6. CONSENT AGENDA
a. Consideration of a request to renew a professional service contract with Lechner and Sons
Inc.of Mount Prospect, IL for the Uniform, Linen, Floor Mat Services Contract in the
amount$26,623.92 from the General Fund.
(On March 10, 2015,the Village Board awarded a contract to Lechner and Sons Inc.of
Mount Prospect, IL for the Uniform,Linen, Floor Mat Services contract.
(The contract provided for an option of three (3) additional renewals through April 30,
2020.
(Lechner and Sons has performed satisfactorily throughout the contract.
(The contract period is from May 1, 2018 through April 30,2019.
(The contract amount reflects a 1.7% increase over the previous year's contract.
(The Director of Public. Works recommends approval.)
b. Consideration of a request to renew a custodial service contract with Crystal Maintenance
Services of Mount Prospect, IL for the annual Cleaning and Custodial Service Contract in
the amount of$32,892.89 from the General Fund.
(On March 24,2015,the Village Board awarded a contract to Crystal Maintenance
Services of Mount Prospect, IL for the Cleaning and Custodial Services contract.
(The contract provided for an option of four (4)additional renewals through

(Crystal Maintenance Services performed satisfactorily throughout the first year of the
contract.
(The contract period is from May 1, 2018. through April 30, 2019.
(The contract amount reflects a 1.7%increase over the previous year's contract.
(The Director of.Public Works recommends approval.)
c. Consideration of a request to renew a professional services contract with C.C. Carthage,
Inc. of Lake Zurich, IL to Furnish and.Deliver Sand and Gravel in the amount of
$37,971.81 from the Water-Sewer Fund.
(On May 24,2016,the Village Board awarded a contract with C.C. Carthage,Inc. of Lake
Zurich, IL to Furnish and Deliver Sand and Gravel.
(The contract provided for an option of four(4) additional renewals through April 30,
2021.
(C.C. Carthage performed satisfactorily throughout the first year of the contract.
(The contract period is from May 1, 2018 through April 30, 2019.
(The contract amount reflects a 1.7% increase over the previous year's contract.
(The Director of Public Works recommends approval.)
d. Consideration to award a purchase contract to Kunes Country Ford of Antioch,IL for the
purchase of(1) Ford Expedition in the amount of$41,899 from the Capital Replacement
Fund.
(In the FY18 budget, funds have been allocated to replace unit 155 due to excessive age.
(The 2018 Ford Expedition is available through the Suburban Purchasing Cooperative
(SPC)contract with Kunes Country Ford.
(Adequate funds are available in the Capital Replacement Fund.
(The Director of Public Works recommends approval.)
e. Consideration to award a professional service contract to Ayers Associates Inc. of
Madison, WI for aerial orthophotography and planimetric mapping updates as part of the
GIS Consortium purchasing agreement in the amount of$49,992 from the Water/Sewer
Fund.
(Elk Grove Village has been a member of the GIS Consortium since 2008.
(The GIS Consortium (GISC) is a group of local communities that have worked together to
develop a cost-effective solution for implementing geographic information systems(GIS)
and related technologies.
(One of those cost-effective solutions is to purchase aerial photography/mapping in bulk
through the Consortium's 34 municipalities.
(The GIS Consortium Board of Directors appointed a Base:Mapping Committee to initiate
a selection process for a photogram metric mapping consultant, and Ayers Associates was
the selected vendor.
(As part of the pricing agreement,Ayers Associates uses economies of scale to provide
their services at a reduced price.
(As part of this year's mapping program,Ayers Associates will produce updated aerial

