ACCOUNTS PAYABLE WARRANT: May 31, 2018 $2,623,585.51 June 19, 2018 $4,316,824.87 6. CONSENT AGENDA a. Consideration of a request to waive permit fees for a roof replacement as part of the anonymous roof donation. (The roof was donated by an anonymous roofing contractor and selection of the roof recipient was coordinated by the Community Character Counts Roof Giveaway Program.) (The Director of Community Development recommends approval.) b. Consideration of a request from Northwest Presbyterian Church, located at 301 Ridge Avenue, to waive FY 2018/19 Vehicle License fees in the amount of $50. (Northwest Presbyterian Church is requesting a fee waiver for FY 2018/19 Vehicle License fees for two passenger vehicles ($25 each) for a total waiver of $50. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Finance recommends approval.) c. Consideration to concur with prior authorization of a request from Elk Grove Park District, located at 1000 Wellington Avenue, to waive the FY 2018-19 business license fee for the June 10, 2018 Art in the Park event in the amount of $135. (The Elk Grove Park District is respectfully requesting their Business License fees in the amount of $135 to be waived for a Temporary General Retail License and Food Caterers Pagel of 9 License for the June 10, 2018 Art in the Park event. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations.) d. Consideration of a request from the Elk Grove Park District to waive permit fees in the amount of $300.00 for parking lot patching at 750 Elk Grove Boulevard. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.) e. Consideration of a request from Queen of The Rosary Parish to waive permit fees in the amount of $2,244.00 to mill and overlay asphalt at 750 Elk Grove Boulevard. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.) f. Consideration of a request from Amita Health to waive permit fees in the amount of $5,615.75 for expansion of the Alexian Archives Museum at 600 Alexian Way. (It has been past practice of the Village Board to grant fee waivers for governmental and non-profit organizations. (The Director of Community Development recommends approval.) g. Consideration to award a professional service contract to Engineering Resource Associates of Warrenville, IL for Phase II Engineering Services for two separate water main replacement projects: 1.) Willow Lane (Germaine Place to Wildwood Road) and 2.) Clearmont Drive (Carswell Avenue to Tonne Road) in the amount of $45,663.87 from the Water and Sewer Fund. (A proposal was solicited from Engineering Resource Associates of Warrenville, IL for Phase II Engineering Services to develop contract plans and specifications for two water main replacement projects: 1.) Willow Lane (Germaine Place to Wildwood Road) and 2.) Clearmont Drive (Carswell Avenue to Tonne Road). (Engineering Resource Associates have successfully completed design and construction engineering services on a number of water main projects for us over the past year. (It is recommended that a professional Phase II Engineering Service contract be executed with Engineering Resource Associates for two water main replacement projects: 1.) Willow Lane (Germaine Place to Wildwood Road) and 2.) Clearmont Drive (Carswell Avenue to Tonne Road) in the amount of $45,663.87. (Adequate funds are available in the Water and Sewer Fund. (The Director of Public Works has recommended approval.) h. Consideration of a request to renew a professional service contract to Temple Display, LTD of Oswego, IL for the Holiday Lighting and Decoration Services contract in the amount of $55,791.52 from the General Fund. (On September 12, 2017, the Village Board awarded a contract to Temple Display, Ltd. of Page 2 of 9 Oswego, IL for the Holiday Lighting and Decoration Services contract. (The contract provided for an option of four (4) additional renewals through January 31, 2022. (Temple Display, Ltd. has performed satisfactorily throughout the first year of the contract. (The contract period is from October 1, 2018 through January 31, 2019. (The contract amount reflects 2.1 % increase over the previous year's contract. (The Director of Public Works recommends approval.) Consideration to award a professional service contract to HR Green, Inc. of McHenry, IL for Phase I and Phase II Engineering Services for the Nerge Road (Rohlwing Road to Devon Avenue) Resurfacing Project in the amount of $60,772.90 from the Capital Project Fund. (The Village requested a proposal from HR Green, Inc. to provide Phase I and Phase II Engineering Services for the Nerge Road (Rohlwing Road to Devon Avenue) Resurfacing Project. (HR Green submitted a proposal in the amount of $60,772.90. (These services will assist the Village in preparation for receiving Federal funding for construction and construction engineering for this project. (The improvements consist of resurfacing and pavement patching Nerge Road from Rohlwing Road to Devon Avenue, ADA intersection improvements, pedestrian signal improvements at the intersection of Rohlwing Road at Nerge Road and Devon Avenue at Nerge Road, along with decorative crosswalk improvements. (The Director of Public Works recommends approval.) j. Consideration to award a professional service contract to Rebechini Studios, Inc. of Elk Grove Village, IL to manufacture a LED illuminated sign in the amount of $74,072 from the Business Leaders Fund. (Rebechini Studios, Inc., of Elk Grove Village, IL has provided the Village a quote to fabricate and install the internal LED illuminated sign in an amount not to exceed $74,072. (The sign is to be located at I-90 and Arlington Heights Road. (Adequate funds are available in the Business Leaders Fund. (The Director of Public Works has recommended approval.) k. Consideration to award a professional services contract to Patrick Engineering Inc. of Lisle, IL for design engineering and permitting of the LED Street Name Signs - Meacham and Rohlwing Roads Project in the amount of $81,791 from the Business Leaders Fund. (A proposal was solicited from Patrick Engineering Inc. of Lisle, IL for design engineering and permitting services to develop plans, specifications and permit applications for work along Meacham and Rohlwing Roads. (The project will consists of the replacement of the existing street name signs with LED illuminated street name signs at various intersections along Meacham and Rohlwing Roads. (Patrick Engineering has successfully completed design -engineering services for the State, County and other Municipalities. (Adequate funds are available in the Business Leaders Fund. Page 3 of 9 (The Director of Public Works has recommended approval.) 