ACCOUNTS PAYABLE WARRANT: MARCH 22, 2016 $480,142.86 6. CONSENT AGENDA a. Consideration of a request from Elk Grove Park District to waive 2016 Alarm License Fees in the amount of$175. (Consideration of a request from Elk Grove Park District to waive 2016 Alarm License Fees in the amount of$175. (It has been past practice of the Village Board to grant fee waivers for govenunental and non-profit organizations. (The Director of Finance recommends approval.) b. Consideration of a request from the Elk Grove Park District (attached) to waive the Liquor License fees for Fox Run Golf Links and Garden Terrace Banquets in the amount of $5,800. (Each Class A Liquor License is valued at$2,900. (It has been past practice of the Village Board to waive fees for governmental and not-for- profit agencies.) c. Consideration to award a professional services contract to Entertainment Management Group of Elk Grove Village, IL to manage and operate a concert as part of the Village's annual Holiday Tree Lighting Ceremony in an amount not to exceed $20,000. Page 1 of 5 (Entertainment Management Group has managed and operated the Village's annual Mid- Summer Classics Concert Series for the past eight years—with the 2016 concert series being the ninth year. (This is the second time that the Village will be holding a concert as part of its annual Holiday Tree Lighting Ceremony. (The concert will be funded by the 1% Hotel/Motel Tax, Video Gaming revenue, Tobacco License fees, and sponsorship donations. (The event will take place on Friday, November 25.) d. Consideration of a request to renew a custodial service contract with Crystal Maintenance Services of Mount Prospect, IL for the annual Cleaning and Custodial Service Contract in the amount of$35,760 from the General Fund. (On March 24, 2015, the Village Board awarded a contract to Crystal Maintenance Services of Mount Prospect, IL for the 2015 Cleaning and Custodial Services contract. (The contract provided for an option of four (4) additional renewals through April 30, 2020. (Crystal Maintenance Services performed satisfactorily throughout the first year of the contract. (The contract period is from May 1, 2016 through April 30, 2017. (The contract amount reflects no economic adjustment over the previous year's contract. (The Director of Public Works recommends approval.) e. Consideration of a request to renew a contract with Dynamic Heating & Piping Company of Crestwood, IL for the annual HVAC Maintenance Services contract in the amount of $81,649.70. (On May 27, 2014, the Village Board awarded a contract to Dynamic Heating & Piping Company of Crestwood, IL for the HVAC Maintenance Services contract. (The contract provided for an option of four (4) additional renewals through April 30, 2019. 1 (Dynamic Heating & Piping has performed satisfactorily throughout the contract. (The contract period is from May 1, 2016 through April 30, 2017. (The contract amount reflects no economic adjustment over the previous year's contract. (The Director of Public Works recommends approval.) f. Consideration of a request to renew a maintenance contract with Lyons & Pinner Electric Company, Inc. of LaGrange, IL for the annual Residential Street Lighting Maintenance Contract in the amount of$111,635 from the General Fund. (On April 8, 2014 the Village Board awarded a contract to Lyons & Pinner Electric Company, Inc. of LaGrange, IL for the Residential Street Lighting Maintenance contract. (The contract provided the option of four (4) amoral renewals through April 30, 2017. (Lyons & Pinner Electric Company has performed satisfactorily throughout the contract. (The contract period is from May 1, 2016 through April 30, 2017. (The contract amount reflects no economic adjustment over the previous year's contract. Page 2 of 5 (The Director of Public Works recommends approval.) g. Consideration of a request to renew a maintenance contract with Lyons & Pinner Electric Company, Inc. of LaGrange, IL for annual Arterial and Business Park Lighting Maintenance in the amount of$131,835 from the BLF Fund. (On April 10, 2012 the Village Board awarded a contract to Lyons & Pinner Electric Company, Inc. of LaGrange, IL for the Arterial and Business Park Maintenance contract. (The contract provided the option of four(4) annual renewals through April 30, 2017. (Lyons & Pinner Electric Company, Inc. has performed satisfactorily throughout the contract. (The contract period is May 1, 2016 through April 30, 2017. (The contract amount reflects no economic adjustment over the previous year's contract. (The Director of Public Works recommends approval.) It. Consideration to commit to the purchase of Bulk Rock Salt through the State of Illinois Joint Purchase Contract for the purchase of 3,000 tons of Bulk Rock Salt for the 2016-17 winter season at a maximum cost of$72.28 per ton to be paid from the FY 2017 Budget in an amount not to exceed $266,250. (Illinois Central Management Services (CMS) is currently preparing bids for the FY 2017 Road Salt Joint Purchasing Program. (The Village's current contract with CMS allows for the renewal of the contract for next winter at a cost not to exceed ten percent(10%) over our current cost of$65.71 per ton. (The CMS contract allows for the purchase of 80 to 120 percent of the designated tonnage. (The Village intends to order 3,000 tons of Bulk Rock Salt in FY 2017. (Adequate fronds are budgeted in the FY 2017 proposed budget. (The Director of Public Works recommends approval.) i. Consideration of a request to renew a contract with Avalon Petroleum of Kankakee, IL for the annual Gasoline and Diesel Fuel Supply Contract in an amount not to exceed $474,000. (On March 31, 2014, the Village Board awarded a contract to Avalon Petroleum of Kankakee, IL for the annual Gasoline and Diesel Fuel Contract. (The contract provided for an option of four (4) additional renewals through April 30, 2019. (Avalon Petroleum performed satisfactorily throughout the contract. (The contract period is from May 1, 2016 through April 30, 2017. (The contract amount reflects no economic adjustment over the previous year's contract. (The Director of Public Works recommends approval.) j. Consideration to adopt Ordinance No. 3448 which amends Section 6-4-5-3 of the Village Code concerning prohibited and restricted parking areas by amending the parking restrictions on Cosman Road. (The Forest Preserve District of Cook County expressed concerns to the Village that Page 3 of 5 people using the preserve are parking to close to the curve on Cosman Road, making it Unsafe. (This Ordinance amends the parking restrictions on Cosman Road, extending the parking restrictions to include the entire north side of Cosman Road.) k. Consideration to adopt the following Resolutions: • Resolution No. 8-16 approving the Plat of Vacation; and • Resolution No. 9-16 approving the Plat of Resubdivision identified as Bubba Acquisitions Plat of Resubdivision (2750 Touhy Avenue). (Chis item was discussed at the JPZ Committee Meeting on March 8, 2016. (The purpose of the resubdivision is to consolidate four (4) separate lots and a vacated alley into one (1) lot for the development of a Dunkin' Donuts restaurant. (The Plan Commission previously approved a rezoning of the property at 2750 Touhy Avenue from I-I to B-3 for the purpose of operating a drive-through at the proposed Dunkin' Donuts.
