Consideration of a request from AMITA Health Alexian Brothers Medical Center to waive permit fees in the amount of $18,018.00 for the expansion of the Cardiac Cath Laboratory at 800 BiesterfieId Road. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.) b. Consideration of a request from Living Hope Church, 777 Meacham Road, to waive permit fees for parking lot maintenance in the amount of $412.00. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.) c. Consideration of a request from the Elk Grove Park District to waive permit fees in the amount of $634.00 for parking lot repairs at Audubon Park, 300 East Elk Grove Boulevard. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.) Pagel of 6 d. Consideration of a request from the Elk Grove Park District to waive permit fees in the amount of $315.00 for asphalt walking path repairs at the Pavilion, 1000 Wellington Avenue. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.) e. Consideration of a request from the Elk Grove Park District to waive permit fees in the amount of $227.00 for parking lot maintenance at the Administration Building, 499 Biesterfield Road. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.) f. Consideration of a request from the Elk Grove Park District to waive permit fees in the amount of $25.00 for parking lot maintenance at Lions Park, 180 Lions Drive. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.) g. Consideration of a request from the Elk Grove Park District, located at 499 Biesterfield Road, to waive FY 2018/19 vehicle sticker fees in the amount of $2,200. (The Elk Grove Park District is seeking a waiver of FY 2017/18 vehicle sticker fees for 5 passenger vehicles ($125), 12 Class-B trucks ($600), 17 Class-D/F trucks ($1,275), and 2 Class-H/J/K trucks ($200) for a total waiver of $2,200. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Finance recommends approval.) h. Consideration of a request from the Fraternal Order of Police Elk Grove Village Lodge No. 35, to waive FY 2018-19 business license fees in the amount of $50. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Finance recommends approval.) i. Consideration of a request from the Rotary Club of Elk Grove Village for the following: • to authorize a $15,000 contribution to the 2018 RotaryFest event that takes place from June 13, 2018 through June 17, 2018; • to authorize the Rotary Club to host a fireworks display on Saturday, June 16, 2018; • to waive the fee for a temporary Class A liquor license ($2,900), a carnival license ($500), and a food caterer's license fee ($100) for a total amount of $3,500; and • to waive the permit fee'for the use of a fire hydrant for non -firefighting purposes in Page 2 of 6 the amount of $75. (The fees requested to be waived are for the Class A liquor license ($2,900), a carnival license ($500), a food caterer's license ($100), and fire hydrant deposit and connection ($75). (It has been past practice of the Village Board to approve these requests for the annual RotaryFest event. The Village provides street sweeping services for the parking lots utilized by the Rotary Club at the conclusion of the RotaryFest. (Adequate funds have been budgeted and are available for this purpose.) j. Consideration to renew an annual purchase contract with On Time Embroidery, Inc. of Elk Grove Village, IL for department uniforms and accessories in the amount of $34,800 from the General Fund. (The Fire Department budgets annually for department uniforms and accessories. (A multi -year consolidated uniform contract was publicly bid in FYI 8. (This is the second year that the Fire Department is purchasing all of its uniforms from a single vendor. (The Fire Department had a successful experience with On Time Embroidery, Inc. of Elk Grove Village, IL under the initial contract period. (The contract provides for an initial one-year term with the option for four (4) renewals. (The term of the contract renewal is May 1, 2018 through April 30, 2019. (Adequate funds are budgeted for this contract in the FYI Fire Department General Fund Budget. (The Fire Chief recommends approval.) k. Consideration to award a purchase contract through the National Purchasing Partners (NPP) government purchasing cooperative Fire/Rescue GPO contract to Municipal Emergency Services (MES) of Deer Creek, IL for the purchase of thirty-four (34) sets of custom turnout gear in the amount of $78,200 from the General Fund. (The Fire Department budgets annually to purchase custom turnout gear. (This is the second year that the Fire Department is exclusively purchasing Globe turnout gear. (Globe Manufacturing turnout gear is a leader in the protective clothing industry and offers a wide variety of custom options. (Municipal Emergency Services, Inc. (MES) of Deer Park, IL is a local Globe distributor and holds a joint purchasing contract through National Purchasing Partners (NPP) government purchasing cooperative Fire/Rescue Government Purchasing Organization. (Fire Department personnel are extremely pleased with the quality of the turnout gear that Globe has produced and delivered during the most recent fiscal year. (Adequate funds are available in the Fire Department General Fund budget. (The Fire Chief recommends approval.) 1. Consideration to award a construction contract to the lowest responsive and responsible bidder Acqua Contractors Corporation of Villa Park, IL for the Storm Sewer Improvement - Beisner R.O.W. in the amount of $601,350.00 from the Water/Sewer Fund. Page 3 of 6 (On Thursday, April 26, 2018 the Village opened sealed bids for the Storm Sewer Improvement — Beisner R.O.W. project. (Eight (8) contractors obtained contract documents, with five (5) submitting bids on the project. (The project includes the replacement of approximately 1,759 lineal feet of 24"-54" diameter corrugated metal pipe with replacement of 54" reinforced concrete pipe, and regrading of the Beisner R.O.W. and along the rear yards of Cheltenham Road properties to maintain overflow route south to Wellington Avenue, and the modification to the existing storm water lift -station located within the R.O.W. (The work also includes pavement, sidewalk and curb removal and replacement, and landscaping restoration. (The lowest responsive and responsible bid was received from Acqua Contractors Corporation of Villa Park, Illinois. (The Director of Public Works has recommended approval.) in. Consideration to award a professional service contract with HR Green, Inc. of McHenry, IL to provide the necessary construction engineering services for the Storm Sewer Improvement - Beisner R.O.W. project in the amount of $78,329.26 from the Water/Sewer Fund. (HR Green, Inc. submitted a proposal to provide the necessary construction engineering services for the Storm Sewer Improvement — Beisner R.O.W. project. (The project includes the replacement of approximately 1,759 lineal feet of 24"-54" diameter corrugated metal pipe with 54" reinforced concrete pipe, and regrading of the Beisner R.O.W. along the rear yards of Cheltenham Road properties to maintain overflow route south to Wellington Avenue and modification to the existing storm water lift -station located within the R.O.W. Consideration to adopt Ordinance No. 3552 granting a variation of Section 3-7:D(3) of the Zoning Ordinance to permit the construction of a six foot (6) high wood fence extending approximately forty six feet (46') beyond a line extended from the nearest front corner of the principal building on an adjacent single-family residential lot with one section running parallel with the sidewalk for property located at 1450 James Way, Elk Grove Village. (This item was previously discussed at the April 24, 2018 Village Board Meeting and currently appears as Unfinished Business under the Village Attorney.) o. Consideration to adopt Ordinance No. 3553 granting an Administrative Special Use to the Village to permit the construction and location in several zoned districts as well as related variations with respect to signage, storage, heights, setbacks, detached accessory structures, fencing and principal structures for the Meacham Fire Station, Fargo Fire Station and the new Devon Avenue Public Works Facility. (The Village is in the process of reconstructing and relocating Fire Stations and Public Works facilities as part of the 2025 Redevelopment Plan; Page 4 of 6 (This ordinance authorizes the Village as a Home Rule Municipality to enact legislation for variations of the Zoning Ordinance and Municipal Code with respect to the height, size and location of the buildings as well as the regulation of the use of open space surrounding such buildings.) 7.