1. Consideration to award a professional service contract to Rebechini Studios, Inc. of Elk Grove Village, IL to manufacture and install LED illuminated facility signs, dedication plaques, and address and identification letters for Fire Stations #8 and #10 in an amount not to exceed $92,250 from the Capital Projects Fund. (Rebechini Studios, Inc., of Elk Grove Village, IL is the designer and manufacturer of custom Village facility signs. Rebechini Studios has provided the Village a quote to fabricate and install LED illuminated monument signs, dedication plaques, and address and identification letters. (The signs are to be located at Fire Station #8 at 700 Fargo and Fire Station # 10 at 676 Meacham. (Adequate funds are available in the Capital Projects Fund for this project. (The Fire Chief recommends approval.) in. Consideration to award a purchase contract off of two (2) separate national purchasing contracts to Forward Space of Oak Brook, IL to purchase, relocate, and install work stations and office furniture within the Municipal Complex and Biesterfield Fire Station in an amount not to exceed $139,330.20 from the Capital Projects and General Fund. (Over the past several years, the Village has implemented a series of organizational changes in an effort to right size Village operations to make them more efficient and effective in dealing with the public. (As part of those changes, various workstations need to be reconfigured. (In addition, a number of existing workstations have exceeded their life cycle and are in need of being replaced. (Some of the office furniture will be purchased off of the National IPA contract for local governments, and the Steelcase products will be purchased off of the NJPA government purchasing agreement. (Forward Space (formerly OEC) has been the supplier and installer for all of the existing workstations used throughout the Municipal Complex, and will be best suited to re- configure and reinstall the existing components. (Moreover, Forward Space has provided a satisfactory work product in all instances. (Adequate funds are available and have been budgeted for this purpose. (The Deputy Village Manager recommends approval.) n. Consideration to award a professional service contract to Civiltech Engineering of Itasca, IL for Phase II Engineering Services for the University Land Reconstruction (Plum Grove Road to Vermont Drive) rehabilitation project in the amount of $177,674.94 from the Capital Project Fund. (The Village requested a proposal from Civiltech Engineering, Inc. to provide Phase II Engineering Services for the University Lane (Plum Grove Road to Vermont Drive) Rehabilitation Project. (Civiltech Engineering submitted a proposal in the amount of $177,674.94. (Civiltech Engineering has successfully completed design and construction engineering for Page 4 of 9 numerous Village projects. (The Director of Public Works recommends approval.) o. Consideration to approve a balancing change order to HVAC contracts with Jensen's Heating & Plumbing, Inc. of Woodstock, IL for the addition of Plymovent vehicle exhaust system installation as part of the Fire Stations #8 and # 10 construction projects in the overall amount of $186,108 from the Capital Projects Fund. (MTI is the Village's construction manager for the Fire Stations #8 and #10 construction projects. (MTI solicited and received pre -qualified packages for the trade bidders, including HVAC contracts. (The lowest bidders were interviewed by MTI to verify that the scope of work included in their bid proposals were in compliance with the plans, specifications, and contract documents. (Jensen's Heating & Plumbing, Inc. of Woodstock, IL was awarded the HVAC contracts for both stations. (Since the awarding of the HVAC contracts, the original scope has expanded to include the installation of a vehicle exhaust system in each station, which is best coordinated through the approved HVAC contractor. (MTI, HR Green, and Village staff are in agreement with these alterations. (Adequate funds are available in the Capital Projects Fund. (The Fire Chief recommends approval.) p. Consideration to concur with prior authorization of a request from CBRE for an additional sign variation at the Elk Grove Technology Park. (A variation is necessary to allow signage in excess of forty five (45) square feet. (The proposed signage is needed to promote the development of the facility and attract tenants/buyers.) (This variation is an addendum to previously approved temporary signage package approved by the Board on March 27, 2018.) (The Director of Community Development recommends approval.) q. Consideration to award a professional service contract to the lowest responsive and responsible bidder Insituform Technologies USA, LLC of Chesterfield, MO for Cured in Place Pipe Sanitary Sewer Rehabilitation Business Park Basin 15 project in the amount of $642,639.10 from the Water/Sewer Fund. (On Thursday, June 7, 2018, the