Class 6B status allows the owner of the property to have the
assessment level lowered for a period of twelve years.The assessment schedule is 10%of
fair market value for ten years then 15%in the eleventh year and 20%in the twelfth
year. Industrial property is currently assessed at 25%of fair market value in Cook County.
(The Director of Business Development and Marketing recommends issuing a Letter of
Receipt.)
t. Consideration to adopt Resolution No. 46-16(attached)authorizing the Mayor to execute a
Letter of Receipt stipulating the terms of an agreement for an application seeking a Cook
County Class 6B Property Tax Exemption status pursuant to the Cook County Real
Property Classification Ordinance as amended February 15, 2006 for certain real estate
located at 2101 Arthur Avenue.
(The Law Offices of Liston&Tsantilis,on behalf of their client Seth Trust D(Applicant)
is seeking a Cook County Class 6B property tax exemption for property located at 2101
Arthur Avenue.
(The subject property consists of an 82,500 square foot building. The building has been
vacant since 2007 and is 51 years old.
(The Applicant intends to significantly to rehabilitate the existing building for a total cost

of approximately $1.5 million. Improvements include: full site detention;complete favade
renovation;installation of ten off street loading docks; complete landscaping renovation;
all new drive surfaces and aprons;clearing of rear drainage ditch, installation of
underground utilities; and building interior renovation.
(Upon completion of all improvements the Applicant will lease the building to an
industrial user.
(The goal of Class 6B is to attract new industry,stimulate expansion and retention of
existing industry and increase employment opportunities. The eligibility requirements for
6B status are new construction, substantial renovation or buildings that have been vacant
for a period of time.
(This site qualifies as it involves occupation of abandoned property for greater than
twenty-four(24)months without a purchase for value,with significant rehabilitation,and
special circumstances.
(Property approved for Class 6B status allows the owner of the property to have the
assessment level lowered for a period of twelve years. The assessment schedule is 10%of
fair market value for ten years then 15%in the eleventh year and 20%in the twelfth

u. Consideration to waive formal bidding procedures and award a purchase contract to
Energenecs, Inc. of Cedarburg, WI to design & furnish a pump station control panel and
integrate the Landmeier Pond Pump Station into the Storm Water SCADA in the amount
of$35,700.00 from the Water& Sewer Fund.
(The Village requested Energenecs to design a control panel for a duplex pump station and
perform integration services into the Storm Water SCADA system. Energenecs is the
successor firm of the Village's SCADA consultants and has performed similar work to the
Village's satisfaction in the past.
(Adequate funds are available in the Water& Sewer Fund for the expenses to complete the
necessary public improvements.

v. Consideration to adopt Resolution No. 47-16 (attached) authorizing the Mayor to execute a
Letter of Receipt stipulating the terms of an agreement for an application seeking a Cook
County Class 613 Property Tax Exemption status pursuant to the Cook County Real
Property Classification Ordinance as amended February 15, 2006 for certain real estate
located at 2080 Lunt Avenue.
(The Law Offices of Liston& Tsantilis, on behalf of their client CH Realty VII, LLC,
(Applicant) is seeking a Cook County Class 613 property tax exemption for property
located at 2080 Lunt Avenue.
(The subject property consists of an approximately 299,815 square foot site. The Applicant
intends to tear down the existing building and construct a new 107,950 square foot
industrial building. The Applicant expects the cost to construct the new building to be
approximately$10,000,000.00.
(The goal of Class 613 is to attract new industry, stimulate expansion and retention of
existing industry and increase employment opportunities. The eligibility requirements for
613 status are new construction, substantial renovation or buildings that have been vacant
for a period of time.

Property Classification Ordinance as amended February 15, 2006 for certain real estate
located at 1055 Arthur Avenue.
(The Law Offices of Liston & Tsantilis, on behalf of their client 1055 Arthur LLC,
(Applicant)doing business as Maman Corporation is seeking a Cook County Class
6B property tax exemption for property located at 1055 Arthur Avenue.
(Maman Corporation is a leading general contractor, construction management, and
carpentry construction firm. Maman will be relocating its entire operation from Palatine
including its forty-five employees.
(The subject property consists of an approximately 10,000 square foot building on an
approximately 25,450 square foot site. The Applicant intends to spend approximately
$481,000 to $640,000 to refurbish the building including: new parking lot and drive
aprons constructed to Village standards; new dock drive and aprons constructed to Village
standards; new office interior; new windows and fagade tuck-pointing, new facade
elements, landscaping, and signage.
(The goal of Class 6B is to attract new industry, stimulate expansion and retention of
existing industry and increase employment opportunities. The eligibility requirements for
6B status are new construction, substantial renovation or buildings that have been vacant
for a period of time.
(This site qualifies as it involves occupation of an abandoned building with less than
twenty four months continuous vacancy with a purchase for value and significant
rehabilitation.
(Property approved for Class 6B status allows the owner of the property to have the
assessment level lowered for a period of twelve years. The assessment schedule is 10%of
fair market value for ten years then 15% in the eleventh year and 20% in the twelfth year.
Industrial property is currently assessed at 25%of fair market value in Cook County.
(The Director of Business Development and Marketing recommends issuing a Letter of

x. Consideration to adopt Resolution No. 49-16 (attached) authorizing the Mayor and Village
Clerk to execute a settlement agreement between the Regional Transportation Authority,
the County of Cook,the Village of Elk Grove Village, et al and the City of Kankakee, the
Village of Channahon and related entities Inspired Development LLC and Ryan, LLC.
(In 2011, the Village joined in the lawsuit filed by the RTA against the City of Kankakee
and the Village of Channahon and various sales tax brokers and retailers claiming that the
defendants had conspired to improperly source sales of goods outside of the plaintiff's
jurisdiction to avoid local and regional sales taxes.)
y. Consideration to adopt Resolution No. 50-16, a Resolution rescinding Resolution No. 20-
15 and approving a settlement agreement with the International Union of Operating

Engineers, Local 150, AFL-CIO.
z. Consideration to award a professional service contract to Entertainment Management
Group of Elk Grove Village, 1L to manage and operate the 2017 Mid-Summer Classics
Concert Series in an amount not to exceed $355,000, of which $50,000 will be reimbursed
back to the Village by the Elk Grove Park District.
(As in years past, the Elk Grove Park District will be co-sponsoring the event, "Unity
Within The Community," and sharing in the overall contract expenses with the Village. As
such, the Park District will be reimbursing the Village $50,000.
(The concert series is funded by the I%Hotel/Motel Tax, Video Gaming revenue, Tobacco
License fees, a$50,000 donation by the Elk Grove Park District, and sponsorship
donations.
(The ninth annual Mid-Summer Classics Concert Series will be held during the month of
July, with concerts taking place on Tuesday, July 4, Tuesday,July 11, Tuesday, July 18,
and Tuesday, July 25.)