imagery of the Village and perform planimetric imagery updates within the Elk Grove
Village community.
(The Assistant Village Manager recommends approval.)
f. Consideration of a request to renew a maintenance contract with Lyons &Pinner Electric
Company, Inc. of LaGrange, IL for the annual Residential Street Lighting Maintenance
Contract in the amount of$115,093.42 from the General Fund.
(On April 8,2014,the Village Board awarded a contract to Lyons&Pinner Electric
Company, Inc. of LaGrange, IL for the Residential Street Lighting Maintenance contract.
(The contract provided the option of four(4) annual renewals through April 30,2019.
(Lyons &Pinner Electric Company has performed satisfactorily throughout the contract.
(The contract period is from May 1, 2018 through April 30, 2019.
(The contract amount reflects a 1.7% increase over the previous year's contract.
(The Director of Public Works recommends approval.)
g. Consideration of a request to renew a maintenance contract with Lyons &Pinner Electric
Company, Inc. of LaGrange, IL for the annual Arterial &Business Park Street Light
Maintenance Contract in the amount of$146,143.38 from the BLP Fund.
(On April 11,2017,the Village Board awarded a contract to Lyons &Pinner Electric
Company, Inc. of LaGrange, IL for the Arterial &Business Park Street Light Maintenance
contract.
(The contract provided the option of four(4) annual renewals through April 30, 2022.
(Lyons &Pinner Electric Company has performed satisfactorily throughout the contract.
(The contract period is from May 1, 2018 through April 30, 2019.
(The contract amount reflects a 1.7%increase over the previous year's contract.
(The Director of Public Works recommends approval.)
h. Consideration of a request to renew a contract with Avalon Petroleum of Kankakee,IL for
the annual Gasoline and Diesel Fuel Supply Contract in an amount not to exceed
$474,000.
(On April 8, 2014,the Village Board awarded a contract to Avalon Petroleum of
Kankakee, IL for the annual Gasoline and Diesel Fuel Contract.
(The contract provided for an option of four(4) additional renewals through April 30,
2019.
(Avalon Petroleum performed satisfactorily throughout the contract.
(The contract period is from May 1, 2018 through April 30,2019.
(The contract amount reflects no economic adjustment over the previous year's contract.
(The Director of Public Works recommends approval.)
i. Consideration to award a professional service contract to the lowest responsive and
responsible bidder Swallow Construction Co., of Downers Grove, IL for the Maple Lane
Water Main Improvement project in the amount of$859,173 from the Water& Sewer
Fund.

(On Thursday,March 29, 2018,the Village opened sealed bids for the Maple Lane Drive
Water Main Improvements project.
(The project consists for the installation of approximately 2,200 linear feet of 12"and 220
linear feet of 8"ductile iron water main, installation of 7 new fire hydrants with valves,the
connection of 49 residential water services to the new main,HMA pavement and PCC
concrete restoration, and all incidental and collateral work necessary to complete the
project.
(A total of eleven(11)contractors obtained contract documents,with eight(8) submitting
bids on the project. `
(The lowest bid was received from Swallow Construction Co. of Downers Grove, IL in the
amount of$859,173.
(Adequate funds are available in the Water i Sewer Fund.
(The Director of Public Works recommends approval.)
j. Consideration to authorize the Village Manager to execute contracts renewing the Village's
health insurance programs as of May 1, 2018:
• To execute a contract with Blue Cross/Blue Shield to provide medical services
and to establish the Monthly Premium Rates for the Blue Cross/Blue Shield
Self-Insured Medical PPO as follows:

k. Consideration of the following:
• To award a construction contract to Kane County Excavating, Saint Charles,IL for
excavation and mass earthwork to be done as part of the Fire Station#8
Construction Project, in the amount of$323,190 from the Capital Projects Fund;
• To award a construction contract to Kane County Excavating, Saint Charles,IL for
site utility work to be done as part of the Fire Station#8 Construction Project, in
the amount of$397,890 from the Capital Projects Fund;
• To award a construction contract to Scott Steel of Crown Point, IN for structural
steel and iron work to be done as part of the Fire Station#8 Construction Project, in
the amount of$275,000 from the Capital Projects Fund;
• To award a construction contract to Abbey Paving of Aurora, IL for site concrete
work to be done as part of the Fire Station#8 Construction Project, in the amount
of$371,330 from the Capital Projects Fund;
• To award a construction contract to Abbey Paving of Aurora, IL for building
concrete work to be done as part of the Fire Station#8 Construction Project, in the
amount of$406,325 from the Capital Projects Fund;
• To award a construction contract to Alumital Corporation of Elk Grove Village, IL
for glass and aluminum work to be done as part of the Fire Station#8 Construction
Project, in the amount of$190,000 from the Capital Projects Fund;
• To award a construction contract to Metalmaster Roofmaster, Inc.of McHenry, IL
for roofing and sheet metal work to be done as part of the Fire Station#8
Construction Project, in the amount of$460,424 from the Capital Projects
Fund;and