Village opened sealed bids for the Cured in Place Pipe Sanitary Sewer Rehabilitation Business Park Basin 15 project. A total of five (5) contractors obtained contract documents, with four (4) submitting bids on the project. (The lowest responsive and responsible bid was received from Insituform Technologies USA, LLC of Chesterfield, MO in the amount of $642,639.10. (The project includes the installation of 19,400 feet of CIPP Liner 6 inch, 8 inch, 10 inch, and 12 inch sanitary sewer mains, the cleaning, preparation and collateral work necessary to complete the project. (The Director of Public Works recommends approval.) Page 5 of 9 r. Consideration of a request to hold a Public Hearing before the Plan Commission to consider a Petition submitted by North Star Foods for a Special Use Permit to operate a food processing facility in the I-2 Industrial District at 2575 American Lane. (The purpose of the Special Use Permit is to expand an existing food processing business into a new property. (A date for the Public Hearing has not been established.) s. Consideration of a request to hold a Public Hearing before the Plan Commission to consider the petition submitted by DARS Holding Group for a Special Use to operate an automotive repair center in the I-2 Industrial District at 850 Elmhurst. (DARS Holding Group is seeking a Special Use for the purpose of operating an automotive repair center at 850 Elmhurst. (A date for the Public Hearing has not been established.) t. Concurrence with prior authorization to award a contract to K-Five Construction Corp. of Westmont, IL for the Elk Grove Boulevard (Victoria Ln to Tonne Rd) Street Resurfacing project in the amount of $299,676 from the General Fund. (The Village is extending the resurfacing of Elk Grove Boulevard from Victoria Lane to Tonne Road with an additional contract amount to be awarded to K-Five Construction in the amount of $299,676. (This additional contract will extend the same unit prices from the original contract to cover the additional resurfacing area. (Appropriate funds for the project are available in the General Fund. (The Director of Public Works recommends approval.) u. Consideration to adopt Ordinance No. 3556 amending Section 6-4-5-2 of the Village Code by establishing parking restrictions adjacent to Adlai Stevenson School. (The Police Department was notified about a parking issue on Armstrong Lane near Adlai Stevenson School. During school hours Armstrong Lane, between the south property line of 1405 Armstrong Lane, two hundred forty-two feet (242') up to the west property line of 1400 Hodlmair Lane is a curved roadway and vehicles parked along the curve roadway do not allow school buses to safely transit the curved roadway.) (The Police Chief recommends parking restrictions be used during school hours to alleviate parking issues along curved roadway.) v. Consideration to adopt Ordinance No. 3557 granting a variation of Section 7E-9(A) of the Zoning Ordinance to permit the construction of an eight foot (8') tall, black wrought iron fence along the perimeter of property with one section extending approximately one hundred and ten feet (110') ahead of the building line along Nicholas Boulevard for property located at 2080 Lunt Avenue, Elk Grove Village. (This item was previously discussed at the May 22, 2018 Village Board Meeting and currently appears as Unfinished Business under the Village Attorney.) w. Consideration to adopt Ordinance No. 3558 amending Title 3, Chapter 3, Section 3-3-7, Number of Licenses of the Village Code of the Village of Elk Grove Village by increasing Page 6 of 9 the Number of Class "AA" Liquor Licenses. (Dos Diablos) (This Ordinance increases the Class "AA" Liquor Licenses from three (3) to four (4).) x. Consideration to adopt Resolution No. 28-18 approving the Plat of Subdivision identified as I I I I Chase Subdivision. (A petition was submitted by CP Logistics to resubdivide the property at 11 I I Chase and 1201 Chase Avenue from the existing two (2) lots into one (1) lot. (This item was discussed at the JPZ Committee Meeting on May 22, 2018 and the JPZ Committee unanimously recommends approval.) y. Consideration to adopt Resolution No. 29-18 ascertaining the Prevailing Rate of Wages for Workmen on Public Works projects. (The State of Illinois requires the Village to adopt this Resolution each year to determine the prevailing rate of wages for laborers performing construction of Public Works projects. (The rates are determined by the Illinois Department of Labor throughout Cook and DuPage Counties.) z. Consideration to adopt Resolution No. 30-18 approving an amended Mutual Aid Agreement for the Northern Illinois Police Alarm System (NIPAS). (The Elk Grove Village Police Department is a member of the Northern Illinois Police Alarm System (NIPAS) which provides essential mutual aid support to the Village during times when a police emergency exceeds our capability. (The NIPAS Board has asked each participating member to seek approval from its corporate authorities to update our NIPAS Agreement. The approval would take the form of adopting Resolution approving continued participation in NIPAS under the Amended Agreement.) aa. Consideration to adopt Resolution No. 31-18 authorizing the Mayor and Village Clerk to execute an Intergovernmental Agreement between the Village of Elk Grove Village and the Forest Preserve District of Cook County for trail connections in Busse Woods. (The agreement provides for the installation, maintenance and repair of the recreational trail connections from Northwest Point Boulevard and Oakton Street to the existing recreational trail in Busse Woods. (The agreement includes payment from the Village in the amount of $6,485.76 for removal of five (5) trees required to construct these improvements.