• To award a construction contract to Hargrave Builders, Inc. of South Elgin, II, for
carpentry and general trade work to be done as part of the Fire Station#8
Construction Project, in the amount of$1,498,550 from the Capital Projects Fund;
(MTI is the Village's construction manager for the Fire Station#8 and#10 Construction
Projects.
(MTI solicited and received pre-qualified packages for various trade bidders for the Fire
Station 8 Construction Project.
(The lowest bidders were subsequently interviewed by MTI to verify that the scope of
work included in their bid proposals were in compliance with the plans, specifications, and
contract documents.
(The following were identified as the lowest responsive and responsible bidders: Kane
County Excavating, Scott Steel,Abbey Paving,Alumital Corporation,Metalmaster
Roofmaster, Inc. and Hargrave Builders,Inc.
(The Fire Chief recommends approval.)
1. Consideration to adopt Resolution No.19-18 amending Article IV, Section 4.4A Medical
and Dental Benefits Policy of the Village of Elk Grove Village Personnel Rules and
Regulations by adding thereto an Opt-Out Program.
(This Resolution adds the Opt-Out provision to the Village's Personnel Manual Section
4.4A Medical and Dental Benefits.Policy.)
m. Consideration to adopt Resolution No. 20-18 approving the Plat of Subdivision identified
as EG Township Plat of Subdivision for property located at 600 Landmeier to divide the
existing lot from one (1) lot to two(2) lots.
(This item was discussed at the Village Board Meeting on March 13, 2018 and currently
appears under Unfinished Business.)
n. Consideration to adopt Resolution No. 21-18 authorizing the Mayor to execute a Letter of
Receipt stipulating the terms of an agreement for an application seeking a Cook County
Class 613 Property Tax Exemption status pursuant to the Cook County Real Property
Classification Ordinance as amended February 15, 2006 for certain real estate located at
1225 Arthur Avenue.
(The Law Offices of Sarnoff&Baccash,on behalf of A&A Partnership,LLC, (Applicant)
is seeking a Cook County Class 6B property tax exemption for 1225 Arthur Avenue.
(The Applicant intends to purchase the vacant property and use it for relocating a recently
acquired competitor from Skokie, as well as expand its own operation. Primedge operation
consists of sharpening knives, blades, and other cutting/slicing materials used in the food
service operation.
(The Applicant currently operates in a facility located directly adjacent to the subject
property(1281 Arthur) in Elk Grove Village.As part of the acquisition,Primedge will
occupy the 24,000 square foot building, with will 18 of the competitor's employees
coming to the new facility, as well as 6 of their own existing employees. Primedge

believes that they will add approximately 10 new employees in the next several years.
(The Applicant intends on investing$270,000 improve the building. Improvements
include; facade, landscaping, signage improvements, new entrance awning, loading dock
upgrades,parking lot resurfacing, new parking area, and interior build out.
(The eligibility requirements for 6B status are new construction, substantial renovation or
buildings that have been vacant for a period of time.
(This site qualifies as it involves vacancy for less than twenty-four(24)months with a
purchase for value, special circumstances, and significant rehabilitation.
(Property approved for Class 6B status allows the owner of the property to have the
assessment level lowered for a period of twelve years. The assessment schedule is 10%of
fair market value for ten years then 15% in the eleventh year and 20% in the twelfth
year. Industrial property is currently assessed at 25%of fair market value in Cook
County.
(Upon approval,the Director of Business Development and Marketing will issue a Letter
of Receipt to the Applicant. The Letter of Receipt will allow the Applicant to file an
application with Cook County.Final approval is at the discretion of the Village Board and
contingent with the Applicant completing the improvements stated in their application.
(The Deputy Village Manager recommends issuing a Letter of Receipt.)
o. Consideration to adopt Resolution No. 22-18 authorizing the Mayor to execute a Letter of
Receipt stipulating the terms of an agreement for an application seeking a Cook County
Class 6B Property Tax Exemption status pursuant to the Cook County Real Property
Classification Ordinance as amended February 15,2006 for certain real estate located at
1461 Lunt Avenue.
(The Law Offices of Sarnoff&Baccash, on behalf of Nick and Stacey Seoles, (Applicant)
is seeking a Cook County Class 6B property tax exemption for 1461 Lunt Avenue.
(Remodelers Mart Inc. currently operates in Hillside,Illinois. Due to business expansion
Remodelers will use the space for the light assembly,distribution and warehousing of
cabinetry.
(The subject property consists of an approximately 23,290 square foot building on an
approximately 45,240 square foot site.
The company will occupy approximately 7,000 square feet and lease out the remaining
space to other industrial users. The property has been 100%vacant since 2014 and is need
of major renovation.
(The Applicant intends on spending$400,000 to improve the building. Improvements
include; facade, landscaping and signage,parking lot and dock restoration, and interior
build out.
(The eligibility requirements for 6B status are new construction, substantial renovation or
buildings that have been vacant for a period of time.
(This site qualifies as it involves vacancy for more than twenty-four(24) months with a
purchase for value,and significant rehabilitation.
